On 8/10/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Mariela Medrano . LPA was granted entry. 6 residents were present during inspection. Administrator (A1) Eva Felix arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Medications were checked and observed kept locked in the closet. Residents’ MARS was reviewed. Atrovastatin 20 mg tab. Take 1 tablet by mouth daily, Alprazolam .5 mg tab ever day, max daily 1 tablet, Amlodipine Besylate 10 mg tablet. Take 1 tablet by mouth daily, Meloxicam 7.5 mg tablet. Take 1 by mouth daily, Melatonin 10 mg, take 1 tablet by mouth every night. *All medications were not initialed for multiple dates. Fire extinguisher was observed with a purchase date of 7/8/26. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 112.1 to 116.7 degrees F in 2 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing. Outside was observed with adequate outdoor seatings available for residents. First-aid kit observed with all of the required items. Fire drill conducted on 7/1/26. All residents files reviewed and there were missing required documents. Samples of staff files reviewed to have all the required documents. The following deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. Licensing Program Analyst (LPA) L. Xiong conducted the complaint investigation visit to the facility. During the course of this investigation LPA reviewed facility files relevant to the complaint investigation. It was determined that the above allegation: Staff does not allow residents to use the restroom, Staff leaves residents soiled for extended periods of time, and Staff does not ensure residents are provided quality food are UNFOUNDED. The evidence from the investigation indicated staff do allow residents to use the restroom, residents were checked and changed if they soiled during the night/day and residents were ensured with quality food. This agency has investigated the complaint alleging (Staff does not allow residents to use the restroom, Staff leaves residents soiled for extended periods of time, and Staff does not ensure residents are provided quality food). Felix on the phone and informing them the purpose of the visit. LPA toured the facility with staff. Facility appeared clean with no obstruction or fire clearance issues. All common areas have adequate seating and lighting. Resident bedrooms toured, rooms observed to have all required accommodations. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Smoke detector and carbon monoxide detectors observed operational during inspection. Fire extinguisher present with a service date of 12/2024. Water temperature observed to measure at 105 degrees F. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to deliver findings on the allegations listed above. LPA met with Licensee, Eva Felix, and explained the purpose of today's visit. Regarding the allegation Staff did not assist residents with toileting needs in a timely manner. LPA interviewed 4 facility staff who all stated the residents are always assisted with toileting timely. LPA interviewed 4 facility residents who all stated they are assisted with toileting timely. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Unsubstantiated Regarding the allegation Staff left residents unattended in facility. LPA interviewed 4 facility staff who all stated there is one staff available at all times 24 hours a day to assist residents, and they have never heard of any situation where staff left residents unattended. LPA interviewed 4 facility residents who all stated there is staff available at all times to assist with their needs, and they have never been left alone at the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation Staff did not treat resident with dignity or respect. LPA interviewed 4 facility staff who all stated they have never witnessed any staff speak rudely, or even be impatient with any of the facility residents. LPA interviewed 4 facility residents who all stated the facility staff has never spoken to them rude or mean. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
