Type of visit: Required - 1 Year

At approximately 12:40 PM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by Caregiver Susan Rapido. Administrator Zenaida Almogela was contacted via telephone and Licensee Gaylord Almogela arrived during visit. Facility is a Residential Care Facility for the Elderly (RCFE) with two (2) residents in care. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 12:50 PM, LPA initiated a tour of the facility with Caregiver and observed the following: Facility is a one (1) story home, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguisher was observed charged and last serviced 1/26. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperature measured 117.5 degrees F, which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed at least a two (2) day supply of perishable and seven (7) day supply of non-perishable food, as well as an emergency water supply. Food was found to be stored in a safe manner with open items covered. There is a seating area in the backyard with outdoor space for activities. Facility has an internet access device and internet available to residents in care, and an updated facility number was gathered during this visit. Continued LIC809C... Continued from LIC809... Facility conducts quarterly disaster drills, and the most recent drill was conducted 3/26. LPA reviewed emergency disaster plan which was last updated 1/26. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights. At approximately 1:25 PM LPA conducted review of two (2) staff records. All required documentation present. At approximately 1:40 PM LPA conducted a review of two (2) resident records. All required documentation present. At approximately 2:00 PM LPA and Licensee conducted a spot check of medication and medication records. Medication is centrally stored and locked. Zenaida Almogela Administrator Certificate 7034417740 expires 10/18/2027. Updated copies of the following documents shall be submitted to CCL by 6/4/2026: LIC500 - Personnel Report LIC308 - Designation of Responsibility LIC400 - Affidavit Regarding Client Cash Resources LIC402 - Surety Bond LIC610E - Emergency Disaster Plan No deficiencies cited. Type of visit: Required - 1 Year

At approximately 1:05 PM, Licensing Program Analysts (LPAs) Magdaleno and Cuadra arrived unannounced to conduct a required 1-year annual inspection and were greeted by Staff. Administrator Zenaida Almogela was contacted and arrived at approximately 1:30 PM. Facility is a Residential Care Facility for the Elderly (RCFE) with two (2) residents in care. Facility is approved for all non-ambulatory residents. At approximately 1:15 PM, LPAs initiated a tour of the facility with Staff and observed the following: Facility is a one (1) story home, was a comfortable temperature, and passageways were free from obstructions. Fire extinguishers were last inspected 1/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperature measured between 118.2 degrees F and 119.1 degrees F, which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPAs observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPAs observed at least a two (2) day supply of perishable and seven (7) day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered, as well as an emergency water supply. Facility has an internet access device designated for resident use and internet service available. Facility telephone was tested and operational during inspection. Continued 809C... At approximately 1:40 PM LPAs conducted a review of two (2) resident records. One (1) of two (2) residents were missing a Needs and Service Plan. (deficiency cited) At approximately 2:00 PM LPAs conducted review of two (2) staff records. All required documentation present. At approximately 2:10 PM LPAs and Administrator conducted a spot check of medication and medication records. Medication is centrally stored and locked. Zenaida Almogela Administrator Certificate 7034417740 expires 10/18/2025. All fees are current as of this time. LPAs and Administrator discussed facility's Emergency Disaster plan, last updated 1/2025. Facility’s last quarterly disaster drill was conducted on 3/25. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500 - Personnel Report LIC308 - Designation of Responsibility LIC400 - Affidavit Regarding Client Cash Resources LIC402 - Surety Bond Deficiencies are cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, or repeat violations within a 12-month period, may result in a civil penalty assessment. Type of visit: Required - 1 Year

On 6/25/2024, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by House Manager, Ana Montez. Licensee, Gaylord Almogela was contacted and notified of the visit. The facility currently has 2 residents admitted, none of which with a diagnosis of dementia. The facility is currently with a COVID positive case. LPA had conducted only the physical facility portion of the inspection due to COVID safety precautions. LPA continued with a tour of the facility with House Manager. Facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Resident common areas, kitchen & food storage areas were inspected. Fire Extinguishers found to be charged on 1/8/2024. Smoke and carbon monoxide detectors found throughout the facility, were tested and found to be functioning. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen refrigerator found to have appropriate coverings. Cleaning supplies and other toxins are safely stored in locked cabinets in the garage and under the kitchen sink, all of which were secured upon inspection. Sharps and other kitchen supplies that could pose danger if available to residents were found secured. There was a supply of hygiene products and paper products available for residents. Water at faucets accessible to residents were measured at 107.4 degrees F which is within regulation. There is one primary emergency exit located in the backyard which were found to be unobstructed. There is an outdoor patio with shade and large outdoor space for residents. No deficiencies cited during today's visit. LPA to continue annual for facility file and medication review at a later date. Licensee, Zenaida Almogela's Administrator Certificate 7034417740 is valid through 10/18/2025.
