Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced Required 1-year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA met with Laura Kim, business office manager for the facility, and explained the purpose of the visit. Licensee/Administrator Steve Kim arrived shortly thereafter. The following 12 (CARE) tool domains were observed and reviewed: Infection Control, Physical Plant/Environment Safety, Operational Requirements, Staffing, Personnel Records/Staff Training, Resident Rights/Information, Resident Records/Incident Reports, Food Service, Planned Activities, Incident Medical and Dental, Disaster Preparedness, and Residents with Special Health Needs. Infection Control: · Infection control plan is on file. Physical Plant/Environment Safety: · The facility is a two story building located in a residential neighborhood. The facility consists of thirty-nine (39) Bedrooms, a kitchen, dining room, library, TV room, activity room/exercise area, main office, medication room with locked storage cabinet for medications and sharps, storage closets for linen, cleaning supplies and dry food and a center courtyard with shaded area and seating for resident use. LPA toured seven (7) resident bedrooms including checking their water temperature, which all fell within the required range of 105 – 120 degrees Fahrenheit. The facility was observed to be in good repair and its signal systems were observed to be operable. Fully charged fire extinguishers are kept on the premises. Operational Requirements: · Fire clearance was approved by LA County Fire Department for a capacity of thirty-two (32) ambulatory residents, forty (40) non-ambulatory residents, six (6) of whom may be bedridden, and a hospice waiver approved for twelve (12) residents. · Care and supervision to meet the clients’ needs was observed. Staffing: · Fifty-seven (57) full-time staff members provide care and supervision to the clients. Personnel Records/Staff Training: · Seven (7) staff files were reviewed for criminal background clearance and training. · All Seven (7) staff records have a health screening with a Tuberculosis clearance, First Aid/CPR trainings that are active, and required annual trainings. · Administrator’s certificate expires on 5/20/2027. Resident Rights/Information: · Physician orders were reviewed for seven (7) resident files. · Medications were also reviewed for seven (7) residents. Resident Records/Incident Reports: · Seven (7) resident files were reviewed containing admission agreements, Physician's Report, medical/functional assessments, Needs and Services Plans, TB clearance, Appraisal/Needs and Services Plan, personal rights, medical consent, and medication records were reviewed. · All seven (7) residents reviewed did not have a documented reappraisal conducted within the past year. Food Service: · The kitchen was inspected and has sufficient supply of 2-day perishable & 7-day non-perishable food. Kitchen, food preparation area, and storage areas were observed to be clean and sanitary. Incident Medical and Dental: · All residents have are assisted with medication management by staff. Disaster Preparedness: · Emergency and Disaster Plan (LIC610E) was posted in the facility and LPA obtained a copy. · The last emergency and disaster drill was conducted on 4/3/2026. Planned Activities: · Sufficient Space is provided to accommodate both indoor and outdoor activities. · Sufficient equipment and supplies are provided to meet the requirements of the activity program. Residents with Special Health Care Needs · There is an adequate number of staff to support each resident’s physical, social, emotional, safety and health care needs as identified in his/her appraisal. Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiency observed during the visit is documented on the LIC809D page. Aug 26, 2025Complaint investigation reportUnsubstantiated
Allegation investigated: The facility only admits residents based on race.
Licensing Program Analyst (LPA) Erik Zaragoza conducted a subsequent complaint visit to address the allegation listed above. LPA was met with Steve Kim, Administrator of the facility, and was granted entrance. The investigation consisted of the following: During the initial visit conducted on 2/23/2024, LPAs Erik Zaragoza, Daniel Konishi, and Christian Gutierrez obtained staff and resident rosters, conducted a tour of the facility interviewed Residents #1 – 6 (R1 – R6), Staff #1 – 4 (S1 – S4), and obtained the resident file for Resident #7 (R7). Since the initial visit LPA interviewed Residents #8-9 (R8 – R9), and Staff #5 (S5). LPA attempted to interview R7, however they were not available to be interviewed. During today’s visit, LPA will deliver the findings of the investigation. Unsubstantiated The investigation revealed the following: Regarding the allegation that “The facility only admits residents based on race," it is alleged that the facility only admits Korean applicants into the facility and refuses to admit any applicants who are of non-Korean descent. During interviews with the staff of the facility, none of them corroborated the allegation that they reject people applying for admission into their facility based on race. One of the staff interviewed, S1, explained that they have admitted non-Korean residents into the facility in the past, and additionally they currently employ one (1) staff member who is not Korean. S1 further explained that the facility is open to admitting any applicant regardless of their ethnic background, and that they have admitted black and Hispanic residents in the past. During an interview with S2, who assists in the admissions process for the facility, they stated that individuals from any ethnic background are welcome to apply for admission into the facility. S2 also stated that if a non-Korean applicant was seeking admission into the facility, they would not turn the applicant away. S2 also added that they would also offer accommodations to the resident if they wished to proceed with admission, including offering alternative meals to the predominantly Korean dishes that are served at the facility. During interviews with the residents, none of them corroborated the allegation that they believe the staff discriminate against non-Korean applicants based on their race. None of the residents interviewed stated that they believe the facility staff uses race as a factor in whether or not they will admit an individual into the facility. One of the residents interviewed stated that it does not matter if an applicant is Korean, Hispanic, or of any other race, the staff does not discriminate against anyone seeking admission into the facility, and they do not treat anyone differently because of their ethnic background either. During