At approximately 1:10 PM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Administrator Cristina Mallari, Licensee Rogie Mallari arrived shortly after. Facility is a Residential Care Facility for the Elderly (RCFE) with five (5) residents in care. Facility has a Hospice waiver for three (3), with one (1) Hospice residents currently in care, and is approved for all non-ambulatory residents. At approximately 1:20 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one (1) story home, was a comfortable temperature, and passageways were free from obstructions. Fire extinguishers were last inspected 8/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperature measured 109.7- and 116.3 degrees F, which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed at least a two (2) day supply of perishable and seven (7) day supply of non-perishable food, as well as an emergency water supply. Food was found to be stored in a safe manner with open items covered. There is a shaded seating area in the backyard with outdoor space for activities. LPA observed one (1) locked shed in the backyard which LPA inspected and observed the contents to consist of extra supplies. Facility has an internet access device and internet available to residents in care, and the phone was tested and operational during today's inspection. Continued LIC809C... Continued from LIC809... Facility conducts quarterly disaster drills, and the most recent drill was conducted 8/25. LPA observed facility's emergency disaster plan which was last updated 6/25. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights. At approximately 1:50 PM LPA conducted a review of three (3) resident records. All required documentation present. At approximately 2:20 PM LPA conducted review of four (4) staff records. All required documentation present. At approximately 2:45 PM LPA and Administrator conducted a spot check of medication and medication records. Medication is centrally stored and locked. Cristina Mallari Administrator Certificate 7022970740 expires 3/21/2027. All fees are current as of this time. 10/31/2024, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. There are currently 5 residents in care. Facility approved/cleared for 6 ambulatory, 3 non-ambulatory, and 1 bedridden. Waiver/Granted for hospice care for 3. LPA was greeted at front door by staff member. Upon arrival LPA observed two staff members on shift. Administrator, Cristina Mallari arrived shortly after. Licensee, Rogie Mallari arrived later during visit. At approximately 1:00pm, LPA and Administrator toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner. Medications were found to be centrally stored. All rooms were equipped with lighting, night stand, and chest of drawers. All rooms were in good repair. Extra hygiene products and linens were available. Water temperature in sinks accessible to residents in care were measured at 114.2 and 114.6 degrees F which is within the range of 105 to 120 degrees F. Fire extinguishers were last inspected Aug, 2024. Facility conducts fire drills quarterly with the last fire drill being conducted on 08/24/2024. Toxins, sharps and other items that could pose threat if available to residents were located in a drawer in the kitchen and were found to be secured. Extra cleaning products are stored in a locked pantry. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. LPA conducted review of 4 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file. continued on LIC809-C LPA conducted a review of 4 resident records. All records had the required documentation. No deficiencies cited during today's inspection. Licensing Program Analyst(LPA) Carol Fowler arrived to conduct a Pre-Licensing Inspection and was greeted by Applicant Cristina L. Mallari, Administrator current Administrator of Aria Care Home (RCFE). Component III orientation was completed with Administrator. LPA and Administrator, conducted a walk through of the facility that will have 6 clients admitted once licensed. LPA observed the following: There was a supply of perishable and nonperishable food. There was a supply of water. Facility had a first aid kit, including the first aid guide book. Each private client room was furnished with all required items per regulation. Additional linens were available for use as needed. Bathroom had grab bars and non-slip flooring/mat for safety as needed. Hot water measured at 116.9 F which is between 105 and 120 F, per regulation. Facility has a backyard that includes a deck, in the for resident use. There is a ramp from the kitchen area slider that leads on to a covered porch which leads to the backyard, which is also the fire exit from the kitchen. Facility has enough dishes and cook ware to accommodate each client. Medications will be stored in a locked medication cart. Toxins/cleaners are stored in locked laundry room. Facility has received a fire clearance approval from the local fire department dated 10/18/2023. Fire extinguisher, one (1), was purchased and tagged, as required, expires 12/01/2024. All smoke alarms, ten (10), combined with carbon monoxide detector, were all working properly during the inspection. Facility has a required infection control plan. Facility has an emergency disaster plan as required. LPA will forward a copy of the prelicensing report to the application Analyst; The application Analyst will notify the applicants of the status of their application. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
