Type of visit: Case Management - Annual Continuation

On 8/27/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Don Danan. LPA was granted entry. 2 residents were present during inspection and the other 2 attended Day Program. Licensee L1 Diane Receno arrived shortly LPA’s arrival. LPAs toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -5 degrees F and refrigerator temperature was maintained at 35 degrees F. Fire extinguisher was observed with a purchase date of: 4/9/26. Fire drill last completed on 8/1/26. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 119.3 and 119.4 degrees F. in 2 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. All residents and a sample of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 07.7 in bathroom 1 and 105 degrees F. in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. LPAs introduced selves, stated the purpose of the visit, and were greeted by Staff (S1) William Lacambra. LPAs was granted entry. 1 resident was present during inspection and the other 3 attended Day Program. Licensee L1 Diane Receno arrived shortly LPA's arrival. LPAs toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Fire extinguisher was observed with a purchase date of: 3/6/24. Fire drill last completed on 9/3/24. Centrally cooling system is currently under repair, however, there are adequate amount of portable air conditioning units placed around the facility. Each resident has a portable air conditioning unit in each room. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 113 in bathroom 1 and 115 degrees F. in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in kitchen cabinet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. All residents and a sample of staff files reviewed to have all the required documents. Allegation investigated: Resident sustained unexplained bruising while in care

On 04/17/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to deliver findings on the above allegation. LPA introduced myself and met with Administrator, Sara Gloria. The purpose of the vist was disclosed to Administrator. Review of resident records and interviews with Administrator revealed that on 04/10/2024, R1 had an appointment to have blood drawn. During the appointment the Lab technician made three attempts to draw blood from R1 resulting in bruising to R1's right arm. This agency has investigated the allegation: Resident sustained unexplained bruising while in care. We have found that the complaint is unfounded, meaning that the allegation was false, could not have happened and/or without a reasonable basis. An exit interview was conducted Administrator. On 10/27/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. introduce self, stated the purpose of the visit and met caregiver Rizza Receno. LPA toured facility with caregiver. Administrator Dianne Receno was called and arrived shortly. LPA toured facility with Administrator. Two resident was present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Fire extinguisher was observed with a service date of: 02/13/23. Fire drill last completed:09/06/23. The temperature is maintained for refrigerator at 36 degrees F and freezer at 0 degrees F. Medications observed locked in kitchen shelf. MARs were reviewed. All bedrooms were observed to have the required furnishings and with adequate lightening. The bathrooms were toured. Bathrooms were observed operational during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested at 107.2 degree F in the bathroom 1 and between 105.6 degree F and 106.1 degree F in master bathroom. Cleaning supplies and chemicals stored and locked in garage cabinet. Outside of facility toured and observed to be free of debris. Side gate observed self-closing and self-latching. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents’ files were reviewed to have all the required documents. Staff files were also reviewed to have current First Aid/CPR, Personnel Record, Criminal record Statement, and Health Screening. Staff were fingerprinted cleared and associated with facility.