record review of the last non-Korean resident’s file, R7, LPA confirmed that this former resident was of non-Korean descent. LPA also determined that the facility currently employs a staff member who is of non-Korean descent as well. Based on statements and interviews conducted with staff, clients, review of client files and facility file records, there was not enough supportive evidence to concur with the reported allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Mayra Cota conducted an unannounced required annual visit. LPA met with Steve Kim, Administrator and explained the reason for the visit. The facility is licensed to serve adults in the age range of (60) and over; approved for (32) ambulatory only and approved for (40) non-ambulatory, of which (6) may be bed ridden. Facility is approved for Hospice waiver for (10). The facility is operating within the scope of its license. The facility is a two story building located in a residential area of Los Angeles and contains the following: Thirty-nine (39) Bedrooms, (42) bathrooms, kitchen, dining room, library, TV room, activity room/exercise area, main office, medication room with locked storage cabinet for medications and sharps, (5) storage closets for linen, cleaning supplies and dry food, (2) laundry rooms and a center courtyard with shaded area and seating for resident use. LPA observed the following during today's visit: Facility was observed to be clean and walkways were clear of obstructions and debris. Resident bedrooms have the required furniture such as bed frames, dressers, lamps/overhead lighting, and chairs. Bedrooms also have sufficient closet space. Resident beds have the required linen and and beds with adaptive handrails have physician order documentation on file. ***Continues on LIC 809-C Resident bedrooms are equipped with call buttons in the bathroom and at bedside. During visit, call buttons were tested and observed operable. Resident rooms with individuals requiring oxygen have the required postings. Six (6) resident bathrooms were observed clean and sanitary. Bathrooms have a working toilet, wash basin, shower, grab bars and nonskid mats. The water temperature was tested in resident bathrooms and measured within the required 105 - 120 degrees F. Kitchen was inspected, however, storage cabinets were observed to have grease stains on the exterior and inside the storage area. Appliances were observed to be clean and working properly. Food inside refrigerators was observed to be properly stored and dated. Sharps are kept locked in a kitchen drawer and are inaccessible to clients. Sufficient supply of 2 day perishable and 7 day non-perishable food was observed and additional emergency food supply was observed in the dry food storage room. Food in the refrigerators and dry food storage room is labeled and kept within expiration limits. The dining area has enough seating for residents and furniture was observed to be in good repair. Rodent droppings were observed in storage cabinets during visit. Facility has a contract with outside vendor for food delivery. Facility manages food preparation and distribution to residents. Menus were observed posted in the dining area. The center courtyard was inspected and observed to be clean. Shaded areas were observed and outdoor furniture is in good repair. Garden is maintained and free of dry plants and fallen foliage. Resident medications and MARs were reviewed and found to be properly dispensed per physician orders and documented accordingly. Eight (8) resident and (6) staff files were reviewed and contained all proper documentation. Fire extinguishers were observed throughout the facility and were charged and last inspected on 12/20/24. Fire alarm system was last inspected on 6/1/25 and the facility conducted their last emergency drill on 4/25/25. Facility is equipped with a sprinkler system and resident rooms are equipped with combination smoke and carbon monoxide detectors which were tested in (4) resident bedrooms and were working properly. Per California Code of Regulations, Title 22, and California Health and Safety Code, deficiencies are cited today (see LIC 809-D). Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) Elizabeth Irra conducted an annual inspection visit. LPA was allowed entry by Joan Baek. LPA discussed the purpose of today’s visit. Ms. Kim (Manager) arrived at approximately 9:35 A.M.. Steve Kim (Administrator) arrived at approximately 9:40 A.M.. This facility is approved for (32) ambulatory and (40) non-ambulatory clients (6) of which may be bedridden and (10) of which may be under hospice. Total capacity is: 72. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Facility does has an Infection Control Plan in place. Operational Requirements: Facility is adhering to the operational requirements. Physical Plant & Environment Safety: LPA toured facility grounds. Fire alarms and Carbon Monoxide detectors were observed. Fire extinguishers are located throughout the property and appeared to be full (last serviced 12/2023). Signal system observed. Hot water temperature measured within regulations. The hot water supply measured at the following temperatures: 112.0* in room #110, 115.0* in room #114, 108.0* in room #208 and 113.5* in room #222 Bathrooms had non-skid surfaces and grab bars. Staffing: Facility is adhering to staffing requirements. Personnel Records-Training: Staff files are maintained at the facility. LPA reviewed staff files for Facility Administrator/S-1 through Staff #5 (S-5). Staff have current First Aid/CPR certification. Staff have their Health Screening and Tuberculosis Screening on file. Refer to LIC 809C for the continuation of this report. Resident Rights-Information: Resident rights are posted and included in Resident files. Planned Activities: Activity schedule is posted. Food Service: There are sufficient food supplies of 2-day perishable and (1) week of non-perishable items. The food is properly stored in the refrigerator. Posted menu observed. Pesticides and cleaning supplies are kept away from the food preparation areas. Kitchen is kept clean and free from rodents and other vermin. Plates, cups and utensils are kept cleaned and stored properly. Dining areas have adequate seating. Resident Records-Incident Reports: LPA reviewed Resident files for Resident #1 (R-1) through Resident #7 (R-7). Resident files are maintained at the facility. Resident files have the required documents Disaster Preparedness: The facility has a Disaster Preparedness plan in place. Residents with Special Health Needs: Facility is adhering to residents with special health needs. Health Related Services/Incidental Medical Services: The medications are stored and locked inside the medication room.
