On 07/25/2026, Licensing Program Analyst (LPA) M Vega arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by staff. Licensee - Maria Eva Felix was notified of licensing visit and not able to attend. Conducted tour with Staff 1 (S1) – Maria, Licensee - Maria Eva Felix arrived a short time later. Furniture and flooring in common rooms observed to be in good repair with adequate lighting throughout. Resident bedrooms have the required furnishings, lighting and bed linens. Smoke and Carbon Monoxide detectors present and in working order. LPA observed supply of extra bed linens, towels, and personal hygiene/grooming products. Resident bathrooms were stocked with hand soap, paper towels and garbage cans. Kitchen observed to have supply of dishes, cups, plates, utensils, pots and pans and cooking utensils in good repair. LPA observed a 7 day of non-perishable food supply. Counter tops and cabinets are clear and appropriate for food preparation. Knives, cleaning supplies and chemicals are stored and locked separate from any food items. A Washer and Dryer were observed with additional shelving for storage. Appliances observed to be in working order and at maintained at proper temperature. Medications will be stored in a locked cabinet along with First Aid Kit. The First aid kit contains all required items. Fire extinguishers are present dated 07/16/2026. Doors and passageways are unobstructed throughout the home. Continuation on LIC 809C Outside of the facility toured. The home does not have a pool or bodies of water. Outdoor activity space with shaded area and seating were located on the backyard patio. Today, Licensing Program Analyst L. Xiong arrived at the facility unannounced to conduct the . LPAs met with staff Crystal Torres and spoke to Licensee/Administrator Maria E. Felix on the phone and informing them the purpose of the visit. LPA toured the facility with staff. Facility appeared clean with no obstruction or fire clearance issues. All common areas have adequate seating and lighting. Resident bedrooms toured, rooms observed to have all required accommodations. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Smoke detector and carbon monoxide detectors observed operational during inspection. Fire extinguisher present with a service date of 12/2024. Water temperature observed to measure at 105 degrees F. Allegation investigated: Staff did not assist residents with toileting needs in a timely manner. Staff left residents unattended in facility.

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to open a complaint investigation. LPA met with facility Licensee EVa Felix, and explained the purpose of today's visit. Regarding the allegation Staff did not assist residents with toileting needs in a timely manner. LPA interviewed 4 facility staff who all stated the residents are always assisted with toileting timely. LPA interviewed 4 facility residents who all stated they are assisted with toileting timely. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Continued.. Unsubstantiated Regarding the allegation Staff left residents unattended in facility. LPA interviewed 4 facility staff who all stated there is one staff available at all times 24 hours a day to assist residents, and they have never heard of any situation where staff left residents unattended. LPA interviewed 4 facility residents who all stated there is staff available at all times to assist with their needs, and they have never been left alone at the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
