On 06/4/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greet by staff Jazpherd Manuel. Administrator (A1) Shannon Steele was called and arrived shortly LPA’s arrival. 5 residents were present during inspection. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Medications were checked and observed kept locked in kitchen shelves. LPA observed MARs and samples of medications. Sharps were observed stored and locked in kitchen shelf. Fire extinguisher was observed with a service date of: 07/28/25. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested range of 107.7 and 108.6 in 2 bathrooms. Washer and dryer observed to be operational. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents’ file and a samples of staff files were reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -10 degrees F and refrigerator temperature was maintained at 40 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Medications were checked and observed kept locked in kitchen shelves. LPA observed MARs and samples of medications. Sharps were observed stored and locked in kitchen shelf. Fire extinguisher was observed with a service date of: 07/02/24. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. All 5 residents were present during inspection. LPAs toured facility with CC. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Medications were checked and observed kept locked in kitchen shelves. LPAs observed MARs and medications. Sharps were observed stored and locked in kitchen shelf. Fire extinguisher was observed with a service date of: 07/02/24. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested range between 105.4 and 105.3 degrees F. in master bathroom and 106.3 in bathroom 1. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents’ file and a sample of staff files were reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. Allegation investigated: Staff overcharging resident for cigarettes.

On 5/07/2024, Licensing Program Analyst (LPA) V. Gorban and Licensing Program Manager (LPM) S. Moua met with Administratr Sundari "Susan" Kendakur and Care Coordinator Jean to discuss the above allegations and deliver findings. The Department conducted interviews and reviewed records. Based on the records reviewed, the facility failed to provide receipts detailing the breakdown of a carton of cigarettes and how resident’s P&I funds were used to purchase the item. The Department was only provided payment receipts that does not indicate how many cartons and what type of cigarettes were purchased. The ledger provided show discrepancies because the resident was charged $32.75 in 2021, $76.91 in 2023, and then #32.75 in 2023. The preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. See citations on the attached LIC. 9099D. Plan of correction: POC – Facility administrator agrees to review the resident’s P&I fund ledgers and reimbursed the resident or their responsible party for any overpayment made during the resident’s stay at the facility. A plan as part of the POC will be submitted to the CCL office by the due date.
