Licensing Program Analyst (LPA) Graham Gunby arrived unannounced at the facility to conduct a required annual inspection utilizing the care tool. LPA contacted Administrator who informed LPA he was unavailable for illness, but the LPA can meet with Caregiver, Mary Thenu, for assistance. Today's census is five residents in care with two residents on hospice services. Facility is licensed for six non-ambulatory of which one can be bedridden, hospice waiver of three. LPA observed two residents in the common area, three residents in their room. During today's inspection, LPA and Caregiver conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. LPA and Caregiver conducted a tour of bathroom, laundry, kitchen, resident bedrooms, office, backyard and the common areas. LPA observed sharps in the kitchen to be locked and secured. LPA observed medications to be organized in individual bins locked in the hallway. LPA observed toxins to be locked separately from other items. LPA inspected the pantry and observed approximately seven days of non-perishable foods. LPA observed refrigerator to contain approximately two days of perishable foods. LPA observed areas to be clean and free of obstructions. LPA reviewed 5 of 5 resident records and 3 of 3 staff records. Both resident and staff binders contained the required signed documentation. Allegation investigated: Resident sustained bruises while in care.

On April 16, 2025, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Caregiver Iwan Wibono. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: Resident sustained bruises while in care. Resident in question moved out of the facility in September 2024 and the department was unable to obtain information regarding the allegation above. Saff and residents interviewed were unable to recall or provide information regarding the allegation above; therefore, the above allegation is unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Licensing Program Analyst (LPA) Talwinder Bains arrived on 09/03/24 to conduct the annual inspection. LPA met with Staff, Iwan Wibowo (S1) and explained the purpose of today's visit. Staff and LPA spoke with Administrator, Hardosubroto Alfonsus via phone who gave permission S1 to assist LPA with today's visit since Administrator was not available. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed two (2) residents and two (2) staff files and found all required documents. LPA and S1 toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher was last serviced on 06/13/24 and was ready for emergency use. Hot water temperature was observed to be 120 degrees F, which is within the regulation range of 105-120 degree. Inside temperature was 74 degree F. Facility was clean and well organized. All required postings were observed. Facility is conducting quarterly fire and disaster drill as required. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by 09/30/24. No deficiencies were observed or cited per Title 22, CCR Regulations during this visit.
