Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an annual required visit and inspection of the facility. LPA was greeted with staff Galina Nikolayevna and was advised for the reason of the visit. At 11:00 AM Karine Sarkisyan who is the licensee met with LPA, explained the reason for the visit. At 11:58 AM, with the assistance of licensee, LPA took a tour of the physical plant. Required postings were observed in the entry area. At 11:56 AM, the smoke alarms were tested and are operational that are located each bedroom, the hallway and kitchen. There are carbon monoxide detectors that functions properly. The fire extinguisher is in the kitchen. The charge date is 1.3.2026. During the visit the facility is at 70 degrees Fahrenheit. The facility is fire cleared maybe for two (02) non-ambulatory residents, five (5) ambulatory. Hospice waiver for six (6); one maybe bedridden bedroom #3. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. Food storage and preparation areas are clean and inaccessible to pests. Garbage cans have tight fitting covers. Cleaning supplies, pesticides or toxic cleaning supplies were stored and locked away in the kitchen below the sink. Office is located in the kitchen. Bedrooms: There were three (3) bedrooms designated for residents' use. Bedroom #1, bedroom #2, and bedroom #3 are shared and is used for residents only, the bedrooms were properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. Continue to LIC 809-C BATHROOMS: The facility has two (2) bathrooms. Bathroom #1 is located by the hallway in beside bedroom #1. Hot water measured at 106.3 degrees Fahrenheit. Bathroom #2 is located inside bedroom #3. Hot water measured at 111.3 degrees Fahrenheit. The bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. Common Areas: LPA toured all common areas of the facility. These included the living room and dining area for residents. The common areas were properly furnished. Residents dining table fits enough for six (6). LPA observed common areas to be very clean and tidy. LPA observed the floors to be in very good condition. No obstructions and or tripping hazards throughout the facility. Furniture in common area was observed to be in good repair. There are no issues with Fire Clearance. Infection control: Facility mitigation plan to make sure licensee was following current infection control recommendations. LPA obtain a copy and reviewed the infection control plan during this visit. Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. The facility does not have a swimming pool or bodies of water. The garage is detached and is converted into a storage. Laundry service: There is enough linen available to change weekly or more if need. Cleaning supplies are being stored in a locked cabinet in the laundry area and is located outside the garage. Staff Files: LPA also conducted a file review of staff records to ensure forms and training are up to date and compliance with licensing forms. Office is located inside the kitchen. Records were checked for expired or missing certificates and clearances: LPA conducted a file review of staff for criminal record clearances and current First Aid. The administrator file was reviewed for current first aid, fingerprint clearance, administrator certificate, and HIV/AIDS and TB training. Continue to LIC 809-C Medications are in a centrally stored and locked place, including over-the-counter medicines; medications are properly labeled and checked for expiration dates. Each centrally stored prescription and PRN medication has been logged in the medications log with proper documentation from the clients’ doctor. Proper medication dispensing instruction are followed and checked for contamination. First-aid has all proper items and is current. Resident records were reviewed for requirements and legibility: LPA reviewed client’s files for current appraisal. Planned activities are offered. Facility is within CA code of Regulations Title 22 or Health and Safety Code. Dec 4, 2025Complaint investigation reportUnsubstantiated
Allegation investigated: Staff are interrupting a resident's phone calls. Staff are not meeting the needs of a resident.
Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an unannounced initial complaint visit to this facility to investigate the above allegations. LPA met with administrator Gayane Azaryan, and explained the purpose of the visit. Allegation #1: Staff are interrupting a resident's phone calls. It was alleged that the staff refused phone calls for resident #1 (R1), that there is always an excuse for R1 not be on the phone. Upon LPA arrival, it was observed that residents were having breakfast at the dining table. Continue to LIC 9099-C Unsubstantiated To investigate the allegation, at 8:52 AM LPA conducted a physical plant tour to ensure the health and safety of the clients in care. LPA interviewed five (5) out of five (5) clients from 9:26 AM to 10:00 AM and three (3) staff from 10:00 AM to 10:30 AM. LPA requested R1’s documents at 10:32 AM included but not limited to staff roster, resident roster, physician report, admission agreement, appraisal needs and service plan, and other pertinent documentarians. LPA received copies of the requested documents at 10:40 AM. Interviews with R1 stated that he is safe and happy living at the facility. R1 advised LPA that they do not want to talk to the caller and advised facility staff that they are unavailable and refuses to take the call. All of the residents stated that they have their own cell phones, and staff never refuses to give/ hand them a call from the facility phone. During interviews with staff, it was revealed that they were told by R1 to advise the caller they are unavailable and refuses to take the phone call. Based observation and interview, there is not enough information to verify the allegation. There is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time. Allegation #2: Staff are not meeting the needs of a resident. It was alleged that the facility does not provide items needed for Resident #1 (R1). During the physical plant tour, LPA observed all the necessary items of the residents in their bedroom and bathrooms storage. During interviews with staff, it was revealed that they provide all the necessary items requested by the residents and never deny them. During interviews with residents, all residents stated they were provided with all necessary supply from the staff upon request. Based on interviews, record review, and observation, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time. Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an annual required visit and inspection of the facility. LPA was greeted with staff Goarik Diloyan and was advised for the reason of the visit. At 1:00 PM Gayane Azaryan who is the administrator met with LPA, explained the reason for the visit. At 1:30PM Licensee, Karine Sarkisyan, who is the licensee arrive and was explained the reason of the visit. At 1:00 PM, with the assistance of administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are operational that are located each bedroom, the hallway and kitchen. There are carbon monoxide detectors that functions properly. The fire extinguisher is in the kitchen. The charge date is 2/08/2025. During the visit the facility is at 71 degrees Fahrenheit. The facility is fire cleared maybe for two (02) non-ambulatory residents, five (5) ambulatory. Hospice waiver for six (6); one maybe bedridden bedroom #3. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Common Areas: LPA toured all common areas of the facility. These included the living room and dining area for residents. The common areas were properly furnished. Residents dining table fits enough for six (6). LPA observed common areas to be very clean and tidy. LPA observed the floors to be in very good condition. LPA was greeted with staff Mwale Besa and was advised for the reason of the visit. At 10:07 AM Gayane Azaryan who is the administrator met with LPA, explained the reason for the visit. At 10:40 AM licensee arrived Karine Sarkisyan, LPA explained the reason for the visit. At 10:08 AM, with the assistance of administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are operational that are located each bedroom, the hallway and kitchen. There are carbon monoxide detectors that functions properly. The fire extinguisher is in the kitchen. The charge date is 2/27/2024. During the visit the facility is at 67 degrees Fahrenheit. The facility is fire cleared maybe for two (02) non-ambulatory residents, five (5) ambulatory. Hospice waiver for six (6); one maybe bedridden bedroom #3. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. Food storage and preparation areas are clean and inaccessible to pests. Garbage cans have tight fitting covers. Cleaning supplies, pesticides or toxic cleaning supplies were stored and locked away in the kitchen below the sink. Office is located in the kitchen. Bedrooms: There were three (3) bedrooms designated for residents' use. Bedroom #1, bedroom #2, are shared used for residents, the bedrooms were properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. Bedroom #3 is private; for bedridden. Continue to LIC 809-C Bathroom #2 is across bedroom #2. There was enough clean linen available in the cabinets in the hallway. Bathroom #3 is located at the end of the hallway that is designated only for staff. Common Areas: LPA toured all common areas of the facility. These included the living room and dining area for residents. The common areas were properly furnished. Residents dining table fits enough for six (6). LPA observed common areas to be very clean and tidy. LPA observed the floors to be in very good condition. No obstructions and or tripping hazards throughout the facility. Furniture in common area was observed to be in good repair. There are no issues with Fire Clearance. Office is located beside bathroom #3. Infection control: Facility mitigation plan to make sure licensee was following current infection control recommendations. LPA obtain a copy and reviewed the infection control plan during this visit. Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. The facility does have a swimming pool it is fence up and lock. The garage is detached and is converted into a storage. Laundry service: There is enough linen available to change weekly or more if need. Cleaning supplies are being stored in a locked cabinet in the laundry area and is located outside the garage. Staff Files: LPA also conducted a file review of staff records to ensure forms and training are up to date and compliance with licensing forms. Records were checked for expired or missing certificates and clearances: LPA conducted a file review of staff for criminal record clearances and current First Aid. The administrator file was reviewed for current first aid, fingerprint clearance, administrator certificate, and HIV/AIDS and TB training. Medications are in a centrally stored and locked place, including over-the-counter medicines; medications are properly labeled and checked for expiration dates. Each centrally stored prescription and PRN medication has been logged in the medications log with proper documentation from the clients’ doctor. Proper medication dispensing instruction are followed and checked for contamination. First-aid has all proper items and is current. Continue to LIC 809-C Medications are in a centrally stored and locked place, including over-the-counter medicines; medications are properly labeled and checked for expiration dates. Each centrally stored prescription and PRN medication has been logged in the medications log with proper documentation from the clients’ doctor. Resident records were reviewed for requirements and legibility: LPA reviewed client’s files for current appraisal. Planned activities are offered. Facility is within CA code of Regulations Title 22 or Health and Safety Code. No deficiencies were found, exit interview conducted, copy of report has been issued and discussed. Bathrooms: There are two (2) bathroom designated for residents' use. The bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 116.1 degrees Fahrenheit for bathroom #1 located in the hallway in between bedroom #1 and bedroom #2.
