Allegation investigated: - Facility staff did not provide continence care to resident - Facility verbally aggressive to resident
On this day, Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit to conclude and deliver findings for a complaint investigation. LPA Tea was greeted and granted entry by facility staff and informed them of the purpose of the visit. Assistant Administrator (AA) Dorbin Santos arrived shortly to assist with the visit. The Department received a complaint on November 14, 2025. During the investigation, LPA Tea conducted interviews with facility staff, residents, and witnesses, and reviewed facility records, resident documentation, and other relevant information. It was alleged that facility staff did not provide continence care to resident. Based on interviews conducted, AA Santos and two caregiver staff stated that residents are provided with regular continence care, (Complaint report continued on LIC9099C) Unsubstantiated including frequent diaper checks and changes throughout the day. Staff reported that Resident 1 (R1) and other residents are checked at routine intervals, including morning, mealtimes, and evening, and are changed as needed. Staff also stated that they assist residents with repositioning to help prevent skin breakdown. One resident who shares a room with R1 reported that staff provide good care and that residents are not left in soiled diapers. However, another resident reported that they were not always changed in a timely manner and experienced a diaper rash that developed into a fungal infection. Additionally, one witness stated that while the facility provides general care, it may not be adequate for continence needs. It was alleged that facility is verbally aggressive to resident. Interviews with three staff indicated that staff do not yell at or verbally mistreat residents. Staff reported that one staff member speaks in a louder tone due to hearing difficulties, which may be perceived as yelling. Staff stated they were not aware of any verbal abuse occurring at the facility. Interviews with residents revealed that two out of three residents stated staff are kind and respectful, and they did not recall any incidents of verbal aggression. One resident reported that a staff member made inappropriate comments regarding their size and weight. A witness interviewed did not observe any verbal aggression and only reported hearing about the concern from R1. Based on observations, interviews conducted, and records reviewed, conflicting information was obtained during the investigation, and there is insufficient evidence to support or refute the allegations. Therefore, the allegations are determined to be UNSUBSTANTIATED, meaning that although the allegations may have occurred or may be valid, there is not a preponderance of evidence to establish that the alleged violations occurred. No deficiencies were cited at this time. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPA Tea was greeted and granted entry into the facility by caregivers and explained the reason for the visit. Lead Caregiver (LC) Frederic Espiritu arrived shortly to help with the visit. Facility is licensed for six non-ambulatory residents, with a hospice waiver for two. Currently there are four residents, of which two are on hospice during today's visit. LPA Tea along with caregivers toured the facility. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of four resident bedrooms, three full bathrooms, living room, dining and kitchen area, and a caregiver area. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. The bathroom flooring in Room #3 was unstable, and LPA observed loose floor tiles. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 109.9 to 113.7 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards; doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen cabinet. LPA also observed toxin substances locked and inaccessible to residents underneath the kitchen sink. Fire extinguishers throughout the facility were fully charged. Kitchen appliances are operational during today's visit. Annual Report continuation on LIC 809-C LPA toured the outside grounds and there is ample seating with shade and one exit gate on the right side of the facility is self-latching and operational. LPA observed emergency food in the kitchen, but emergency water supply was very low. LPA explained that there needs to be a 72-hour supplies worth for emergencies. LPA Tea observed residents watching television in the living room and relaxing in their room. Activities for the residents are limited due to the resident's physical condition, but sometimes the staff takes them out for a walk if they are able to. The residents read and play board games depending on their condition. LPA reviewed medication storage and administration. Medications are stored in a locked closet by the living room. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. LPA reviewed four resident files and two staff files. There were discrepancies noted in the review of residents and staff files. Lead caregiver is currently reorganizing the residents and staff files. Resident forms require signatures from the residents or responsible parties. Annual staff training was incomplete. Administrator certificate expires on December 18, 2027. The following deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPA Tea was greeted and granted entry into the facility by caregivers, Flordeliza Balabbo and Remedios De Leon Marquez and explained the reason for the visit. Licensee (LE) Evangeline Galvez arrived shortly to help assist with the visit. Facility is licensed for six non-ambulatory residents, with a hospice waiver for three. Currently there are four residents, of which two are on hospice during today's visit. Around 8:30 AM LPA reviewed four resident files and two staff files. Residents files and staff files contained all required documentation. Administrator certificate expires on July 4, 2025. LPA Tea along with LE Galvez toured the facility at 9:23 AM. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of four resident bedrooms, three full bathrooms, living room, dining and kitchen area, and a caregiver area. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 106.5 F degrees to 108.8 F degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen cabinet. LPA also observed toxin substances to be locked and inaccessible to clients in care underneath the kitchen sink. Fire extinguishers throughout the facility were fully charged. Kitchen appliances are operational during today's visit. Annual Report continuation on LIC 809-C LPA toured the outside grounds and there is ample seating with shade and one exit gate on the right side of the facility is self latching and operational. LPA observed emergency food and water supply. LPA Tea observed residents watching television, eating lunch and relaxing in their room. At 9:40 AM LPA reviewed medication storage and administration. Medications are stored in a locked closet by the living room. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. The following deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year
On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1). LPA Quiroz called Administrator (AD) Maureen Solanga and explained the nature of the visit. (AD) Solanga arrived to the facility on or about 4:47pm. This facility is a Residential Care Facility for the Elderly and licensed to provide services to six (6) Non-Ambulatory Residents, and has a hospice waiver for two (2) residents. There are four (4) residents in care of which (2) two residents are receiving hospice care services. There are no active COVID-19 cases in the facility at this time. Administrator Maureen Solanga has an Administrator Certificate with expiration date of 10/24/2024. LPA Rosie Quiroz along with (CG1) toured the interior and exterior of the facility. During today's inspection tour, LPA Quiroz observed three of four residents in their bedroom resting and 1 of four residents in the living-room area with staff supervision. LPA Quiroz interacted and interviewed with staff and residents during today's visit. Between 3:16pm-3:25pm while conducting inspection tour of the kitchen area, LPA Quiroz observed expired canned food and food items with expiration dates varying from 4/2022- 1/15/2024. This was verified with(CG1) and (AD) Solanga upon arrival to the facility. (SEE LIC 809-D) LPA Quiroz observed emergency water; however due to expired items observed in pantry area, facility does not have a supply of emergency food readily available for residents in care. LPA Quiroz inspected resident's bedrooms and bathrooms. Water temperatures were recorded to be between 114.7-119.4 degrees Fahrenheit. LPA Quiroz inspected resident’s bedrooms and appeared to be clean. Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits. LPA Quiroz observed the emergency and disaster and evacuation plan. LPA Quiroz toured the outside of the facility and observed seating and shaded area backyard for residents and visitor's enjoyment in backyard area. CONTINUED ON NEXT LIC 809-C PAGE... CONTINUED...While reviewing 4 of 4 resident records, LPA Quiroz observed R4's physician report indicating bedridden status. This was verified with (CG1) and (AD) Solanga upon arrival to the facility. (SEE LIC 809-D and LIC 421-IM) During today's visit, LPA Quiroz provided Consultation on Title 22, Infection control, Maintenance and Operation, Fire Clearance and General Food Service Requirements. Citations were issued during today’s visit and Civil Penalty was assessed.
