On 6/17/2026 at 8:43 AM, Licensing Program Analyst (LPA) LaVette Farlow arrived unannounced to conduct the required annual comprehensive inspection of the facility. LPA were greeted and granted access into the facility by Caregiver, Maria Sandoval and introduced self and stated purpose of the visit. LPA asked Maria to informed the Administrator Irma Rodriguez of LPA's arrival. LPA greeted and met with Irma and explained the purpose of the visit. The facility has 3 bedrooms, 1 staff bedroom, 1 office, 3 bathrooms, kitchen, dining area, family room, laundry area in the hallway, attached garage, and backyard. LPA completed a walk through of facility, review of records, and conducted a random audit of the medication. Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 74 degrees Fahrenheit. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting. LPA inspected client bathrooms; bathrooms were clean and appliances were found functional. During the inspection of the front bathroom LPA observed a container of Ajax under the front bathroom sink. LPA advised administrator to secure the items. A Technical Violation issued. LPA observed that facility has a sufficient supply of hygiene items for residents in care. Water temperatures tested at 116.2,116.3 and 115.1 degrees Fahrenheit. The facility is equipped with operational smoke detectors, carbon monoxide alarms and charged fire extinguisher. LPA observed poster on display for personal rights, and disaster plans were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept in secure cabinets, inaccessible to clients. There was a designated storage space for client/staff files. Medications and first aid kit were in secure cabinets and inaccessible to clients. There are no firearms or ammunition in the facility. Overall, the facility is clean, in good repair, and operating in safe conditions. Food Service: LPA observed that the facility has a sufficient supply of perishable and non-perishable items. The facility has sufficient supply of dishes, cups, and utensils were also stored properly. Yards/Outside: LPA observed one shaded patio, a side gate with self-latching handle on the right side of the house that leads into the backyard. All outdoor pathways were free of obstructions. Record Review: LPA reviewed Administrator and 1 staff files for First Aid/CPR certification, criminal record clearance, training, health screenings and TB test. The personnel file appeared to maintained. LPA reviewed 2 residents files for admissions agreements, pre-admissions appraisals, physician's reports, needs and service plan. LPA observed 2 out of 2 clients were missing needs and service plan, and or a current physicians report. Technical Violations issued. LPA conducted a random audit of 2 out of 2 residents medication and centrally stored medication log. During the review LPA observed that 1 out of 2 residents were missing the centrally stored medication log and medication that was stored was missing on the log. LPA also, observed the resident MARs for PRN and prescription medication and observed for 2 out of 2 residents in care MARs was missing the initial stating the medication was issued, PM medication was already initial and haven't been dispensed, and if the resident receives an AM, NOON, and PM dose the initial appeared once or only twice. There was not an initial for each dose given or an explanation for the missing initial. Deficiencies Cited. LPA reviewed Facility folder for Infection Control Plan, Emergency Disaster Plan, Liability Insurance, Fire Drills, Poster, and other pertinent documents. LPA observed the Infection Control Plan and the Emergency Disaster plan have not been reviewed and updated since 9/9/2023. Technical Violations issued. During today's visit two (2) deficiencies and five (5) technical violation were cited during this visit. Type of visit: Case Management - Deficiencies

On March 3, 2026, at 9:55 AM, Licensing Program Analysts (LPAs), LaVette Farlow and Andrew Martinez, conducted a Case Management - Deficiencies inspections. LPAs met with Licensee, Irma Rodriguez, who accompanied LPAs on facility inspection. Licensee confirmed two (2) additional staff and four (4) residents were present at the facility. At 11:25 AM, LPAs interviewed staff member, Sandra Romero, who stated they hold the job title as Caregiver and has been working various shift times, mainly during the day, at the facility for the past five (5) months. Sandra stated she assists with everything at the facility including assisting residents with going to the bathroom. Upon LPAs staff file reviews, Licensee was unable to provide a staff file on Caregiver Sandra Romero. LPAs inquired with Licensee the job duties of Sandra Romero and Licensee stated that she was mainly here to assist with cleaning the facility and periodically help with meals a couple days out of the week, but has now become more frequent to at least 5 days a week. Licensee stated that she provides a service to the facility. LPAs informed Licensee that based on the duties being performed, Sandra is required to have been fingerprinted and obtained a criminal record clearance prior to working at the facility. LPAs informed Licensee of the deficiency and that a civil penalty will be assessed. *** Continued on LIC809-C *** Based on interviews and observations during today's visit, an immediate civil penalty is being assessed at the rate of $100.00 per day, for a maximum of 5 days, totaling $500.00. Civil penalties may continue to accrue until the deficiency is corrected in accordance with Health and Safety Code regulation 1522(c)(1), 1568.09(c), 1569.17(c), and 1596.871(c). Type of visit: Prelicensing

Licensing Program Analyst (LPA) Michelle Echeverria , arrived at All-Loving Senior Care, to conduct an announced Pre-Licensing visit for licensure. LPA was greeted by licensee, Irma Rodriguez and spouse Otto Ramos. LPA introduced self and stated purpose of the visit. LIC200 application was submitted on 09/09/23 for (5) non-ambulatory residents and (1) bedridden resident. Fire Safety Inspection clearance was granted for (5) non-ambulatory residents and (1) bedridden resident on 04/15/24. LPA toured the facility inside and outside and observed the following: Structure: Facility is a one story house with (3) resident bedrooms, (1) staff bedroom, (3) bathrooms, office, living room, dining area, kitchen, laundry, backyard, and attached garage. Heating/Cooling System: Central heating and air conditioning system installed with a central panel located in the hallway to control entire house. Bedrooms: Two bedrooms accommodates two non-ambulatory residents in each room, and one bedroom accommodates one non-ambulatory resident and one bedridden resident. Bathrooms: The bathrooms have a working toilet, wash basin, and shower with an adequate supply of toilet paper and soap. Kitchen/Laundry: An adequate supply of dishes, glasses, utensils, pots, and pans were observed. Knives, sharps, detergent, and chemicals are stored in locked compartments. There was a pantry stocked with non-perishable food and perishable food found in the refrigerator. LPA observed the stove to be operational. Refrigerator/freezer were in working condition. Water tested in the bathroom faucet measured at 114.5 degrees fahrenheit. A functional washer and dryer are located inside a closet in the hallway. Living/Family room: There was a furnished living room with a tv observed. Linens and Hygiene Supplies: An adequate supply of linens and hygiene supplies stored in a closet. Yards/Outside: A self-latching handle gate on the right side of the house that leads into the backyard. There are no firearms, ammunition, swimming pool or bodies of water observed. All outdoor pathways were free of obstructions. Emergency Phone Numbers, and Exit Plan: Facility sketch, ccl complaint poster, and personal rights were observed posted by the entrance. General items: The smoke and carbon monoxide detectors were tested and are operable. There was a fully charged fire extinguisher observed. Resident/Staff records stored inside the locked office. First Aid kit with required components, and a locked compartment for medication storage was observed. Emergency kits were observed inside the garage. The facility has a functioning telephone and line. The facility was evaluated in accordance with the California Code of Regulations (CCR), Title 22. Based on the observations and evaluation of the facility this date, the facility is ready for licensure. LPA completed COMP III with the Licensee at the conclusion of the inspection. Licensee will be notified once facility is licensed.
