Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Javier Prieto conducted an unannounced annual required visit. LPA was greeted and granted entry to the facility by Executive Director Jennifer Sanchez. LPA explained the nature of today's visit. LPA accompanied with MS Sanchez, conducted a general overall inspection, which included, but was not limited to the following: Physical Plant: The facility was not operating over capacity or beyond any conditions and limitations on the license. Facility is being maintained at a comfortable temperature for residents. All outdoor and indoor passageways are kept free of obstruction. Hot water temperature was measured at resident bathrooms are within regulation. There are grab bars for each toilet, bathtub and shower used by residents. Smoke detectors and carbon monoxide devices were tested by the Fire Department and found to be in working order. Inspection document was obtained. Facility sprinkler and kitchen sprinkler / extinguisher systems were tested and found to be in working order. Food Service: There is a minimum of one week supply of nonperishable foods and 2 days of perishable foods. Kitchen is well staff and aware of resident's specific dietary needs. Kitchen area is clean and sanitary. Care and Supervision: The facility has ensured sufficient and competent staff to provide the services needed to meet resident needs. The facility is appropriately staff during night shift hours Record Review: LPA requested and reviewed (10) resident and (5) staff files. LPA reviewed staff files for current CPR/1st aide certificates, TB results, and required training's. LPA reviewed client files for admissions agreement, physician report, and needs and service. Administration: LPA did not observe any excluded individuals on the premises at time of visit. The Administrator appears to be on the premises a sufficient number of hours to manage and oversee the business operation. Medical Related Services: Prescriptions and non-prescription PRN medications contain a signed and dated written order from a physician. Medications are centrally locked in the staff office and inaccessible to residents in care. Medications are being administered as prescribed by physician's directions. No deficiencies cited. Allegation investigated: Staff do not answer residents calls for assistance timely Staff do not ensure residents bathroom is cleaned timely

Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conduct a complaint investigation regarding the above allegations. LPA Prieto met with Executive Director Ramirez and explained the elements of the complaint. Allegations #1 - LPA Prieto spoke with Executive Director Ramirez (S1) who states the they reviewed call records and no calls were made from resident #1 (R1) in question. S1 adds that staff is called 5 minutes after a call is made for a response. R1 was interviewed who stated that her calls are being responded to when made. Allegation #2 - Health Services Director (S2) states that R1 is on medication management and is visited at least four times a day. S2 adds that R1 has not made any concerns related the cleanliness of her bathroom. R1 was interviewed by LPA and she states there is no concerns with her bathroom not being cleaned. Unsubstantiated Based on the information obtained there is not enough evidence to support the allegations made in this complaint. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) LaVette Farlow made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection LPA was granted entry to the facility by front desk staff. LPA later met with Hannah C. Lewis, Activity Director. The facility is a Residential Care Facility for Elderly (RCFE), Licensed capacity is (77) current census (61). LPA Farlow was accompanied by Activity Director, Hannah Lewis and Health Service Director, Kiara Estrella to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected residents bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperatures in the bathrooms, and kitchen to be at 117.9, 118.0 and 116.2 degrees F. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. The facility has a covered patio for residents in care in the Assisted Living units and Memory Care unit. There was a designated storage space for client/staff files. Medications are kept inside Med-Tech Room inaccessible to residents. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department. Record Review: LPA reviewed six (6) resident files for admission agreements, updated physician reports, and needs and services plans. LPA Farlow also reviewed four (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings. LPA observed two (2) out of six (6) residents had expired or missing physician report and 1 resident file was not available for review to verify the admission agreement. A deficiency was cited. LPA observed two(2) out of five (5) staff file were missing a CPR/training or criminal records clearance. A technical violation issued. LPA observed the facility did not review or have available for review the LIC610E Emergency Disaster Plan For Residential Care Facilities For the Elderly and LIC9282 Residential Infection Control Plan. A technical violation issued. Medications were audited at random and appeared to be dispensed appropriately by staff members. Based on the observations made during today’s visit, One deficiencies was cited and two (2) technical violation were issued per Title 22, Division 6, of the California Code of Regulations. Allegation investigated: Staff do not ensure that staff follow hand hygiene protocol

Licensing Program Analyst (LPA) Javier Prieto arrived at the facility to investigate the above-mentioned complaint. LPA Prieto met with Executive Director Sanchez and explained the details of the complaint. Allegation #1 - LPA Prieto interviewed facility nurse (S1) regarding the staff hand hygiene protocol. S1 described the facility's hand hygiene measures, stating that hand sanitizing stations are located throughout the facility. S1 added that staff are required to wear gloves when dispensing medication, addressing resident care needs, bathing, and changing residents' incontinence needs. Hand sanitizing gels are used throughout the day while working at the facility. Executive Director Sanchez confirmed the hygiene protocols followed by the staff and added that the facility adheres to procedures outlined in the Mitigation Plan to prevent the spread of COVID-19 and other infectious diseases. LPA Prieto toured the facility as part of a Health and Safety inspection and found no immediate health or safety concerns. Unsubstantiated Based on the information obtained, there is insufficient evidence to support the allegation that staff do not follow hand hygiene protocols. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. On 09/05/2024 at 09:00 AM, Licensing Program Analyst (LPA) Javier Prieto and LPA LaVette Farlow conducted an announced Pre-licensing visit. This is an announced Pre-Licensing visit conducted with Executive Director Jennifer Sanchez who assisted in the tour of inside and outside of the facility and the evaluation. The facility has the proper posting throughout the facility. The facility was evaluated in accordance with the California Code of Regulation (CCR), Title 22 Chapter 6, Division 8. Based on the observations and evaluation of the facility this date, the facility’s ready for licensure. A COMP III orientation was also conducted during today's visit with .
