On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Administrator June Que arrived shortly after. The facility is a one-story house and is licensed for six non-ambulatory residents, of which six may be on hospice and zero bedridden. There are a total of four bedrooms and currently there are 5 residents present during today's visit. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two bathrooms. The hot water temperature measured at 159.6 degrees Fahrenheit and all smoke detectors were operational. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed unsecured medication storage in the facility dining room. LPA observed an unsecured Nyquil in facility refrigerator. LPA observed shaded area for residents in backyard with ample seating. LPA observed 3 out of 5 residents with rails without physician's order. LPA observed half full egg carton on kitchen counter and not refrigerated. LPA observed staff bedroom in storage shed in facility backyard. LPA reviewed two out of two staff records, not including training as Administrator stated training records are off site. LPA conducted a complete review of resident records. LPA confirmed that administrator has a current administrator certificate which expires May 29, 2028 . Based on observations made during today's visit deficiencies are being cited per Title 22. Allegation investigated: Visitor got bit by facility dog

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit to deliver findings on the above allegation received on August 19, 2024. LPA was greeted and granted entry into the facility and met with Caregiver Glenn De Los Reyes. LPA explained the reason for the visit. Administrator (AD) Jundith Que arrived shortly after. This Department has investigated the complaint alleging that visitor got bit by facility dog. Regarding the allegation the following was revealed: During the initial visit on August 23, 2024, and subsequent visit on November 25, 2025, LPA tour the facility and did not observe a dog in the facility. During the course of the interviews AD reported that the incident happened in the front property. During the course of the investigation LPA reviewed the City of Orange Accessory Dwelling Unit (ADU) building permit dated March 31, 2022. Per building permit, the permit was issued for 887 and not for A Touch of Care Assisted Living VIII. During the course of the interviews with the staff, Staff 1 (S1) reported that the incident happened by the driveway next to 887 and stated that there is not a dog in the facility. CONTINUED ON LIC9099-C... Unfounded Therefore, the allegation is deemed UNFOUNDED, meaning the allegation is false, could not have happened and/or is without a reasonable basis. For today’s visit, there were no citations issued per Title 22, Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit for the Required 1 Year Inspection. LPA explained the purpose of today’s visit, and was greeted and granted entry by Caregiver Judibryn Costiniano. Administrator (AD) Jundith Que arrived shortly after. For today’s visit, LPA observed a total of six residents in care and two staff members on duty. LPA observed the Administrator's Certificate for facility AD Jundith Que which expires on May 29, 2026. LPA Ramirez toured the interior and exterior portions of the facility with AD Que. The facility is a one-story house and is licensed for six non-ambulatory residents, of which six may be on hospice and zero bedridden. There are a total of four bedrooms, of which four are resident bedrooms. LPA Ramirez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detectors and auditory exit alarms were tested and operational. There are a total of two restrooms. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature tested between 109.2-115.2 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguisher was charged, mounted and located by the living room. CONTINUED ON LIC809-C.. LPA Ramirez observed the emergency disaster and evacuation plan, which is posted by the dining room. LPA observed that the First Aid Kit had all the required components. LPA observed that medications and toxins were locked and inaccessible to residents in care. Medications are locked in a cabinet by the dining room. For the exterior portion, LPA Ramirez observed a shaded area, patio furniture, and the grounds were free of any hazards. There is one gate in the backyard. No bodies of water were observed. LPA reviewed five resident files and three staff files. LPA interviewed residents and staff present. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD Que. Type of visit: Prelicensing

Licensing Program Analysts (LPA) Alvaro Ramirez, Jr. conducted an announced visit to the facility to conduct the pre-licensing inspection. LPA met with Applicant Jundith Que and toured the facility. An initial application to operate a Residential Care Facility for the Elderly (RCFE) was submitted to Community Care Licensing (CCL) on November 30, 2023. The facility is to have a capacity of 6 residents, (0) ambulatory, (6) non-ambulatory and (0) bedridden residents. Facility phone number is 661-269-6358. LPA observed the following. Structure: The facility is a one-story house with four resident bedrooms, one staff bedroom, two full size bathrooms, a living room, a kitchen, a dining room, and an attached car garage. LPA observed the See Something, Say Something poster (PUB 475) in the facility mounted on the wall by dining room. There is one gate in the backyard, which both is self-closing and self-latching. There is a shaded seating area and LPA did not observe any obstacles or hazards in the backyard. Air/Heating: Central air/heating system installed with a central panel to control entire house. Resident Bedrooms: There are four Resident bedrooms. All resident bedrooms had the required furnishings. LPA observed all beds had linens and blankets. CONTINUED ON LIC809-C... Bathrooms: All bathrooms are clean and have working plumbing. Hot water measured between 109.5-111.5 degrees Fahrenheit. Linens & Hygiene Supplies: A supply of extra linen for each resident was stored in the closet by the resident's bedroom hallway. Emergency Phone Numbers, Exit Plan & Menu: Posted & readily available for review an emergency disaster plan with means of exiting and emergency phone numbers listed. Menus posted and available. Monthly menu available and posted by the dining room. Food Service: There are six residents living in the facility at this time. A supply of 2-day perishable and 7-day of non-perishable food was observed and will be maintained on hand. Smoke Detectors/Carbon Monoxide Detectors: Smoke detectors/carbon monoxide detectors are hardwired and tested operational. Fire extinguishers were charged, mounted and one was located by the main entrance hallway and one in the staff's bedroom. Appliances: There is one, four gas burner stove which lights unassisted, one oven, microwave oven mounted above the stove, a refrigerator, dishwasher, washer, and dryer. All appliances are clean and operational. Toxins: All and any toxic chemicals, cleaning solutions, laundry toxins and disinfectants are inaccessible to residents and will be stored in the garage. Water Temperature: Hot water was measured in all bathrooms. Hot water measured between 109.5-111.5 degrees Fahrenheit. Medications, First-Aid Kit & Book: The first aid kit and the first aid manual are stored in a cabinet next to the dining room. The first aid kit has all the required elements. Medications will be stored in locked cabinet next to dining room. Resident & Staff Files: The Resident and Staff Records will be kept in a cabinet next to the dining room. Reading Material, Games, Equipment & Materials: Arts and craft and board games are stored in the living room cabinet. There is one large screen television mounted in the living room. Fire clearance: Fire Clearance approved by a fire inspector of Orange County Fire Authority on February 28, 2024. Special conditions noted, "Fire clearance approved under the above noted conditions, capacity for 6 nonambulatory." Component III: Conducted at the Pre-Licensing visit, information provided about how to operate the facility within compliance and reporting requirements. The applicant has met all pre-licensing requirements. LPA will submit notification to CAB in Sacramento for final review prior to license being issued. Applicant was informed today that the final approval will be processed by CAB (Central Applications Bureau) in Sacramento.
