Type of visit: Required - 1 Year

On June 16, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Licensee (LI) Bahira Hanna was present and assisted on today's visit. LPA observed that Bahira Hanna's has a valid Administrator certificate which expires on August 20, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which one can be bedridden, and has a hospice waiver for four. The facility is a single story home which consist of six resident bedrooms, one staff bedroom, three bathrooms, a living room, a dining room, a kitchen, and an attached two car garage. LPA, accompanied by the LI, conducted a tour of the interior portions of the facility. On today's visit, LPA observed four residents in care and one care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the six resident bedrooms and observed them to be clear of any hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in hallway cabinets. LPA inspected the two resident bathrooms and observed them to be clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 107.9 and 109.5 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the facility has a two day perishable and a seven day non-perishable food supply in the kitchen. CONTINUED ON LIC809-C LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed a fire extinguisher to be mounted in the facility which were observed to be charged and up to date on service. LPA tested the wired smoke detectors and individual carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on June 4, 2026. LPA observed the centrally stored medication to be kept in a locked cabinet located in the kitchen. LPA observed there is also a first aid kit stored in the kitchen which has all the required components. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage and laundry. LPA observed chemicals and toxins to be kept in the garage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the LI, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be clear of any obstructions or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all five resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the residents' medication and medication administration records. LPA reviewed four staff files. LPA observed that the facility did not have Health Screening Reports on file for Staff #2 (S2), Staff #3 (S3), or Staff #4 (S4). All staff are background cleared and associated to the facility. Based on the observations made during today's visit, a deficiency is being cited on the attached LIC809-D page. An exit interview was conducted with Licensee Bahira Hanna. Jun 25, 2025Facility evaluation reportReport on file

Type of visit: Required - 1 Year

On June 25, 2025 at 1:15 PM Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver (CG) Mavern Nichol. Administrator (ADMIN) Bahira Hanna was contacted over the phone and LPA explained the purpose of the visit. ADMIN Hanna stated to LPA Kim that they would not be able to attend today’s visit and CG Nichol could sign on the facility’s behalf. The facility is licensed to operate for six (6) non-ambulatory, of which 1 may be bedridden and have a hospice waiver for four (4) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: Six (6) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, attached 2 car garage, kitchen, and an outside covered patio area. LPA Kim toured indoor and outdoor of the physical plant with CG Suzette Robinson. There are no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, Resident 6, and Staff Room 1. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 115.8 degrees F and 117.8 degrees F. A comfortable temperature of 78 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, and sharps objects were stored and not accessible to residents. . Evaluation Report Continues on LIC 809-C The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. LPA Kim observed emergency food and water are stored in the garage. The facility has smoke detectors and carbon monoxide detectors that were operable. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). Fire extinguisher is mounted in the kitchen. A working telephone (714-962-0250) remains available. First Aid Kit contained all the necessary elements. LPA conducted an audit of all resident files (R1-R3), and medication and medication administration record. Staff files for S1 and S2 were missing LIC501, LIC503, and training records. LPA Kim conducted three (3) resident interviews and one (1) staff interview. Deficiencies were cited during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8). LPA observed S1's and S2's LIC501, LIC 503, and training records were missing. LPA observed front left oven burner was not lighting unassisted. A technical violation was issued for PUB475 measured at 8.5” by 11” instead of the regulated 20” x 26”. Type of visit: Required - 1 Year

On 6/12/2024, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit using the CARE Inspection Tool. LPA was greeted by staff and granted entry after stating the purpose of the visit. Administrator (Admin) Bahira Hanna was present to assist with the facility inspection on today's date. The facility is licensed for six (6) non-ambulatory residents with approved hospice waiver for four (4) residents. Currently, there are one (1) Hospice resident present during today’s visit. This is a single story with a two-car garage facility. The facility has six bedrooms, two full bathrooms and one half bathroom. At around 10:30 AM, LPA conducted a tour of the physical plant accompanied by Administrator Hanna, and the following was observed: There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the client's personal belongings was observed. Bed linens, comforters, and bath towels were available during the visit. Bathrooms were operational with water temperature measured at 114.2 degrees F. A comfortable temperature of 73 degrees F. was maintained in the facility. LPA observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene and sharps objects were stored and not accessible to residents. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Facility has one fire extinguisher which was mounted and fully charged. A review of the Medication Records Administration (MAR) was conducted, and LPA observed the records are in compliance. During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents, and sanitizing stations in common areas and restrooms. LPA observed the CONTINUED ON LIC 809C the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. LPA observed First Aid Kit was maintained. A working land line phone was operational. The last fire drill was conducted on 6/7/2024. The facility had operational smoke and carbon monoxide in bedrooms and common areas. The facility has current liability insurance on file effective 2/25/2024 - 2/25/2025. A review of four residents (R1-R4) service files and three staff (S1-S3) personnel files revealed to be complete. The facility has the current administrator's certification on file for Bahira Hanna # 7019658740 - Expiration 8/20/2025. No deficiencies during this inspection visit.
