On 12/9/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Irma Tecomolupa. LPA was granted entry. 6 residents were present during inspection. Licensee (L1) Hripsime Makaryan arrived shortly LPA’s arrival. LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. Fire extinguisher was observed with a purchase date of: 11/11/25. Fire drill last completed on 11/11/25. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 116.4 and 116.7 degrees F. in 2 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. All residents and a sample of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 12/23/25: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. Licensing Program Analyst (LPA) J. Leffall conducted a Pre-licensing Inspection on this day of 1/21/25. LPA met with Licensee Hripsime Makaryan. A tour of the facility was conducted together. This is a new facility with no residents in care. The facility was observed to be at a comfortable temperature, clean, and in good repair. No passageway obstructions or fire hazards were observed inside or outside. Common areas were properly furnished and well-lit throughout. The dining room is equipped with a table and chairs, living room is equipped with adequate sofas and chairs for residents, adequate outside space for rest and recreational. Freezer temperature is maintained at 0 degrees F and refrigerator is maintained at 36 degrees F. Seating and shaded area provided by large patio gazeebo. Gate is self-closing and self-latching. Perishable and non-perishable food supply appeared adequate. Knives will be locked in kitchen cabinet with medications, and first aid kit. Cleaning and Chemical supplies are kept locked in the laundry room cabinet. Residents' bedrooms were observed to be adequately furnished with bed, dresser, folding chair and adequate lighting. Mattresses and linen were in good condition. Extra linen and towels are available. Carbon monoxide and smoke alarm detectors installed and operational. 2 bathrooms observed with grab bars installed in shower. Bathroom temperature ranged from 109.2 to 112.8 degrees F in both bathrooms. Toilet equipped with grab bars. Non-skid mats in place, hand soap and paper towels available for use. Trash cans with tight fitting lids are in place. Fire extinguisher was serviced and fully charged with a purchase date of 11/21/24. Complaint poster posted, resident council info posted, residents' rights posted; emergency disaster plan posted. First aide kit observed to have all of the required items. Component III was completed by Licensee. Exit interview was conducted. Pre-licensing requirements were met. An exit interview was conducted with Licensee. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information.
