Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by staff and explained the reason for the visit. Facility is licensed for six non-ambulatory residents of which one may be bedridden. Facility has an approved hospice waiver for four residents and the facility has no residents on hospice during today's visit. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limed to testing all smoke detectors, testing hot water temperature in four out of four resident bathrooms. The hot water temperature measured between 106.1 and 112.4 degrees Fahrenheit and all smoke detectors were operational. The facility’s last fire drill was conducted on October 15th, 2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. LPA Mendivil observed emergency food and water in facility pantry and garage. LPA reviewed five of five staff training and fingerprint records and conducted a complete review of resident records. LPA confirmed that administrator has a current administrator certificate which expires on July 15th 2026. No deficiencies noted during today's visit. Type of visit: POC

Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced visit to the facility for the purpose of a Plan of Correction (POC) visit, based upon the deficiencies cited in LIC form 809D on 10/15/2024. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. *Deficiency cited under Title 22 Regulation 87412(c) pertaining to Staff Training has been cleared. Licensee provided proof of correction. Licensee has complied with the terms of the POC. *Deficiency cited under Health and Safety Code 1569.695(c) pertaining to Emergency Drills has been cleared. Licensee provided proof of correction. Licensee has complied with the POC. *Deficiency cited under Title 22 Regulation 87608(a)(3) pertaining to postural supports has been cleared. Licensee provided proof of correction. Licensee has complied with the POC. Licensee addressed items on advisory note issued on 10/15/2024. Licensee has been advised to maintain all items in compliance with Title 22 regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to Genesis Senior Living II. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the facility and explained the reason for the visit. Facility is licensed for six non-ambulatory residents of which one may be bedridden. Facility has an approved hospice waiver for four residents and the facility has no residents on hospice during today's visit. Administrator Noime Garsin Pabale arrived during the visit. LPA Lyman along with Administrator toured the facility at 8:38 AM. LPA toured the physical plant, checked food service, first aid kit and reviewed records. The home consists of five resident bedrooms, 2 common restrooms, 2 resident restrooms, staff room, living room, dining room, and kitchen. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident rooms are single and double occupancy. LPA observed four residents with half bed rails. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105.2 and 115.8 degrees F in all facility bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the elements including thermometer, tweezers and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Smoke detectors and carbon monoxide detectors tested operational during today's visit. Fire extinguishers are fully charged. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there are no safety concerns noted. Exit gates are unlocked, self latching and operational. LPA observed ample emergency food and water. LPA reviewed the emergency disaster plan and infection control and plans are thorough and complete. Facility provides activities in the form of exercise, music and games. CONTINUED ON LIC 809C DATED 10/15/2024. Facility does not have documentation of required quarterly emergency drills. LPA reviewed five resident files and three staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Two out of four residents with half bed rails do not have corresponding physician orders for bed rails. Staff files reviewed contained required documentation of medical clearance/ TB, CPR training and criminal record clearance. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today's visit, the following violations are being cited per California Code of Regulations, Title 22, Division 6, Chapter 8.
