Type of visit: Required - 1 Year

On 04/10/2026 Licensing Program Analyst arrived at the facility to complete an unannounced annual visit. LPA met with Care Giver, Butad Boniza, explained reason for visit and was permitted entry into the facility. Licensee, Jerome Esteban was contacted and stated they were unavailable to come to the facility. Licensee gave permission for LPA to complete visit with Care Giver, Butad. LPA completed a tour of the facility inside and out. Due to Care Giver not having the key LPA was unable to check the 2 sheds in the back yard. A health and safety check was completed on residents in care. 2 residents were present during todays visit. Pathways and doors were clear and free from obstruction. Facility was without odor. Common areas were adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced on 08/05/2025. Last fire drill on conducted on 4/5/26. Resident rooms observed to have the required furnishings and with adequate lighting. Medications were located in locked cupboard in the kitchen. LPA observed sufficient seating under covered patio areas. Due to time constraints LPA will return for an annual continuation visit. If required, deficiencies will be issued at that time. LPA requested the following documents to be submitted to CCL by 04/17/2026: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-E), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. Type of visit: Required - 1 Year

On 4/25/25 Licensing Program Analyst (LPA) M. Garza arrived unannounced for an annual visit. LPA met with Licensee, Jerome Esteban. LPA explained reason for visit and was permitted entry into the facility. Currently there are no residents that are residing in the facility. Facility was toured inside and out. Pathways and doors were clear and free from obstruction. Facility was clean and without odor. Common areas were clean, adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors were present and operational at time of visit. Fire extinguisher last serviced 08/13/2024. Last fire drill on 3/11/25. Resident rooms observed to have the required furnishings and with adequate lighting. Linen supplies are kept in linen closets. Sharps, chemicals and medications were locked. LPA observed sufficient seating under covered patio areas. Sample staff files reviewed. The following issues were observed during todays visit: Pre-admission appraisals and reappraisals were not completed in R1's file reviewed. TV's provided per Title 22. Annual training for staff was not completed for 2 of 2 staff. Deficiency cited per Title 22 on 809D. LPA requested the following documents to be submitted to CCL by 5/2/25: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-E), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. Exit interview completed with Licensee, Jerome. Due to IT issues a copy of this report, deficiency, TV's and appeal rights will be provided via email. Type of visit: Prelicensing

On 5/8/2024 Licensing Program Analyst (LPA) M. Garza arrived to complete an announced pre-licensing visit. LPA met with Licensee, Jerome Esteban. LPA was permitted entry into the facility. This visit is being conducted as a follow up visit. Corrections were needed in order for the facility to be licensed. The following issues were observed during the last visit: non-skid mats missing from bathrooms and visiting/complaint policy postings missing. LPA toured the facility and observed the restrooms with non-skid mats. Visiting and complaint postings were observed posted in entry way of the facility. The corrections have been made as required.The Component III was conducted with Licensee during 5/6/24 visit. LPA will send notification that facility is ready to be licensed. On 5/6/24 Licensing Program Analyst (LPA) M. Garza arrived for an announced pre-licensing inspection visit. LPA was met by Licensees, Jerome Esteban and Maricris Esteban. LPA introduced self, reason for visit and was permitted entry into the facility. Facility is in the process of pre-licensing. Currently there are no residents residing at the facility. Fire clearance was approved for 6 non-ambulatory residents. Tour of facility inside and out was completed. During the last visit on 4/23/24, the following issues were observed; 2 of 4 bedrooms observed without required lamps, night stands and dressers. Outside walkway observed to have tripping hazard with a 2-4 inch drop to level ground. Master bedroom door has a 2 inch drop that is a potential tripping hazard. 2 of 2 side gates are not self latching and 1 of 2 gates was missing latch. Bathrooms missing non-skid mats. Visiting policy and complaint policy postings missing. Corrections have been made to all with the exception of non-skid mats missing from bathrooms and visiting/complaint policy postings missing. Component III completed during this visit. A return visit has been scheduled for 5/8/2024. At this time the facility is not ready to be licensed. Exit interview completed with Licensee, Jerome and Maricris. Due to technical issues a copy of this report will be emailed for signature. On 4/23/24 Licensing Program Analyst (LPA) M. Garza arrived for an announced pre-licensing inspection visit. LPA was met by Licensees, Jerome Esteban, Maricris Esteban and Administrator, Sally Esteban. LPA introduced self, reason for visit and was permitted entry into the facility. Facility is in the process of pre-licensing. Currently there are no residents residing at the facility. Fire clearance was approved for 6 non-ambulatory residents. Tour of facility inside and out was completed. All required postings are posted. Common areas observed with adequately furnishings and lighting. LPA observed an extra supply of linens and personal hygiene/grooming products. Kitchen observed with dishes, plates and utensils. Cleaning supplies/chemicals observed in a locked garage cabinet. Medications will be kept in a locked file cabinet off the kitchen. Fire extinguishers present and last serviced 8/16/23. Smoke and carbon monoxide detectors are present and operational at time of visit. Exits free of obstructions. Facility has a functioning phone. Phone number is: (559) 458-5206. First aid kits observed with required items. Water temperature measured at 108.8 degrees F in master bathroom. The following issues were observed during todays visit. 2 of 4 bedrooms observed without required lamps, night stands and dressers. Outside walkway observed to have tripping hazard with a 2-4 inch drop to level ground. Master bedroom door has a 2 inch drop that is a potential tripping hazard. 2 of 2 side gates are not self latching and 1 of 2 gates was missing latch. Due to corrections being needed, the Component III was not conducted during this pre-licensing visit. Administrator will contact CCL and reschedule for a re-inspection once corrections have been made. At this time the facility is not ready to be licensed. Facility Type: RCFE Application Type: Initial Capacity: 6 Census (if any clients in care): 0 COMP II Participants: Jerome Esteban - Applicant and Sally Esteban - Administrator Interview Method: Virtual interview (Skype, Go To Meeting, etc) On 03/28/2024, applicant/administrator participated in COMP II. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
