On July 21, 2026, Licensing Program Analysts (LPAs) Brandon Lopez and Tran Nguyen made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPAs were greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Loida Manguray and Licensee (LI) Norma Ureta were notified via telephone and later arrived to assist with the inspection. LPAs observed that Loida Manguray has a valid administrator certificate which expires on May 1, 2028. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has an approved hospice waiver for six. The facility is a single story home with six private resident bedrooms, three shared resident bathrooms, a living room, a dining room, a kitchen, and an attached two car garage. LPAs, accompanied by the AD, conducted a tour of the interior portion of the facility. On today's visit, LPAs observed four residents in care and two care giving staff present. LPAs observed residents relaxing in the living room. LPAs observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPAs inspected the six private resident bedrooms and they were observed to be free of hazards. LPAs observed residents bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPAs observed resident beds had clean linens and blankets. LPAs observed additional linens to be stored in each resident's bedroom. LPAs inspected the three resident bathrooms. Resident bathrooms were clean. Resident bathrooms were equipped with grab bars. LPAs observed that one of three resident bathrooms did not have a non-skid floor mat present. Faucets and toilets were operational. Hot water temperature measured between 108.8 and 109.4 degrees Fahrenheit. LPAs observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPAs observed kitchen appliances to be clean and operational. CONTINUED ON LIC809-C LPAs observed kitchen knives and sharps to be stocked in a locked kitchen cabinet. LPAs observed a fire extinguisher to be mounted in the kitchen and it was observed to be charged and serviced as of June 2, 2026. LPAs tested the individual smoke detectors and carbon monoxide detector which tested operational. LPAs observed the facility did not conduct an emergency disaster drill in the second quarter of 2026. LPAs observed the facility conducted their most recent emergency disaster drill on January 10, 2026. LPAs observed the garage door to be kept locked and inaccessible to residents in care. LPAs observed a staff to be living in the two car garage. LPAs observed a bed, a dresser, a chair, and additional personal belongings which indicated that the staff currently resides in the garage. LPAs observed the centrally stored medication to be kept in the garage. LPAs observed the facility has a first aid kit stored in the garage and it has all the required components. LPAs observed chemicals and toxins to be stored in the garage. LPAs observed the facility has a three day emergency food and water supply stored in the garage. LPAs, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPAs observed a shaded outdoor seating area with furniture for resident use. LPAs observed a shed located in the backyard which was observed to be kept locked and inaccessible to residents in care. LPAs observed the perimeter gate of the facility can be opened in an evacuation. There are no bodies of water on the premises. LPAs reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPAs reviewed residents' medication and medication administration records. LPAs reviewed three staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, deficiencies are being cited on the attached LIC809-D page and a technical violation is being issued. Licensing Program Analysts (LPAs) Michael Tea and Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPAs were greeted and granted entry by Loida Manguray, Administrator (AD) and LPAs explained the purpose of the visit. The facility is a two-story, six bedroom, two bathroom residence with an approved fire clearance of six non-ambulatory residents of which three may be on hospice. Staff member and an additional person live on the second floor who is not background cleared. An immediate civil penalty of $500 LIC 421BG was assessed for the additional person. The facility currently has a census of five residents in care with four on hospice. A technical violation will be given. Licensee produced email communication requesting an increase of hospice waiver to four residents. LPA advised AD and Licensee (LE) to not accept additional hospice residents until the waiver is approved and to follow-up with LPA or Licensing Program Manager (LPM) for a timely response. During today’s visit, LPAs toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured 105.2 degrees Fahrenheit and all smoke detectors were operational. The fire extinguisher is charged and was serviced on April 8, 2025. The facility’s last fire drill was conducted on March 10, 2025. Administrator was advised to conduct fire drills quarterly and a technical violation was given. (Continued from LIC 809-C) (Continued from LIC 809) LPAs inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. There are plenty of fruits and vegetables in the refrigerator. Emergency water is located in the garage and emergency food is in the front closet. Sharps and knives are secured under the sink and hazardous chemicals are stored in the garage. All appliances were in working order. LPA Tea observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. LPAs toured the exterior of the property and there were no hazards or obstacles in pathways and there were plenty of shaded seating areas for residents to sit outdoors. There is a large area for outdoor activities and LPAs observed fitness cycling equipment for residents to use. Outdoor area is shared between the two Huntington Beach Guest Homes 1 and 2 which are adjacent to one another. LPAs reviewed two of two staff training and fingerprint records and conducted a complete review of resident records. LPA Tea interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPAs confirmed that administrator has a current administrator certificate which expires on May 1, 2026. LPA confirmed facility profile information was correct, fees are current and the primary language is English. The following deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. On August 30,2024 8:00am, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Administrator (AD) Loida Manguray and explained the purpose of the visit. The facility is licensed to operate for six (6) non-ambulatory residents and have a hospice waiver for 3 residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedroom, two (2) bathrooms, living area, dining area, kitchen, and outside covered patio area. LPA Kim toured inside and outside of the physical plant with AD Manguray. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, Resident Room 6, and staff room. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured between at 109.9 degrees F and 111.3 degrees F in Resident 2 Bathroom. A comfortable temperature of 73 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished. The facility has operable smoke detectors and carbon monoxide detectors. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. Emergency food is stored in the kitchen pantry. A working telephone (714-536-2375) remains available.. Fire Extinguisher was fully charged, mounted in the kitchen, and serviced on April 10, 2024. First Aid Kit is complete and has all the necessary elements. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents, and sanitizing stations in common areas and restrooms. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). Emergency drills are conducted quarterly and last conducted on June 15, 2024. LPA Kim conducted an audit of residents #1-6 (R1-R6) files, and staff #1-#3 (S1-S3) l files, and medication were all in order and complete. LPA Kim conducted 1 staff interview and 1 resident interview. No deficiencies were cited during this visit.
