Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Angelica Boyles conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Administrator Sherwin Marcos. LPA, accompanied by staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows and screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters accessible to residents. Medications were labeled, as required, and stored in locked areas. (CONTINUED ON LIC 809C) (CONTINUED FROM LIC 809) No pools or bodies of water were observed on the premises. Per Administrator, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA reviewed multiple staff and resident records/files. The reviewed files contained required documents. Confidential records were stored in locked areas. Proof of current/active business liability insurance was presented. No deficiencies were observed or cited on today's visit. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to conduct the continuation of the required annual inspection. LPA was greeted by Administrator Sherwin Marcos, identified herself, and was granted entry into the facility. The current census was five (5) residents in care. LPA Correia conducted the remainder of the facility tour during today's visit. The facility ambient temperature was 78 degrees Fahrenheit. Resident bathrooms and bedrooms were equipped with the required furnishings and safety measures. Medications, toxins, and sharp objects were kept in locked cabinets and inaccessible to residents in care. The facility has a shaded outdoor area and a large common area for resident activities. LPA observed the required amount of food appropriately stored and labeled. Per Administrator Marcos there is no ammunition or weapons kept on the premises. Carbon monoxide, smoke detectors, and fire extinguisher were present, operational, and or up to date. LPA observed the required postings. Administrator Marcos’s certification is active. The facility’s Limited Liability Corporation (LLC) and liability insurance were active. The last disaster drill was conducted on July 21, 2025. Based on the inspection the facility was in compliance and no deficiencies were cited. An exit interview was conducted with Administrator Marcos, and a copy of this report along with Licensee/Appeal Rights (LIC9058 FAS 01/16) will be provided at the conclusion of the visit and signature on this form acknowledges receipt of the rights and a copy of this report.the state’s words, verbatim · CDSS document, Sep 9, 2025

Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to conduct the required annual inspection, LPA was greeted by Administrator Sherwin Marcos, identified herself, and was granted entry into the facility. Licensee Michelle Retzer was contacted and notified Licensing was present to conduct their annual inspection. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory, three (3) can be receiving Hospice Services, and one (1) resident may be bedridden. The current census is four (4) residents in care. LPA Correia conducted a resident records review and a partial facility tour. Per today's facility visit resident records were complete and up to date. During the facility tour LPA observed smoke and carbon monoxide detectors were present and operable, fire extinguishers were present and current on inspection, and the facility has a landline. An overall inspection of the facility began today, however due to time constraints LPA was unable to complete the visit and will return later to conduct the remaining portion of this inspection. No deficiencies were cited during today's visit. This report was discussed with Administrator Sherwin Marcos. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Juliana Barfield conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Carina Bigayan. Licensee Michelle Retzer joined LPA shortly thereafter. According to the facility’s license, the facility has a maximum capacity of six (6) residents, of whom six (6) may be non-ambulatory of which six (6) may be bedridden. During today’s inspection, there were a total of six (6) residents in care. This facility does not feature a secured perimeter or delayed egress doors. LPA, accompanied by Carina Bigayan, toured the interior and exterior of the facility, and inspected each room. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. Hot water temperature at taps accessible to clients were all compliant. There was at least 2 days supply of perishable food, and at least 7 days non-perishable food present. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters observed available to clients. Medications were labeled, as required, and stored in locked areas. (CONTINUED ON LIC809-C) (CONTINUED FROM LIC809) No pools or bodies of water were observed on the premises. Per the licensee's staff, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. Confidential records were stored in locked areas. Michelle Retzer also presented proof of current/active business liability insurance. No deficiencies were observed or cited during today's annual inspection..
