Type of visit: Required - 1 Year

On 9/19/25, Licensing Program Analyst (LPA) Cynthia Tamayo arrived unannounced to conduct a required - 1 year inspection visit. LPA met with Licensee, Vinh Tran (S1), and Licensee Lisa Saephan (S2) and explained the purpose of the visit. An entrance interview was conducted. Administrator certification expires on 6/19/25. The facility is licensed to serve up to 6 non-ambulatory residents. Hospice waiver approved for 3. Current census is 5. LPA toured the facility with S1. LPA toured the physical plant including but not limited to the common area, kitchen, dining area, resident bedrooms; resident bathrooms, laundry room, and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA observed the facility is clean and in good repair. LPA observed required furniture and lighting throughout the facility. LPA observed supplies of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days maintained on the premises. LPA observed staff preparing and serving lunch to residents during this visit. The hot water temperature was measured at 115.8*F which was within the required range of 105-120*F. The temperature inside the facility measured at 70* F which was within the required range of 68-85*F. LPA observed the centrally stored medications area to be locked and inaccessible to residents. LPA observed the fire extinguisher(s) and first aid kits were up to date. LPA observed smoke and carbon monoxide detector(s) in the facility were in good repair. LPA requested resident and staff files for review. LPA reviewed (4) staff files and (5) resident files, including criminal record clearances. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. Allegation investigated: Staff are not responding to resident's call button in a timely manner Staff left resident in soiled diapers for extended period of time

On 9/26/24, Licensing Program Analyst (LPA) Tung Truong arrived unannounced to commence a complaint investigation with the allegations mentioned above. LPA met with Administrator Vinh Tran and explained the purpose of the visit. During today’s visit, LPA conducted interviews and reviewed records. Based on records review, and staff and resident interviews, there is not a preponderance of evidence to substantiate the allegations mentioned above. Regarding the allegation, “Staff are not responding to resident's call button in a timely manner” LPA obtained the following information through interviews. resident (R1) stated that staff response to call pendant anywhere from 15 -30 minutes on multiple occasions. Interviews conducted did not reveal any corroborated statements of staff not responding to resident's call button in a timely manner. Staff (S1) stated that resident is tended to within 3-5 minutes when resident call for help. Regarding the allegation, “Staff left resident in soiled diapers for extended period of time” LPA obtained the following information through interviews. There is insufficient evidence indicating that resident (R1) was left soiled for an extended period of time. Continued on 9099-C Unsubstantiated According to staff, R1 is turns and has diaper change every two hours. Staff stated that R1 doesn’t want to be disturbed after 11:30 pm unless resident calls for help. According to home health nurse, R1’s skin rash around the groin area is improving from a stage 2 to a stage 1. Based on statement obtained, there is no indication that staff left resident soiled for an extended period of time. Facility staff and home health nurse stated that R1 is very challenging and demanding. Staff stated that R1 is never satisfied with care. According to interviews and records review, R1 has a history of noncompliance and frequently yelling at staff. As a result of the investigation, LPA finds the allegations above to be UNSUBSTANTIATED- A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Allegation investigated: Staff handled resident in a rough manner Staff made inappropriate comments to resident

On 9/19/24, Licensing Program Analyst (LPA) Tung Truong arrived unannounced to complete and delivery findings for a complaint investigation received on 8/28/2024. LPA met with Licensee Lisa Saephan and explained the purpose of the visit. Throughout the course of the investigation, LPA conducted interviews and reviewed records. Based on records review, and staff and resident interviews, there is not a preponderance of evidence to substantiate the allegations mentioned above. No interview confirmed that the staff is making inappropriate comments to residents or handle residents in a rough manner. Residents stated that they feel safe and have no concerns with care. Staff (S1) denied making inappropriate comments to resident (R1) or handle R1 in a rough manner. Based on records review, it was learned that R1 has a history of noncompliance and frequently cursing at staff. Type of visit: Case Management - Deficiencies

Licensing Program Analyst (LPA) Tung Truong arrived unannounced to conduct a case management visit on 9/19/2024. LPA met with Licensee Lisa Saephan and explained the purpose of today’s visit. The purpose of today's visit is in response to deficiencies observed during an annual inspection visit on 9/19/24. LPA Truong observed an alteration to the building, a conversion of a single garage into a bedroom. Based on record review, it was learned that the facility has obtained a building permit since May of 2023. The construction was stated in April 2024 and finished in June 2024. Licensee advised that the bedroom is for live-in staff currently. It was learned that the facility has converted the single garage into a staff room for the live-in staff without notifying the Department. The licensee was advised to provide the Department with an updated facility sketch and plan of use of the bedroom. Deficiencies were cited on the LIC 809-D pursuant to the California Code of Regulations, Title 22, and California Health and Safety Code. Plan of correction: Licensee shall review Title 22 Regulations, Section 87305 and submit a written statement to LPA stating knowledge of, understanding of regulation by POC due date. The Licensee shall provide LPA with building permits, plan of use of the bedroom and an updated facility sketch.

Type of visit: Collateral

An unannounced case management visit made out to the facility on 07/31/2024 at 10:10 AM, by Licensing Program Analyst Pang Lee who was met by caregiver Barbara Rubio who then called the administrator VinhTran. The administrator arrived approximately 15 mins later and joined the visit. The current census is 4 with 1 facility staff during today’s visit. LPA Lee explained the purpose of this case management visit was to interview resident 1 (R1) in regards to a compliant control # 27-AS-20240417144857 that is unrelated to this facility. During today’s visit LPA Lee also collected MAR for the month of June 2024. Per California Code of Regulations, Title 22, no deficiencies were observed during this visit.
