Licensing Program Analyst (LPA) Michael Tea conducted an unannounced annual inspection of the facility on this date. The purpose of the visit was to complete the required annual inspection. LPA Tea was greeted by caregiver staff, granted entry, and explained the purpose of the visit. Administrator (AD) Gladys Ambion arrived shortly thereafter to assist. The facility is licensed to serve six non-ambulatory residents and holds a hospice waiver for six residents. At the time of the visit, there were four residents in care, two of whom were receiving hospice services. LPA Tea reviewed four resident files and two staff files. All files were complete and contained the required documentation. The Administrator’s certificate is valid through July 19, 2027. LPA Tea, accompanied by AD Ambion, conducted a tour of the facility. The physical plant includes six resident bedrooms, a staff room, six and a half bathrooms, a living room, family room, dining room, office area, kitchen, and an attached garage. Smoke detectors and carbon monoxide detectors were observed in common areas and bedrooms and were operational at the time of inspection. Resident bedrooms were properly furnished with appropriate beds, linens, and sufficient storage space. Bathrooms were inspected and found to be in good condition, with functioning toilets and faucets, secure grab bars, and showers free of mold and mildew. Water temperature measured approximately 118°F. Adequate supplies of towels, toiletries, and personal hygiene items were available. (Annual Inspection continued on LIC809C) Common areas were clean, organized, and free of hazards. Doorways and exits were unobstructed. The first aid kit was complete and included all required items such as bandages, dressings, tweezers, a thermometer, and scissors. The kitchen was inspected and found to be clean and fully operational. Perishable and non-perishable food supplies were sufficient. Sharps were stored in a locked kitchen drawer, and cleaning supplies and other toxins were secured and inaccessible under the sink and in the garage. Fire extinguishers throughout the facility were fully charged. The most recent disaster drill was conducted on March 20, 2026. Outdoor areas were also inspected. The backyard includes a shaded patio with ample seating for residents. There are two self-latching exit gates on either side of the home, both functioning properly. The outdoor space includes a garden area where residents may spend time and enjoy the environment. Emergency food and water supplies were observed in both the kitchen and garage. Activities are provided based on residents’ preferences and health conditions, including options such as karaoke, board games, and light outdoor exercise. At the time of the visit, residents were observed resting comfortably in their rooms. Medication storage and administration were reviewed. Medications are stored in a locked cabinet in the kitchen and are administered in accordance with physician orders. LPA Tea conducted interviews with residents regarding their care and spoke with staff regarding services provided. Based on observations made during today’s inspection, no deficiencies were cited in the areas evaluated in accordance with Title 22, Division 6 of the California Code of Regulations. This report was reviewed with the facility. Copies of LIC809, LIC809-C, LIC858, and LIC859 were provided. Type of visit: Collateral

On October 16, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced Collateral visit to the facility. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Direct Care Staff (DCS) Kim Miranda was notified and later arrived to assist with today's visit. The Collateral visit is being conducted in conjunction with complaint 22-AS-20220120123059 for the previous licensed facility at this location, Paradise for the Elderly 306002822. The previous licensed facility at this location was closed as of April 26, 2023. During today's visit, LPA accompanied by the DCS, conducted a tour of the physical plant of the facility. LPA observed the facility to be clear of any hazards. No health or safety concerns were observed. LPA conducted resident and staff interviews. LPA additionally collected pertinent documents to the complaint such as the current resident roster, current staff roster, and resident records. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 1:00 PM, LPA Tea was greeted and granted entry into the facility by caregiver staff and explained the reason for the visit. Administrator (AD) Gladys Ambion arrived shortly to assist with the visit. Facility is licensed for six non-ambulatory residents with a hospice waiver for six. Currently there are five residents, of which three are on hospice during today's visit. At 1:20 PM, LPA Tea reviewed five resident files and three staff files. Resident files and staff files contained all required documentation. Administrator certificate expires on July 19, 2025. LPA Tea along with AD Ambion toured the facility at 1:47 PM. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of six resident bedrooms, staff room, six and a half bathrooms, living room, family room, dining room, office area, kitchen and an attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 122.3 F degrees and 125.6 F degrees. The administrator did adjust the water temperature and signs were posted around the sinks to caution about the hot water. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, and doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply were checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen drawer. LPA also observed toxin substances to be secured and locked and inaccessible to clients underneath the kitchen Annual inspection continued on LIC809-C sink and the garage. Fire extinguishers throughout the facility are fully charged. The facility’s last disaster drill was conducted in March 18, 2025. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating with shade and there are two exit gates on both sides of the house that are self-latching and operational. The backyard has a shaded patio area for clients to sit. There are various plants in the backyard garden, sometimes the residents come outside to enjoy the weather. LPA observed emergency supplies, food and water in the garage and kitchen. Activities are based on resident’s personal preference or health condition. Facility provides activities like karaoke and board games. At the time of the visit, LPA observed residents relaxing in their rooms. LPA then reviewed medication storage and administration. Medications are stored in a locked cabinet in the kitchen area. Medications are being administered per physician order. However, there was a minor error with evening medication given on the MAR record. LPA interviewed clients regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Gladys Ambion and explained the reason for the visit. The Administrator's, Administrator's Certificate expires July 19, 2025. The last emergency disaster (fire) drill took place on March 12, 2024. Facility is a one story home with, 7 bedrooms, 6 bathrooms living room, eat in kitchen, dining room, family room and an attached 3 car garage that is used for storage. There are 2 extra rooms by the garage that are being used as an office. There is also a detached guest house that is currently being used for storage. LPA observed the See Something Say Something Poster posted in the living room. LPA observed the fireplace in the living room is screened. Facility phone number is 657-251-0408. LPA and Administrator toured the facility. LPA observed the kitchen is clean and organized. LPA observed the stove lights unassisted. There is a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen drawer. Soaps and chemicals are kept locked under the kitchen sink. LPA observed the medications are kept locked in a storage cabinet in the kitchen. Smoke detectors/carbon monoxide detectors tested operational. LPA observed the fire extinguishers are fully charged. Hot water in all bathrooms measured 108.3 degrees Fahrenheit. All bathrooms were clean and operational. LPA observed all resident rooms had the required furnishings. LPA observed the inside of the facility is clean and free of obstacles and hazards. LPA and Administrator toured the backyard. No bodies of water observed. LPA observed the detached guest house and storage shed are used for storage. The path to both exit gates on each side of the house are clear and free from obstruction. Both exit gates are latched and operational. No obstacles or hazards observed in the backyard. LPA inspected the first aid kit. The first aid kit had all the required elements. LPA reviewed 5 resident files, no discrepancies observed. LPA reviewed 5 resident medication and medication records, no discrepancies observed. LPA reviewed 3 staff files, no discrepancies observed. No deficiencies observed during the visit. No deficiencies are being cited as a result of this visit. LPA consulted with Administrator concerning reporting requirements and file requirements.
