Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Ivashia Wright conducted an unannounced annual required visit. Upon entry, LPA was greeted by Norma Martinez, Caregiver, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and five (5) residents present. Facility Overview: The facility is a single story home with five (5) residents' bedrooms, one (1) staff bedroom, 2.5 bathrooms, a dinning room, a laundry room, a kitchen, a living room and a garage. There is no gated pool and there are no firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked under the kitchen sink and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was measured at 105.7 F, meeting the department requirements. LPA observed fire extinguishers to be in compliance with the department's requirements and with an expiration date of 6/3/2026. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for three (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Three (3) resident files were reviewed and contained all required documentation. LPA observed first aid kit to be stored in a locked closet inaccessible to residents. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for 4 (four) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire/earthquake drill conducted on 2/21/2025, which did not met department requirements. A Technical violation was issued. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted an unannounced visit for a required annual inspection. The LPA was greeted by the Caregiver Aylin Rico, notified them of the purpose for the visit and was allowed to enter the facility to conduct the inspection. The Licensee/Administrator Jazmin Cerda joined the visit at a later time. Facility Overview: The facility is a single story building with 5 resident bedrooms, 1 staff room, 2.5 bathrooms, a dinning room, a laundry room, a kitchen, a living room and a garage. There is no gated pool and there are no firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department's requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked under the sink in the kitchen and inaccessible to clients. The smoke detector and carbon monoxide detector were in good working condition. The water temperature was tested within regulations measuring at 108.3 F Continued 809-C.... Care & Supervision/Administration: Adequate staff were present to supervise residents during the visit. The Administrator holds a current Administrator’s certificate with expiration date of 04-24-26 and a CPR certification with the expiration date of 06-20-26. Record Review and Resident/Staff Files: LPA reviewed files for 2 staff members, confirming criminal clearance, updated training, and CPR/First Aid certification. 3 residents' files were reviewed and contained all required documentation. LPA observed first kit to be locked and inaccessible to the clients in care. The clients and staff files were kept locked and inaccessible to unauthorized individuals Health-Related Services/Incidental Medical Services: All residents' medications were securely locked in a cabinet and located in the kitchen area. LPA reviewed medications for 2 residents, confirming that all medications were listed and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last emergency drill conducted on 2-21-2025, which met the department's requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Jul 25, 2024Complaint investigation reportSubstantiated
Allegation investigated: Facility did not provide a refund.
Licensing Program Analyst (LPA) Janira Arreola, conducted an unannounced visit to the facility in order to deliver findings for the above allegation. LPA met with Licensee, Yazmin Cerda, who was informed of the purpose of the visit. The investigation consisted of conducted interviews and conducted records reviews. It was alleged that the facility did not provide a refund to R1’s family after R1 passed away 1/28/2022. A payment of $4215 was made for February 2022 to the facility before R1’s passing, of which $2000 is still outstanding back to the family of R1. Substantiated LPA conducted a record review of check issued to the facility showing payment of $4215. LPA also reviewed text messages between staff #1 (S1) and R1’s family member. It was found that on 2/7/2022 a text thread was started to retrieve the amount paid to the facility. An agreement was made to pay back $4000 and pay in installments of $1000, in June, July, August and September of 2022. The text messages show efforts made by R1’s family to contact S1 resulting in an outstanding $2000 as of 1/9/2023. LPA conducted interview with staff which revealed that S1 was hesitant to pay R1’s family member as they were not the legal representative for R1. However, the screenshots of payments made from a Zelle account show that S1 had previously been paying R1’s family member directly, and had paid $2000 to them. Based on interviews and records reviewed, the preponderance of evidence standard has been met. The allegation that the facility did not issue a full refund is substantiated. This requirement was not met as evidenced by: Based on interviews and records review, the facility did not issue a full refund to the R1’s responsible party within the required time frame. May 7, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On May 07, 2024, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Required Annual Inspection and met with the Administrator, Yazmin Cerda. The facility file review was conducted at the Regional Office and additional forms were requested and reviewed on site. The facility is operating at the full capacity of six Elderly Adults, (740). LPA Mixson toured the facility along with the Administrator and inspected the facility inside and outside, and there were no obstructions to the indoor or outdoor passageways at the time of this visit. The facility is a single-story home located at 37212 Edgemont Drive, Murrieta, CA 92563. Physical Plant: The facility phone number is (951) 249-9041 and it is operable. The LPA observed the residents’ bedrooms, and each was equipped with required furniture as per Title 22. LPA Mixson inspected facility bathrooms, and the hot water temperature tested within regulations. The bathrooms were clean, and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. The LPA observed required postings such as "If you See Something, Say Something" the "Personal Rights" and the Ombudsman postings. The cleaning supplies and sharp items were kept locked and inaccessible to the residents in care. There was a designated storage space for the residents and staff files, and it was locked and inaccessible to residents in care currently. The overall facility is clean, the furniture is in good condition. The facility heating system and other appliances were operable at the time of this visit, and there were safety lights for night. Medications: Were locked and inaccessible to residents in care, and there was a sufficient supply of medication for each resident. There were no observable missed medications, or errors reviewed at this time. Food Service: The non-perishable and perishable food supply are sufficient per regulations, and there are a variety of food types available for residents at the time of this visit. Dishes and utensils were in sufficient supply and stored properly, and sharps are locked. Care & Supervision: Facility has sufficient staff, two staff on site at the time of this visit, and the Administrator arrived shortly after the LPA for a total of three. Records Review: The LPA reviewed resident and staff files, conducted staff interviews, but residents were occupied with daily activities. Previous Community Care Licensing forms were reviewed. There were no Title 22, Division 6 Regulation violations observed or cited during today’s visit.
