Licensing Program Analyst (LPA) Magda Malcore conducted an announced Pre-licensing inspection of the facility. LPA identified self and met with Yanira Saldivar. The License application is for a changed of ownership, Residential Care Facility for the Elderly (RCFE). A fire clearance was granted by the Apple Valley Fire Department on March 12, 2026, for a capacity of ten (10) non-ambulatory and two (2) bedridden residents. LPA toured the interior and exterior areas of the facility. Physical Plant/Environment: The property consist of eight (8) bedrooms, seven (7) bathrooms, laundry room, office, family room, living room, attached garage and guest house. The Indoor and outdoor passageways were kept free of obstructions. The facility has no swimming pools or similar bodies of water. No firearms are stored at the facility. The facility has operating night lights, fire /carbon monoxide alarms, fire extinguishers, telephone service, laundry equipment and covered fireplace. There is a private office were resident and staff files will be stored. The facility maintains a sufficient supply of bed linen, blankets, towels, and personal hygiene supplies. Resident bedrooms were equipped with mattresses, bed linen, nightstands, chairs, and lighting. Resident bathrooms were equipped with grab bars, non-slip material in showers, and operating bathroom equipment. The bathroom hot water temperatures tested at 106 degrees Fahrenheit (F). Food Service/Dining Areas: Kitchen/dining areas were maintained clean and odor free. Utensils and dishware are in good repair and ready for resident use. Sharps, cleaning solutions, and other toxins were kept locked. There is a minium of seven (7) day non-perishable and two (2) day perishable food supply. The dining is area located near the kitchen has sufficient space for resident use. Health Related Services: The facility maintains a locked cabinet were resident medications are centrally stored. Activities: The facility has sufficient indoor and outdoor space for residents. Backyard is equipped with shaded activity space and enclosed with latching gates. ***continued on LIC809-C*** Postings: The facility has posted: emergency telephone numbers, Licensing Complaint poster, Ombudsman poster, and Resident Personal Rights. Comp III orientation was completed during today's inspection. An exit interview was conducted. COMP II by CAB successfully completed Method: Phone Call at CAB Applicant/administrator participated in COMP II at CAB telephone call with analyst at CAB. Identification of the applicant and administrator was verified by presenting photo ID via phone. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB.
