On August 11, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Annual Inspection and met with the Lead Caregiver, Elva Flores. The facility file review was conducted at the Regional Office and additional records were requested and reviewed on site. The facility is licensed for six Elderly Residents and is currently operating at the capacity of six Elderly Residents. For a 740 facility type. The facility is operating within the scope of its license. LPA Mixson toured the facility along with Elva Flores and made observations pertaining to the Annual visit. LPA inspected the facility inside and outside. There were no obstructions or debris to the indoor or outdoor passageways observed currently at the time of this visit. Additionally, there were no bodies of water seen on the premises at the time of this visit. The facility is a single-story facility located at 26450 Helene Drive Moreno Valley CA. 92555. Physical Plant: The facility phone number is (909) 548-1769, and it is operable. LPA Mixson observed the residents’ bedrooms, and each was furnished as per Regulations and Title 22. LPA Mixson inspected the facility bathrooms, and the hot water temperature tested within regulations. The bathrooms were clean, and appliances were operating appropriately currently. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. LPA Mixson observed required postings such as "If you See Something, Say Something,” and the "Personal Rights” directly behind the front door. The cleaning supplies and sharp items were locked and inaccessible to the residents in care presently. There were designated storage spaces for the residents’ and staff’s files, and it was locked and inaccessible to residents in care at present. Medications: Were locked and inaccessible to residents in care. Additionally, there were a sufficient supply of medication for the residents in care. There were no documented errors observed on the centrally stored medication forms, and medications were stored in their original containers at the time of the visit. The facility has two caregivers on site at the time of the visit. Food Service& furniture: The non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents at this time. Dishes and utensils were in sufficient supply and stored properly, and sharp items are locked. The overall facility is clean; the furniture is in good condition and arranged in a manner which provides space for residents to move safely. The facility cooling system and other appliances were operable at the time of this visit. The Administrator informed LPA there were safety lights for night throughout the facility. Care & Supervision/Administration: There were adequate staff present for the supervision of residents in care. The floor plans, telephone numbers and personal rights were found posted in the facility. The listed Administrator, Teresa Fuentes possess a current administrator’s certificate with an expiration date of 09/18/2027, and it is posted in the facility, behind the front door. Records Reviewed and Resident/Staff Files: LPA Mixson reviewed staff files and the facility's staff schedule. The staff files reviewed comprise required documents, along with current TB tests. The resident files reviewed had the essential paperwork as per Regulations at the time of this visit. Caregiver Training Binder: LPA observed the facility training binder and the binder was organized and had a clear table of contents. Infection Control: LPA Mixson reviewed the facility's emergency and disaster plan as well as the disaster training binder. LPA observed the last fire drill was conducted July 31, 2026, met the Department standards, and was logged. LPA Mixson observed the hand washing stations in the facility restrooms. LPA observed PPE equipment and cleaning supplies to conduct regular cleaning of the facility. LPA reviewed the facility's infection control plan and found required infection control measures met the Department requirements. Gloves were observed throughout the facility. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Janira Arreola conducted an announced visit to the facility in order to conduct the prelicensing inspection. The LPA met with Applicant, Nick Vermani and with administrator Teresa Fuentes who was informed of the purpose of the visit. At the time of the visit there were (4) staff and (1) clients present. The facility is seeking a change in ownership for a residential care facility for the elderly. The facility is a one story home with (4) bedrooms and (2) and a half restrooms. There are no pools or firearms located on the property. LPA conducted a tour of the facility and reviewed documents. LPA observed the client bedrooms, bathrooms and common areas. Required furniture was present and in good repair. LPA observed where the client and staff records will be storage. The facility had the required postings. LPA observed where the cleaning supplies, and dangerous/ sharp objects are being kept locked and inaccessible to clients. LPA observed the smoke and carbon monoxide detectors in the facility were operational during the visit. The hot water was read at 113F. The facility has a working land line at (951) 807-3397. LPA observed first aide kit with all required items. The exits are free of obstructions, the facility has a documented fire drill and emergency supplies such as emergency lighting and food. LPA observed the client medications are being stored locked and in their originally received containers. LPA reviewed staff files which had required paperwork. The facility has operating video surveillance, LPA observed the cameras are in common areas and reviewed the consent forms will be part of the new admission agreement provided to facility residents. All staff present are finger print cleared and the licensee was advised on having all staff fingerprinted prior to working or residing at the facility. Identification of the applicant and administrator was verified by confirming driver’s license number. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB.
