On 07/09/26 at 3PM, Licensing Program Analyst (LPA) D Panlilio arrived unannounced to conduct a Required - 1 Year inspection. LPA met with staff (LC, S1) and explained the purpose of the visit. LPA observed ADM has a current administrator certificate # 7016461740 which expires on 07/09/2028, At 3:15PM, LPA toured the facility including but not limited to the front entrance, screening station, kitchen, bathrooms, bedrooms and common areas. There is one central entry point for universal screening for staff, residents and visitors. A sign-in policy, visitor’s logs, no touch thermometer, additional face masks and hand sanitizers were observed at the screening station. Emergency Disaster Plan, Complaint poster, Personal rights, Cough/sneeze etiquette, proper hand-washing signs were observed posted in common areas. Facility has a sufficient 2-day perishable and 7-day non-perishable food supply. Facility has a 30-day supply of PPEs, paper, medications locked in cabinets. Comfortable temperature is maintained at 74 deg F. Facility has a mitigation plan in place and the infection control leader is the administrator. Inside and outside pathways were free of obstruction and fire hazards. Fire extinguishers were observed fully charged and last inspected on 08/20/25. Smoke and Carbon monoxide detectors were operational. LPA reviewed 3 staff and 4 resident files. At around 3:15PM, LPA observed the following deficiencies: Hot water temperature measured at 126.2.deg F with no warning signs posted Open trash bins in bedrooms, bathrooms Deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Plan of correction: On or before POC due date, ADM agrees to adjust hot water temperature and submit a photo of water thermostat reading showing hot water temperature between 105 deg F and lower than 120 deg F. ADM agrees to post warning signs on hot water faucets above 125 deg F to ensure safety.

Allegation investigated: Facility staff is financially abusing resident

On 03/10/26 at 4PM, Licensing Program Analyst (LPA) Daisy Panlilio conducted an unannounced complaint investigation, met with administrator (ADM) and explained the purpose of the visit. LPA conducted staff )S1, S2) interviews, gathered information and delivered investigation finding to ADM. During investigation, LPA interviewed reporting party (RP), staff (Licensee, ADM) and obtained and reviewed the following documents from ADM: Personnel record (LIC500), Resident roster(LIC9020), R1's appraisal report, physician's report, admission agreement, needs & services plan, after visit discharge reports, incident report, payment records. Continued on next page, LIC9099-C Unsubstantiated At 4:10PM, LPA interviewed staff (S1) who stated that the resident (R1) was first admitted at the facility on 12/21/24. Review of R1's admission agreement dated 12/22/24 showed R1 was her own responsible party (POA) and paid for her monthly basic services and requested cash advances with her personal checks from 12/21/24 until 01/16/26. Review of R1's personal checks issued from 12/22/24 to 01/16/26 showed R1 paid for the facility's monthly basic services and requested cash advances from the administrator to purchase personal items such as Coloplast (Skin cream), diapers, underpads and favorite snacks (chips, sunflower seeds). LC also stated that R1 issued personal checks to pay for her primary residence's home association fees (HOA). LC and ADM denied financially abusing R1. ADM stated R1 was sent to the hospital on 02/03/26 for a change in condition. R1 returned back to the facility from the hospital on 02/20/26 under hospice care. R1 passed away on 02/28/26 and family collected her personal belongings on 03/01/26. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation that facility staff was financially abusing resident is UNSUBSTANTIATED.
