Type of visit: Required - 1 Year
On 09/4/26, Licensing Program Analysts (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and requested to meet with Administrator. LPA was greeted by Program Manager (S1) Jason Gavilan. Administrator (A1) Caroline Alacar was called, stated unavailable to attend meeting, and authorized Program Manager to sign and receive report. LPA toured facility with S1. One resident was present during inspection. R1 was observed in the common area with wheelchair seatbelt on. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. First aid kit observed with required items. Fire extinguisher was observed with a service date of 08/19/25. Temperature maintained for refrigerator at 30 degrees F and freezer at 0 degrees F. 2 days perishable food and 7 days non-perishable food were observed. Expired nonperishable and perishable food observed in the kitchen. Cleaning supplies observed locked under kitchen sink and laundry room shelves. All bedrooms were observed to have required furnishings and with adequate lighting. Bathrooms toured and observed with securely fastened grab bars and non-skid mats. Hot temperature tested to be maintained at 105.4 degree F in both bathrooms. Extra linens and towels observed in hall closet. (CONTINUED TO LIC 809C) (CONTINUED FROM LIC 809) Outside of facility toured and observed to be free of debris. Adequate outdoor seating is available for residents. Side gate observed self-closing. Carbon monoxide and smoke detectors were tested and observed to be operational. Medications observed locked in cabinets. MARs were reviewed and medications were checked. All residents and a sample of staff files were reviewed to have all required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6 see attached 809D. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 09/10/26. Forms requested: Lic 308, Lic 500, Lic 610E, current liability insurance, and current Administrator certificate. An Annual visit was conducted by Licensing Program Analyst (LPA) K. McClurg. LPA met with Administrator (Admin) Caroline Alacar & Program Manager (PM) Jason Gavilan, greeted both, stated purpose of visit, & was allowed entry. This facility has a wood burning fireplace that is not used & does not have a gas starter or plumbed for gas. This facility has no open bodies of water such as pools, spa, fountains, etc. This facility is fire cleared for all Non-Ambulatory residents. Physical plant toured. Kitchen appeared to be clean & organized. Sufficient supply of perishable & nonperishable food on the premises. Refrigerator & freezer appeared to be clean & maintained at appropriate temperatures. Food appeared to be stored in a safe and healthful manner. Knives & cleaning products stored in separate locked areas making inaccessible to residents. Front room & Dining/living room sufficiently furnished with adequate lighting. Furnishings appeared to be in good repair. Resident bedrooms sufficiently furnished with adequate lighting. Resident bathrooms appeared to be clean with all fixtures in working order. No unpleasant odors. Grab bars in toilet & tub/shower areas. Sufficient supply of linens. Continued. (Continued) Interior passageways & exits observed to be clear with no obstructions Interior passageways & exits observed to be clear with no obstructions, including exit door in each resident bedroom. An Annual visit was conducted by Licensing Program Analyst (LPA) K. McClurg. LPA met with Administrator (Admin) Caroline Alacar, introduced self, provided business card, stated purpose of visit & was allowed entry. LPA also met Program Manager (PM) Jason Gavilan & Lead Staff (LS) Dina Rosario during visit. Facility has a granted Hospice care waiver for 3 residents. None of residents are currently receiving Hospice care services. This facility has no open bodies of water such as pools, spa, fountains, etc. Facility has wood burning fire place that is not used & access to is limited. Physical Plant toured. Kitchen appeared to be clean & organized. Knives & cleaning products in separate locked areas making inaccessible to residents. Sufficient supply of perishable & nonperishable food on the premises. Dining/living room sufficiently furnished. Furnishings appeared to be in good repair. Sufficient lighting. Resident bedrooms appeared to be sufficiently furnished with items in good repair & adequate lighting. Linens on beds & additional supplies available as needed. Resident bathrooms appeared & smelled to be clean with no unpleasant odors. Fixtures operational. Grab bars in toilet & shower areas. Sinks in resident bathrooms are on sensors & delivers mix of hot & cool water. Hot water measured at kitchen faucet measuring 108 degrees F. Interior passageways & exits observed to be clear with no obstructions, including exit door in each resident bedroom. Smoke & carbon dioxide detectors operational. Fire extinguisher service date: 8/9/24. Outside area clear of debris. Backyard sufficiently maintained. Sufficient seating in good repair. Passageway to exit gate clear & unobstructed. Garage access off of front of facility & kept locked for security purposes. Garage area with inaccessible storage for cleansers, etc. that may pose a risk to clients. Medications observed to be inaccessible to clients & organized. Medication Administration Records (MARs) & Centrally Stored Medication & Destruction Records (CSMDRs) documentation completed & maintained. Resident files maintained. Staff files maintained. An unannounced Annual/year visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Program Manager (PM) Jason Gavilan, introduced self & asked if Administrator (Admin) Caroline Alacar was on the premises. Admin was not on the premises, however LPA was able to speak with Admin over phone. Admin authorized PM to sign for receipt of report. Facility toured. Kitchen appeared to be clean & organized. Knives locked in drawer. 7 days Non-Perishable & 2 days Perishable food on the premises. Dining/living room sufficiently furnished & furnishings appeared to be in good repair. Resident rooms toured. Rooms sufficiently furnished & furnishings appeared to be in good repair. Resident bathrooms appeared & smelled to be clean with no unpleasant odors. Fixtures operational. Sinks in resident bathrooms on sensors so hot water was tested at kitchen faucet. Hot water measured at 124 degrees F. Hot water tank controls adjusted. All interior passageways & exits observed to be clear with no obstructions, including exit doors of each resident bedroom. Smoke & carbon dioxide detectors tested & operational. Fire extinguisher service date: 7/26/23 Outside area toured. Sufficient seating in good repair observed. Backyard area tidy. Passageway to exit gate clear & unobstructed. Garage access off of back yard observed to be locked making contents inaccessible to residents. Deficiency issued. Exit interview conducted with PM.
