Allegation investigated: Staff did not communicate with responsible party regarding resident's care

On 11/13/25 at 5:30pm Licensing Program Analyst's (LPA) J. Leffall conducted an initial complaint visit to open and to deliver findings on above allegations. LPA met with Administrator Eddiener Sison. The Department conducted interviews with staff. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. On 9/23/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Segundina Cerlas, LPA was granted entry. 6 residents were present during inspection. Administrator (A1) Eddiemer Sison was present during LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in kitchen cabinet. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 12/19/24. Fire drill completed on 7/7/25. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in closet. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 115.7 to 116.7 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at 0 degrees F and refrigerator temperature maintained at 37 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. Client’s and staff’s files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. On 1/21/25 at 10:30 am Licensing Program Analyst (LPA) J. Leffall conducted an initial complaint visit to open and investigate above allegations. LPA met with Administrator A1 Eddiemer Sison and stated purpose of visit. The Department reviewed records and conducted interviews with staff, residents and facility Administrator. The Department toured the facility and checked medications and Resident's MARS. 2 residents were having lunch after interviews concluded. Based on interviews that were conducted residents stated there are no issues with staff, or resident's diapers. Medications with MARS were observed and accurate. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. On 10/28/24 at 8:30am Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Geofrey Leyva . LPA was granted entry. 4 residents were present during inspection. Designee Bang Sison (D1) was called and arrived shortly after LPA’s arrival. LPA toured facility with D1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. Samples of medications were checked and observed kept locked in the kitchen cabinet. Residents’ MARS was reviewed. Fire extinguisher was observed with a purchase date of 12/20/23. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 118 in resident bathroom 1, 119 degrees in bathroom 2. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. First-aid kit observed with all of the required items. Fire drill completed on 10/3/24. All residents and a samples of staff files reviewed to have all the required documents. Continued on LIC-809C......... No deficiencies detected in accordance to California Code of Regulations, Title 22,Division 6. On 10/26/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. introduce self, stated the purpose of the visit and met Eddiemer Sison, Designated Representative. All four residents were present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. The temperature is maintained for refrigerator at 37 degrees F and freezer at 0 degrees F. Fire extinguisher was observed with a service date of: 09/26/22. Fire drill last completed:10/02/23. Medications observed kept locked in kitchen shelf. MARs were reviewed. Cleaning supplies and chemicals stored and locked in garage cabinet. All bedrooms were observed to have the required furnishings and with adequate lightening. The bathrooms were toured. Bathrooms were observed operational during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested at 147.7 degrees F in the bathroom 1 and range between 148.6 and 147.8 degrees F in master bathroom. Outside of facility toured and observed to be free of debris. Side gate observed self-closing and self-latching. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents’ files were reviewed to have all the required documents. Staffs’ files were also reviewed. Staff files were observed to have current First Aid/CPR, fingerprinted clear and associated to the facility. A deficiency and an immediate Civil Penalty of $1000 was assessed. See Lic 421IM is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted.
