On 05/21/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings. LPA introduced self, stated the purpose of the visit, and met with staff Haze Torres. Licensee Jennifer Jeffries was called and authorized staff to sign report. LPA discuss findings with Licensee via telephone. During the course of the investigation, the Department conducted interviews, obtained copies of records, and toured the facility. The facility provides a cellphone for all clients to use. R1 no longer resides at the facility. Therefore, based on interviews conducted and records reviewed, the allegations alleging staff did not ensure the client’s hygiene needs are met, staff did not follow the client’s special diet, staff did not ensure mediations are taken as prescribed and staff confiscated R1’s phone, the preponderance of evidence standard has not been met. Today, Licensing Program Analyst L. Xiong arrived at the facility unannounced to conduct the Infection Control Inspection. LPAs met with staff Haze Torres and inform her the purpose of the visit. LPA toured the facility with staff. Facility appeared clean with no obstruction or fire clearance issues. All common areas have adequate seating and lighting. Resident bedrooms toured, rooms observed to have all required accommodations. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Smoke detector and carbon monoxide detectors observed operational during inspection. Fire extinguisher present with a service date of 01/2026. Water temperature observed to measure at 105 degrees F. Licensing Program Analyst (LPA) L. Xiong conducted the complaint investigation visit to the facility. During the course of this investigation LPA reviewed facility files relevant to the complaint investigation. It was determined that the above allegation: Staff consumed drugs while on shift, Staff stole resident's money and Staff transported residents in care without having a driver's license is UNFOUNDED. The evidence from the investigation indicated staff did not consumed drugs while working, staff did not steal resident's money and all staff transporting residents have current california driver's license. This agency has investigated the complaint alleging (Staff consumed drugs while on shift, Staff stole resident's money and Staff transported residents in care without having a driver's license). Licensing Program Analyst (LPA) L. Xiong conducted the complaint investigation visit to the facility. I met with Jennifer Jeffries, Licensee and informed her the purpose of the visit. During the course of this investigation LPA reviewed facility files and interviewed staff and resident relevant to the complaint investigation. It was determined that the above allegations: Staff did not respond to resident's call for help, Staff leave residents alone at facility, Staff scream at each other in front of residents, and Unqualified staff transporting the residents are UNFOUNDED. The evidence from the investigation indicated staff not responding the resident's call didn't happenned, staff did not leave the residents alone, staff didn't scream at each other in front of the residents and only staff with current driver's license drove and are authorized to transport residents. This agency has investigated the complaint alleging (Staff did not respond to resident's call for help, Staff leave residents alone at facility, Staff scream at each other in front of residents, and Unqualified staff transporting the residents).
