Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. Upon arrival LPA observed the facility to be under construction with a "Do Not Enter" notice on the front door due to it being unsafe to occupy. LPA called Administrator (AD) Florence Tolentino and discussed the purpose of the visit. AD arrived at the facility within 5 minutes of LPAs phone call. AD reported to the Department that a fire had taken place in the garage on May 14, 2025 and all residents were moved out. LPA toured the outside of the facility and was able to look inside windows and the front door and observe that construction was taking place and residents were not residing in the facility. LPA confirmed that care is not being provided at this time. AD informed LPA that the process is going slow due to whenever something is finished they have to wait for the city to come and inspect the finished work before moving on. LPA advised AD to obtain a new fire clearance when the facility construction is completed and to contact LPA after a new fire clearance has been obtained before accepting new residents. No deficiencies were noted pet Title 22 Division 6 of the California Code of Regulations. Type of visit: Case Management - Deficiencies
On 11/15/2024 LPAs Dwayne Mason Jr. and Nancy Guillen arrived at the facility for the purpose of conducting a Case Management visit in order to cite for deficiencies observed on 10/14/2024. LPAs met with Florence Tolentino, Administrator and explained the purpose of the visit. LPAs observed a partial wall installed in the living room. When asked if anyone resides behind the wall in the living room, facility staff stated that two staff members sleep there. LPAs noted the wall does not reach the ceiling. LPAs determined the staff residing behind the wall in the living room are not afforded adequate privacy due to them residing in the living room of the facility behind a partial wall. LPAs determined the facility is not in compliance with the following regulation: PERSONAL ACCOMMODATION AND SERVICES 87307(a) The facility shall be large enough to provide comfortable living accommodations and privacy for the residents, staff, and others who may reside in the facility. Plan of correction: Administrator stated they will consult with the fire marshall to see if they will allow the facility to install a full wall in place of the partial wall. AD stated they will keep LPA updated on their communications with Fire Marshall and may need to request an extension in case the process takes longer than expected.

Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility unannounced for the purpose of conducting a required annual inspection. LPA was greeted at the facility by Benjamin Aguila, Caregiver. Florence Tolentino, Administrator joined the inspection as well. LPA explained the purpose of the inspection. The facility is one-story home with four resident bedrooms, three resident bathrooms, kitchen, dining room, living room, staff room, attached 2-car garage and backyard with pool. Pool is enclosed on all four sides. One side is enclosed by a brick wall separating the yard from a neighboring yard. The other three sides are enclosed by an iron fence with a gate. The gate to the pool remains locked and the pool is currently empty. Facility interior appears clean, safe and sanitary. LPA observed unused exercise/cleaning/renovating equipment in the backyard. A citation is being issued. LPA observed facility has the necessary postings posted on the walls. All resident rooms had the required elements, including bed, chair, closet space and ample lighting. Facility has extra linens and hygiene supplies for residents in hallway cabinets. Restrooms are stocked with soap and paper towels and have hand washing postings. Hot water measured between 105 and 120 degrees F. LPA observed facility has emergency food and water supply. LPA observed the fire extinguisher was serviced on 9/12/2023 according to the attached service tag. A citation is being issued. Smoke/Carbon Monoxide detector were tested and noted as operational. LPA observed hazardous items such as knives, chemicals and cleaners to be locked up in the kitchen and garage. Knives are locked up separate from toxic chemicals. Medication for each resident is kept locked in a closet in the facility. Exit gates are unlocked. LPA observed exit gates to be unobstructed. Based on Record Review, LPA determined, the facility's Plan of Operation is not stored in the facility. A citation is being issued. LPA reviewed three resident files and three staff files. LPAs also reviewed medication for three residents. LPA interviewed one staff and one resident. Based on today's inspection, three citations and four technical assistances are being issued.
