At 10:05 AM Licensing Program Analyst (LPA), Huma Rahimi, conducted an unannounced annual inspection at the facility mentioned above. LPA met with the Administrator Alina Solloway and explained the reason for the visit. Physical tour was conducted with the Administrator and LPA observed the following: Kitchen: At approximately, 10:15 AM LPA toured the kitchen area and observed enough supplies of staple non-perishable for minimum 1 week and perishable for 2 days at the facility. All knives and sharps observed to be locked in a kitchen drawer. LPA observed all cleaning supplies to be locked under the kitchen sink. The kitchen is equipped with a refrigerator, microwave oven and a sink. A fire extinguisher was hanging on the kitchen wall and last serviced on 10/01/2025. Medications: At approximately, 10:20 AM LPA observed medications are centrally stored and locked in a cabinet in the living room. LPA reviewed residents medication and did not find any discrepancies. All medications were given as prescribed. Bedrooms: LPA observed total of four (4) bedrooms designated for residents use. The facility is fire cleared six (6) non-ambulatory residents. Bedroom #1 and #3 are shared. The bedrooms have sufficient lighting and closet space. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Facility has an awake staff. Bathrooms: The facility has three (3) bathrooms. The bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms measured at 109.4 degrees. Continue on LIC 809C Common Areas: The facility maintains a comfortable temperature at 72°F. The living room and dining area appeared clean and were properly furnished. The living room has a television, comfortable furniture. No obstructions and or tripping hazards throughout the facility. Garage/Laundry: The facility does not have any garage. The laundry is located in a closet in the hallway by bedroom number one (1). Laundry detergents, cleaning agents and other toxins are locked in a separate closet next to the laundry closet. Outside areas: At approximately, 10:25 AM LPA toured the outside area of the facility. LPA observed appropriate outdoor furniture, with a covered shaded area for residents. LPA observed a locked storage where additional cleaning supplies were kept. Smoke detectors/carbon monoxide. The smoke alarms and carbon monoxide detector are dual and hard wired, and at 10:35 AM they were tested and observed to be operational. Between 10:45 AM to 11:45 AM, LPA reviewed records of six (6) residents and two (2) staff. Residents and staff records appeared to be complete and updated. Administrative: LPA collected Certificate of Liability Insurance, and LIC500. No deficiency cited during today’s visit. At 11:45 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced case management-other visit to the above facility. LPA met with the Administrator Alina Solloway and explained the reason for the visit. LPA was informed that Resident #1 (R1) who have been relocated from Santa Clarita Hills Senior Living is currently residing in the above facility. At 11:50 AM, LPA requested resident’s roster and reviewed which confirmed that R1 is residing at the facility as of 04/01/2025. No deficiencies issued during today’s visit. Type of visit: Post Licensing

At 10:00 AM, Licensing Program Analyst (LPA) Huma Rahimi made a comprehensive Post Licensing visit to this facility and met with the Administrator Alina Solloway, and LPA explained the reason for the visit. During the time of the visit, LPA observed four (4) residents present at the facility. LPA toured the physical plant areas outside and inside, the resident's four (4)bedrooms, three (3) bathroom and personal accommodation. LPA reviewed the food service areas and food supply (perishable & nonperishable) as well as received the residents and staff records. The storage of toxic and hazardous items were locked and inaccessible to residents in care. The medication were observed locked in a cabinet located in living room. The fire extinguisher was checked located in the kitchen and purchase on 10/18/2023, and the hot water temperature was measured in one of the bathrooms. The temperature measured at 108.9 degrees Fahrenheit in one of the bathrooms, which is the required range for residents comfort and safety. All required forms & poster were displayed on the facility's wall in the hallway adjacent to the laundry room. LPA observed the facility's temperature to be comfortable and was measured at 77 degrees Fahrenheit. No citations were issued. Exit Interview was conducted. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Michael Cava conducted a Pre-Licensing Inspection with the applicant representative and administrator, Alina Solloway and Armine Dishoyan. An Application to operate a Residential Care Facility for the Elderly (RCFE) was received by Community Care Licensing (CCL) on August 31, 2023. A fire clearance was approved on October 19, 2023 for six (6) non-ambulatory residents of which one (1) may be bedridden, for a total capacity of six. The applicant is also requesting a hospice waiver to retain four (4) residents. The smoke alarms and carbon monoxide detector are dueal and hard wired. The facility has one new fire extinguisher that was purchased on October 19, 2023, that is located in dining room/kitchen area. A tour of the physical plant was initiated at approximately 9:45am and the following was observed: KITCHEN: The facility has a Kitchen area that is equipped with a refrigerator, microwave oven and sink. There were adequate supplies nonperishable food and dining ware to accommodate a maximum capacity of six (6). Knives were observed locked in a kitchen drawer. BEDROOMS: There are four (4) bedrooms designated for client use. Per STD 850, Bedroom #1 and #3 are shared rooms but cleared for only one bedridden at any time. Bedroom #2 is cleared for non-ambulatory. Bedroom #4 is cleared for ambulatory only. The applicant furnished the resident bedrooms with beds, night stand, chairs, dresser, bedding and linen. The bedrooms have sufficient lighting and closet space. BATHROOMS: The facility has three (3) bathrooms. The bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. The hot water delivered in the bathrooms measured at 105 degrees. COMMON AREAS: These included the living room which was equipped with a couch, chair, a television and two tables. The dining area has a large dining room table to accommodate six (6). There were no visible immediate hazards. LAUNDRY ROOM: The laundry room is located in the hallway by room #1. The washer/dryer are brand new. Detergents and cleaning supplies will be kept in another locked closet. MEDICATIONS: The medication closet is located in the same hallway as the laundry room and room #1. It was observed locked will be maintained inaccessible to the residents at all times. A complete first aid kit and manual stored in the medication closet OFFICE/STAFF WORKSTATION: For the time being, although Room #4 is cleared for ambulatory only, it will be temporary used for staff office until needed to accept a resident that is ambulatory only, not to exceed the fire-clearance for a capacity of six (6). SURROUNDING GROUNDS: The driveway, passageways and entrance to the home was clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a patio and backyard furniture to accommodate the six (6) residents. The facility backyard has sufficient yard space. No pools or bodies of water observed in the front or back yards. In addition to the Pre-Licensing inspection, a Component III power point presentation was held between 9:10am to 9:45am. Pursuant to Title 22, Division 6 of the CA Code of Regulations, the facility's physical environment appears to be compliant and ready for licensure.
