Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently 31 residents in care. Facility approved/cleared for 62 non-ambulatory and hospice waiver for 16. LPA and Administrator toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Facility has a board of residents with modified diets for kitchen staff to be aware. During tour LPA observed residents interacting in a daily morning stretching routine. Facility has daily activities scheduled throughout the day for residents to participate. Facility has a generator surrounded by secured fencing. All rooms were furnished per regulation. All rooms were in good repair. LPA and administrator observed feces smeared on a common chair outside a residents room (Technical Violation). Extra hygiene products and linens were available. Water temperature in sinks accessible to residents in care were measured at 109.4, 126.3, 108.3 and 109.9 degrees F, administrator turned down the water heater near the room reading 126.3 degrees F. Fire extinguishers were last inspected October 2025. Facility has a 5 year testing for auto sprinkler system that was tested by the State Fire Marshall on 04/07/2023. Facility has emergency supplies located in a hallway closet. Medications were found to be centrally stored. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. Facility conducts quarterly emergency disaster drills, with the last one being conducted 12/08/2025. continued on LIC809C LPA conducted a review of 8 resident records. All records had the required documentation. LPA conducted review of 6 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file. Administrator certificates are current for both Rachel Hylton (#7011341740; 03/04/2024 - 03/04/2026) and Nancy Morquecho Bravo (#7018628740; 10/05/2025 - 10/04/2027). No deficiencies cited during today's inspection. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Loera arrived unannounced to conduct a Case Management - other and met with Administrator, Rachel Hylton and Administrator Nancy Morquecho. The purpose of the visit was to provide information regarding regulations 87615- Prohibited Health Conditions, 87609- Allowable Health Conditions and the Use of Home Health Agencies, and 87616- Exceptions for Health Conditions. LPA discussed with administrators the above regulations as facility is not allowed to retain or accept a resident with a stage 3,4, and/or unstageable wound as per regulation 87615. Facility is required to have all home health records on file in the facility as per regulation 87609. LPA provided administrators with copies of regulations 87615, 87609, and 87616. Type of visit: Required - 1 Year

01/14/2025, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. There are currently 32 residents in care. Facility approved/cleared for 62 non-ambulatory and hospice waiver for 16. At approximately 1:00pm, LPA and Administrator toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner being labeled and dated. Medications were found to be centrally stored. All rooms were equipped with lighting, night stand, and chest of drawers. All rooms were in good repair. Extra hygiene products and linens were available for residents in care. Water temperature in sinks accessible to residents in care were measured at 114.0 and 116.4 degrees F which is within the range of 105 to 120 degrees F. During the walk through LPA observed residents to be participating in activities. Facility has a calendar for daily activities. Fire extinguishers were last inspected 10/2024. Facility has a generator surrounded by a fence. LPA observed outdoor patio to have caution tape around as it is currently under construction due to a recent storm eroding the hillside. Facility has emergency supplies located in a hallway closet. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. Facility conducts quarterly fire and disaster drills with the last one being conducted on 12/10/2024. LPA conducted a review of 8 resident records. All records had the required documentation. LPA conducted review of 8 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file. continued on LIC809-C No deficiencies cited during today's inspection. Type of visit: Required - 1 Year

At approximately 9:00 AM Licensing Program Analyst (LPA) Helena Rummonds arrived unannounced to conduct an annual required inspection and met with Administrator, Nancy Morquecho. LPA and Administrator discussed the purpose of the visit. LPA and Administrator initiated a tour of the facility around 9:15 AM and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in sinks accessible to residents measured at 112, 107, 113, 112, and 118 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cleaning supplies were stored in closet in kitchen, separate from food storage. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Medications were centrally stored. LPA observed the medication room door to be open leaving medications accessible to residents in care. Administrator immediately closed and locked the medication room door. Emergency food and water supplies are rotated to keep them from expiring, and are stored in the facility kitchen. Personal Protective Equipment is stored in an outdoor storage shed. Fire extinguishers were last serviced November 2023. Facility has combination Smoke/ Carbon Monoxide detectors and a sprinkler system which was last inspected by the fire department on 12/13/2023. Most recent fire/disaster drill was conducted on 11/10/2021. LPA confirmed with Administrator that drills are to be conducted on a quarterly basis, as per Title 22 regulations. Continued on LIC809-C Continued from LIC809 5 staff files and 5 resident files were reviewed. Staff have required First Aid and CPR certificates. Training records were reviewed. Administrator Certificate for Co-Administrator, Rachel Hylton (6038480740), is up to date and expires 03/03/2024. Medications and medication records were reviewed. LPA observed medications to be pre-poured up to one day ahead. LPA had a conversation with Administrator and Med Tech on duty reminding them that medications are to remain in the original container in which the medication was received in until administration. Per conversation with Administrator, there are currently 8 residents in the facility who are bedridden. Facility currently has a fire clearance for 0 bedridden residents. LPA is requesting the following to update the facility fire clearance: -Updated facility sketch indicating which bedrooms are for bedridden residents -Updated LIC200 requesting bedridden fire clearance in section 1b LPA is requesting the following documents to be submitted to CCL by 02/26/2024: LIC 500 Personnel Report Liability Insurance LIC 9020 Resident Roster LIC308 Designation of Facility Responsibility Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Exit interview conducted.
