Type of visit: Required - 1 Year

Unannounced Annual visit made out to this facility on 02/09/2026 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility caregivers, Jhonaleen Monreal and Rumel Valdez, who were briefly interviewed at this time. This LPA requested that they go ahead and contact the facility designated Administrator, Claire Rabang, to inform her that CCL was present at this time. The facility designated Administrator, Claire Rabang, arrived shortly thereafter to this facility. Current census was 5 residents. It was learned that there weren't any residents under the care of hospice at this time. This facility does not have an approved waiver to be able to accept and retain any residents under the care of hospice at any given time. It was learned that this facility has a program to be able to accept and retain dementia residents at any given time. It was learned that there weren't any resident diagnosed with dementia at this time. It was learned that there weren't any residents receiving services through home health at this time. It was learned that this facility does not have a bedridden fire clearance and unable to accept or retain any residents deemed to be bedridden at any given time. Tour of this facility was conducted. Dining area, living area, and all other areas intended for resident use were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Linen closet, located in facility hallway closet, was reviewed and observed to contain a sufficient supply of towels, sheets, and bedding able to meet the needs of the residents at this time. Kitchen area was toured. Kitchen drawers and cabinets were opened and reviewed. Food supply for 2-day perishable and 7-day nonperishable quantities was reviewed to make sure that they were in compliance at all times. Pantry area was toured. Garage area was toured. Laundry area, located in the garage area, was toured and observed to be locked at this time. A review of the bleach, detergent, and all other cleaning supplies was conducted to make sure that they were locked and made inaccessible to the residents at this time. Administrator certificate, # 7032721740, for Claire Rabang was observed to have an expiration date of 08/26/2026 and in compliance at this time. Medication cabinet, located in the facility office cabinets, was observed to be locked and made inaccessible to the residents at this time. First aid kit, located in the medication cabinet, was reviewed. This LPA observed that it did contain all of the required components at this time. Fire extinguisher located hanging on the wall adjacent to the kitchen was observed to have been annually inspected by the local fire extinguisher company, Jorgensen Fire, with the inspection date of 11/13/2025 and in compliance at this time. Facility resident bedrooms were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Facility resident restrooms were toured. Grab bars and non skid mats were observed to be present and in good repair at this time. Hot water temperatures were taken to make sure that they were within the allowed range of 105-120 degrees at all times. A tour of the facility exterior grounds was conducted. A review of the facility perimeter fence, side gates, and all other exits was conducted. A review of (5) facility personnel records was conducted and noted on the following LIC 859. A review of (5) facility resident records was conducted and noted on the following LIC 858. The following forms and documents were requested to be updated and submitted into CCL for review by this LPA: LIC 308 LIC 400 LIC 500 LIC 610 There were no deficiencies observed or cited during today's annual visit. Type of visit: Required - 1 Year

Licensing Program Analyst Jason Lund arrived unannounced to conduct an annual/required visit. LPA was met by facility staff and later with Administrator Claire Rabang and explained the reason for the visit. Census: 5 LPA Lund & Administrator Claire Rabang toured/inspected the facility and grounds including but limited to kitchen, dining area, office, common room, bathrooms, resident and staff bedrooms, garage, and yard. LPA observed that there was adequate furniture and lighting for the residents. There were 7-days of non-perishables and 2- days of perishables on hand. Knives were locked in a drawer adjacent to the sink. Toxic items were locked in a separate cabinet above the counter. The fire extinguisher was located in the kitchen/dining area and observed to have been recently inspected on 11/13/2024 by the local fire extinguisher company and in compliance at this time. Medications were locked and secured in a cabinet. Refrigerated medications were also locked in a small refrigerator in the office. The Medication Administration Record and dispensing log were reviewed and observed to be in compliance at this time. Grab bars and handrails were observed to be present and functional along with non-skid surfaces in the restrooms and shower area. Bedrooms were inspected and observed to be in good repair at this time. Resident bedrooms were observed to contain adequate furniture, including night stands and night lights, in order to meet the needs of the residents at this time. LPA toured the exterior grounds and observed the facility to be free and clear of obstacles. The garage was also inspected and had locked cabinets for cleaning supplies and other toxic items. LPA reviewed 3 resident and 2 staff records. Resident files were found to be complete and current. Facility is conducted staff training as required. No deficiencies observed or cited during visit. Allegation investigated: Client was sexually abused while in care

Licensing Program Analyst (LPA) Jason Lund arrived unannounced to complete a complaint investigation. LPA Lund met with Administrator Claire Rabang and explained the reason for the visit. Census: 5 Client was sexually abused while in care - Based on interviews conducted with Reporting Party (RP), witnesses and Client (C1). C1 lived at Lighthouse Manor from Mid-August 2020 through November 2022. C1 is currently living with family. C1 has had service coordinator with Valley Mountain Regional Center (VMRC) for over five years. VMRC staff would meet with C1 on a quarterly basis and C1 never stated any sexually abuse while living at Lighthouse Manor. Unsubstantiated Based on interviews conducted with Reporting Party (RP), witnesses and Client (C1) the information provided, it was unclear if facility client was sexually abused while in care therefore the allegation was deemed UNSUBSTANTIATED. The Department (CCLD) has found the allegations. Unsubstantiated. A finding that the complaint allegation(s) are UNSUBSTANTIATED means that although the allegation(s) may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation(s) occurred. Type of visit: Required - 1 Year

Licensing Program Analyst Jason Lund arrived unannounced to conduct an annual/required visit. LPA was met by facility staff and later with Administrator Claire Rabang and explained the reason for the visit. Census: 6 LPA Lund & Administrator Claire Rabang toured the facility and grounds including but limited to: kitchen, dining area, office, common room, bathrooms, resident and staff bedrooms, garage, and yard. There were no bodies of water on the premises. Tour of the facility kitchen area was conducted. LPA observed that there was adequate furniture and lighting for the residents. There were 7-days of non-perishables and 2- days of perishables on hand. Knives were locked in a drawer adjacent to the sink. Toxic items were locked in a separate cabinet above the counter. There were separate refrigerators for staff and residents. The fire extinguisher was located in the kitchen/dining area and observed to have been recently inspected on 01/18/2024 by the local fire extinguisher company, Jorgensen and Company, and in compliance at this time. This facility had an office located off the dining area. Medications were locked and secured in a cabinet. Refrigerated medications were also locked in a small refrigerator in the office. The Medication Administration Record and dispensing log were reviewed and observed to be in compliance at this time. The water temperature in one of resident bathrooms was tested and observed to be within safety guidelines of 105-120 degrees. The water temperature was measured at 112.4 degrees. Grab bars and handrails were observed to be present and functional along with non-skid surfaces in the restrooms and shower area. Bedrooms were inspected and observed to be in good repair at this time. Resident bedrooms were observed to contain adequate furniture, including nightstands and night lights, in order to meet the needs of the residents at this time. Shared rooms also included a divider for privacy. LPA toured the exterior grounds and observed the facility to be free and clear of obstacles. There was a locked storage shed and fruit trees present. The garage was also inspected and had locked cabinets for cleaning supplies and other toxic items. LPA reviewed 3 resident and 2 staff records. Resident files were found to be complete and current. A review of staff records indicates that all facility staff have received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current first aid certificates. Facility is conducted staff training as required. There were no deficiencies observed or cited during today's annual visit.
