Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced Required 1-year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA met with Thomas Trice, administrator for the facility, and explained the purpose of the visit. The following 12 (CARE) tool domains were observed and reviewed: Infection Control, Physical Plant/Environment Safety, Operational Requirements, Staffing, Personnel Records/Staff Training, Resident Rights/Information, Resident Records/Incident Reports, Food Service, Planned Activities, Incident Medical and Dental, Disaster Preparedness, and Residents with Special Health Needs. Infection Control: · Infection control plan is on file. Physical Plant/Environment Safety: · The facility is a single-story home located in a residential neighborhood. The facility consists of four (4) resident bedrooms, one (1) staff room, a laundry room, a kitchen, dining room, a living room. The facility contains two (2)) bathrooms including checking their water temperature, which both fell within the required range of 105 – 120 degrees Fahrenheit. The carbon monoxide detector is operational in the home. A fully charged fire extinguisher is kept on the premises. Operational Requirements: · Fire clearance was approved by LA County Fire Department for a capacity of six (6) non-ambulatory residents, one (1) bedridden resident and a hospice waiver approved for four (4) residents. · Care and supervision to meet the clients’ needs was observed. Staffing: · Seven (7)) full-time staff members provide care and supervision to the clients. Personnel Records/Staff Training: · Five (5) staff files were reviewed for criminal background clearance and training. · All five (5) staff records have a health screening with a Tuberculosis clearance, First Aid/CPR trainings that are active, and required annual trainings. · Administrator’s certificate expires on 12/1/2026. Resident Rights/Information: · Physician orders were reviewed for five (5) resident files. · Medications were also reviewed for five (5) residents. Resident Records/Incident Reports: · Five (5) resident files were reviewed containing admission agreements, Physician's Report, medical/functional assessments, Needs and Services Plans, TB clearance, Appraisal/Needs and Services Plan, personal rights, medical consent, and medication records were reviewed. Food Service: · The kitchen was inspected and has sufficient supply of 2-day perishable & 7-day non-perishable food. Kitchen, food preparation area, and storage areas were observed to be clean and sanitary. Incident Medical and Dental: · All residents have are assisted with medication management by staff. Disaster Preparedness: · Emergency and Disaster Plan (LIC610E) was posted in the facility and LPA obtained a copy. · The last emergency and disaster drill was conducted on 7/15/2026. Planned Activities: · Sufficient Space is provided to accommodate both indoor and outdoor activities. · Sufficient equipment and supplies are provided to meet the requirements of the activity program. Residents with Special Health Care Needs · There is an adequate number of staff to support each resident’s physical, social, emotional, safety and health care needs as identified in his/her appraisal. Administrator was informed that the facility has past due licensing fees for the past 5 years. Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies were observed during the visit. Licensing Program Analyst (LPA) Galarza conducted an unannounced annual inspection visit. The purpose of the visit was explained to caregiver Salvador Huerta. Administrator Thomas Thrice arrived shortly after. The facility serves elderly residents ages 60 and older. The following were observed/inspected: Infection Control: The Infection Control Plan was reviewed and includes environmental cleaning and disinfection activities. Facility has sufficient Personal Protective Equipment. Operational Requirements: A Dementia and hospice waiver for 4 residents has been approved. A fire clearance for 6 non-ambulatory and 1 bedridden resident is in place. Facility does not handle resident P & I monies. Liability Insurance in the amount of at least ($1,000,000) per occurrence and ($3,000,000) in total annual aggregate is current with an expiration date of 10/21/2025. Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood. The facility is a single story home located in a residential neighborhood consisting of 4 resident bedrooms (2 shared and 2 private), 2 bathrooms, living room, dining room, kitchen, den, live-in staff room, outdoor covered patio area, carport, and detached laundry room. Smoke and carbon monoxide detectors were tested and are operational. The facility has one (1) fire extinguisher. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. The facility has a 1st Aid Kit and Manual. Cleaning supplies, knives, and toxic substances are inaccessible to residents. Exit doors are free of any obstruction and there are no pools or large bodies of water. The last Emergency Disaster drill was conducted on 6/28/2025. Oxygen was in use in room #1, but no sign was posted. Staffing: A total 7 staff members provide care and supervision to the clients. Personnel Records/Staff Training: Administrator certificate expires 12/1/2026. Staff have criminal background clearance and training. Four (4) staff files were reviewed. Proof of staff training, health clearance, and 1st Aid/CPR training is current. Resident Records/Incident Reports: A total of six (6) resident files were reviewed. They contained admission agreements, Physician's Reports, Appraisals, TB clearance, Physician's Orders, medical consent. Medication Administration records are in place. RCFE and Ombudsman complaint posters are posted. Planned Activities: Sufficient space to accommodate both indoor and outdoor activities was observed. The facility does not have a Resident Council. Food Service: Sufficient food supply is stored in the kitchen and pantry areas consisting of: 2-day perishables, 7-day non-perishables, and emergency food supplies. Residents have modified diets. Incident Medical and Dental: Centrally stored resident medications were reviewed. 