On July 21, 2026, Licensing Program Analysts (LPAs) Brandon Lopez and Tran Nguyen made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPAs were greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Loida Manguray was present and assisted with today's inspection. LPAs observed that Loida Manguray has a valid administrator certificate which expires on May 1, 2028. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has an approved hospice waiver for six. The facility is a two story home. The first floor consist of six private resident bedrooms, three shared resident bathrooms, one staff bedroom, a living room, a dining room, a kitchen, and an attached two car garage. The second story is for staff use only and consist of three staff bedrooms and one staff bathroom. LPAs, accompanied by the AD, conducted a tour of the interior portion of the facility. On today's visit, LPAs observed five residents in care and two care giving staff present. LPAs observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPAs inspected the six private resident bedrooms and they were observed to be free of hazards. LPAs observed residents bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPAs observed resident beds had clean linens and blankets. LPAs observed additional linens to be stored in a hallway closet. LPAs inspected the three resident bathrooms. Resident bathrooms were clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 105.2 and 108.4 degrees Fahrenheit. LPAs observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPAs observed kitchen appliances to be clean and operational. CONTINUED ON LIC809-C LPAs observed kitchen knives and sharps to be stocked in a locked kitchen cabinet. LPAs observed a fire extinguisher to be mounted in the kitchen and it was observed to be charged and serviced as of June 2, 2026. LPAs tested the individual smoke detectors and carbon monoxide detector which tested operational. LPAs observed the facility conducted their most recent emergency disaster drill on July 21, 2026. LPAs observed the garage door to be kept locked and inaccessible to residents in care. LPAs observed the garage to be used for storage and laundry. LPAs observed the centrally stored medication to be kept in the garage. LPAs observed the facility has a first aid kit stored in the garage and it has all the required components. LPAs observed chemicals and toxins to be stored in the garage. LPAs observed the facility has a three day emergency food and water supply stored in the garage. LPAs inspected the four staff bedrooms and one bathroom observed them to be free of any hazards. LPAs, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPAs observed a shaded outdoor seating area with furniture for resident use. LPAs observed the perimeter gates of the facility can be opened in an evacuation. There are no bodies of water on the premises. LPAs reviewed all five resident files. All the required documentation were present and current in the resident files reviewed. LPAs reviewed residents' medication and medication administration records. LPAs reviewed two staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. Licensing Program Analysts (LPAs) Rose Ruppert and Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPAs Ruppert and Tea were greeted and granted entry into the facility by Administrator (AD) Loida Manguray and explained the reason for the visit. The facility is licensed for six non-ambulatory residents, with a hospice waiver for three. Currently there are four residents, two are on hospice during today's visit. LPA reviewed two staff files and four resident records. Resident and staff files contained all required documentation. Administrator certificate expires on May 1, 2026. The last disaster drill was conducted on March 10, 2025. LPAs reminded the administrator that disaster drills need to be conducted quarterly, within 3 months. LPA Tea along with AD Manguray toured the facility. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of six resident bedrooms, one staff room, three full bathrooms, a living room, dining room, and kitchen. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. The water temperature measured 112.1 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards; doorways were free of obstructions. First aid kit had all the required elements including bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at the time of visit. LPA did observe expired non-perishable food in the pantry and advised AD Manguray to periodically check for expired ones and discard them. LPA observed sharps secured and locked underneath the kitchen sink. LPA (Annual inspection continued on LIC809C) also observed toxin substances stored and inaccessible to residents in the garage of the neighboring sister facility. The fire extinguisher in the kitchen was fully charged with an inspection date of April 8, 2025. Kitchen appliances are operational. LPA toured outside ground; they share the backyard with their sister facility. There is patio seating with umbrellas where residents can sit and relax. There are two exit gates on both sides of the facilities that are self-latching and operational. LPA observed emergency food and water supply in the shed in the backyard and staff room. Facility provides activities based on their personal choice and health condition. Residents participate in numerous activities like doing exercises and walking around the facility. They also participate in sing a longs and dancing to be active. At the time of the visit, LPA observed residents having breakfast. LPA Ruppert reviewed medication storage and administration. Medications are stored in a locked cabinet in the staff room. Medications are being administered per physician order. LPA Tea interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting a required one-year annual inspection. LPA was greeted, granted entry by staff, and explained the reason for the visit before entering the facility. Around 10:30 am LPA Haley began the tour of the facility with staff. There were four residents present for the visit. Right next to the front door there is a screening station and sign in sheet. Next to the front door is a closet with comforters and board games. All resident bedrooms were clean, organized, and had all the necessary requirements: night stand, chair, lamp, and storage space. Resident bathrooms were clean and organized. Hot water temperature was measured in the range of 105.2 – 105.8 degrees Fahrenheit. The kitchen was clean and organized. All knives and sharp objects were locked under the sink. All burners on the stove were operational. The facility has a two-day supply of perishable food items and seven-day supply of nonperishable food items. The garage is used for storage and had clear walkways. There’s a washer and dryer in the locked garage and a supply of hazardous cleaning chemicals. There are additional refrigerators in the garage for the residents, and one for staff with an additional supply of food items. An additional supply of nonperishable items are stored in the garage as well as a supply of fruit. There’s a supply of COVID items, including gloves, gowns, N95 mask, hand sanitizers, and disinfectant sprays for use in a large bucket. Miscellaneous items for the facility including a wheelchair, and a locked filing cabinet for resident and staff files. The backyard was clean and well organized. Walkways were free of obstruction. There's a table with chairs and a sun shade. There’s a locked storage shed in the backyard with a supply of various facility items. In the lock shed is an emergency supply of food, mattresses, bed frames, walkers, and other items. Continued on LIC809C There were no bodies of water observed. All smoke detectors were tested and are operational. The carbon monoxide detector tested operational. Emergency evacuation drills are conducted quarterly, and the last drill was conducted April 15, 2024. A review of all resident files and medications was conducted, as well as a review of 3 staff files. No deficiencies are being cited during today’s visit.
