On 7/16/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Designee (D1) Mario Ramos . LPAs was granted entry. All 5 residents were present during inspection. Licensee Lorik Sheakalee was present during visit. LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 44 degrees F. Fire extinguisher was observed with a purchase date of: 10/17/25. Fire drill last completed on 5/1/26. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 109.2 degrees F in bathroom 1, and 117.6 degrees F in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. Melatonin 1 mg take 1 tablet orally at bedtime was not listed, labeled or initialed by staff in MAR and is not listed in Centrally Stored Medication Record. First aide kit observed with all of the required items. Samples of residents and a samples of staff files reviewed to have all the required documents. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. LPA introduced self and stated the purpose of the visit, and was greeted by Staff (S1) Joseph Whigan. LPA was granted entry. 5 residents were present during inspection. Designee Mario Ramos (D1) arrived shortly after LPA’s arrival. LPA toured facility with D1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -18 degrees F and refrigerator temperature was maintained at 40 degrees F. Fire extinguisher was observed with a purchase date of: 10/14/24. Fire drill last completed on 5/1/25. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature at a range of 109.9 and 110.8 degrees F. in 3 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. Samples of residents and a sample of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. LPAs introduced selves, stated the purpose of the visit, and was greeted by Staff (S1) Gerardo Diamat. LPAs was granted entry. All 3 residents were present during inspection. Licensee Lorik Sheakalee was called and stated unable to attend inspection. Designee Mario Ramos (D1) arrived shortly after LPA’s arrival. LPAs toured facility with D1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 8.9 degrees F and refrigerator temperature was maintained at 45 degrees F. Fire extinguisher was observed with a purchase date of: 10/12/23. Fire drill last completed on 6/5/24. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 110.4 and 109.1 in bathroom 1 and 107.9 and110.4 degrees F. in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. Allegation investigated: Staff confined resident to their room while in care. Staff did not ensure that resident's hygiene needs were met while in care

On 10/20/2023 Licensing Program Analysts (LPA's) M. Garza and M. Flores arrived at facility for an unannounced complaint visit. LPA met with Direct Care Staff, Joseph and Gamare Whigan. Licensee, Lorik Sheakalee was contacted and stated they were unavailable to come to the facility. LPA was informed Assistant, Mario Ramos would arrive some time later. LPA's completed a tour of the facility inside and out and a health and safety check on residents in care. Residents observed in common areas and in rooms. During visit LPA's completed interviews with staff and resident(s). LPA's reviewed documentation (physicians reports, needs and assessments, pre-placement appraisal). Interviews with RP and S1 indicated that R1 had hygeine needs were not met. Interview conducted with medical profession indicated that R1 was being confined to their room. The preponderance of evidence standard has been met per Title 22. The allegations listed above are SUBSTANTIATED. Deficiency cited on 9099D. 87411 Personnel Requirements-General (a) Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs…Additional staff shall be employed as necessary to perform office work, cooking, house cleaning, laundering, and maintenance of buildings, equipment and grounds. The licensing agency may require any facility to provide additional staff whenever it determines through documentation that the needs of the particular residents, the extent of services provided, or the physical arrangements of the facility require such additional staff for the provision of adequate services. This requirement was not met as evidence by: LPA interviews of RP, staff and residents. Plan of correction: Licensee to provide in-service training to all staff. In-service sign in sheet and training material to be provided to CCL by POC date. Licensee stated they will be generating a log and provide to CCL.

Type of visit: Case Management - Deficiencies

On 10/20/23, Licensing Program Analyst (LPA’s) M. Flores and M. Garza made an unannounced case management visit. LPAs were granted entry to the facility by care staff, Joseph and Gamaree Whigan. Licensee, Lorik Sheakalee, was contacted by and stated she was unable to come to the facility. LPA was informed that Assistant, Mario Ramos arrived at the facility minutes later. LPAs completed a tour of the facility inside and out and health and safety check on residents in care. Residents observed in common areas and in rooms. During the facility tour the following was observed by LPAs: 1. Lock installed on top of the entrance door. 2. Cleaning supplies unlocked in the bathroom. 3. Chemicals unlocked in the laundry room. 4. Door alarm to the sliding door turned off. 5. Side walkway was accessible to residents and observed to be unleveled and missing paver rocks.