30-day supply of medications was observed. Medical and dental transportation is provided by family. Disaster Preparedness: Emergency and Disaster Plan LIC 610E was reviewed. Facility has a First Aid Kit and Manual. Residents with Special Health Needs: Two (2) resident receives hospice services and two (2) resident receives home health services. No residents have prohibited health conditions. Bed rails, hoyer lifts, and special equipment are used by residents for mobility assistance. Pursuant to Title 22, a citation was issued. Type of visit: Case Management - Health Checks

LPA Nicol Wesley conducted an unannounced case management visit and met with Hanna Villarreal and Salvador Huerta and discussed the purpose for todays visit. During the visit Administrator Thomas Trice arrived and joined the visit. During the visit LPA Wesley toured the physical plant, checked the food, view the medication, residents records, staff records and residents rooms. LPA Wesley received a copy of the staff roster and resident roster. Hot water was tested and measured 110.5 degrees F. The facility is clean, warm, and there are six residents in place. Everyone appears to be doing well. There are no deficiencies cited per the Title 22 regulations or the Health and Safety codes.the state’s words, verbatim · CDSS document, Mar 14, 2025

Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Jose Villalobos conducted an unannounced Required- 1 year visit to continue and complete using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA met with Administrator Thomas Trice and the purpose of the visit was discussed. The inspection was conducted to completed the remaining (6) domains of the CARE tools: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were observed. Visitors are no longer screened for COVID-19 or required to sign in. The facility has an Infection Control Plan and COVID-19 Mitigation Plan. Operational Requirements: The facility has a Dementia Waiver in place. A Hospice Waiver for 4 is approved. A fire clearance for 6 non-ambulatory adults 60 and over; of which one (1) may be bedridden in room 3 only. Required Liability Insurance is in place. A surety bond is not applicable. Facility does not handle resident's money. Incident Medical and Dental: LPA reviewed medications for (6) Residents. 30-day supply of resident medications were observed. Centrally Stored Records for medications are kept. Medication stored matches the medication record for each resident. Medical and dental transportation is provided by family members. Residents with Special Health Needs: Four (4) residents receive home health services. Postural support observed for one resident. Physician order is on file. Two (2) residents had full rails. Both residents are on Hospice. Total of (2) Residents are on hospice. No residents have prohibited health conditions. Staffing: Sufficient caregiver staff provide care and supervision to the clients. Personnel Records/Staff Training: Administrator certificate expires 12/1/2024. Personnel files were reviewed. LPA reviewed a total of (5) Staff fIles including the administrator. LPA observed required documents for each.Proof of staff training was reviewed. Current 1st Aid/CPR records are current. No deficiencies are being cited on todays visit. The following CARE tools domains were utilized during the inspection. Operational Requirements: The facility has a Dementia Waiver in place. A Hospice Waiver for 4 is approved. A fire clearance for 6 non-ambulatory adults 60 and over; of which one (1) may be bedridden in room 3 only. Required Liability Insurance is in place. A surety bond is not applicable. Facility does not handle resident's money. LPA still to complete this section at a later date. Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood consisting of 4 resident bedrooms (2 shared and 2 private), 2 bathrooms, living room, dining room, kitchen, den, live-in staff room, outdoor covered patio area, carport, and detached laundry room. The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. Kitchen drawers containing knives/sharp objects were locked and inaccessible to residents in care. The facility has one (1) fully charged fire extinguisher and a fire pull alarm. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Resident Records/Incident Reports: A total of six (6) resident files were reviewed containing admission agreements, Physician's Reports, Appraisals, TB clearance, COVID-19 vaccine cards, Functional Capability Assessment, and emergency information. Residents Right-Information: RCFE complaint poster and Personal rights were observed and its posted near the entrance. Continued on LIC 809-C Planned Activities: Sufficient space to accommodate both indoor and outdoor activities was observed. Indoor and outdoor activities are performed. The facility does not have a Resident Council. Food Service: Sufficient food supply is stored in the kitchen and pantry areas consisting of: 2-day perishables, 7-day non-perishables, and emergency food supplies. Modified diets in place. \ Disaster Preparedness: Emergency and Disaster Plan LIC 610E is in place and was posted today. There are currently six (6) Dementia residents 60 years and older residing in the facility. Two (2) residents receives home health, and two (2) are receiving hospice services. Due to time constraints, LPA to return at a later date to complete the inspection. Six (6) of Twelve (12) Domains are still needed to complete inspection.
