Type of visit: Case Management - Other
On 1/14/26, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit and met with caregiver S1. Administrator arrived to assist as well The purpose of the visit was to review and discuss the decline and passing of R1 that occurred on 12/26/25 as well as to discuss the procedure being considered by licensee for resident care plan documentation. Records and interviews showed that R1 was experiencing physical decline and reduced intake. Appropriate communication occurred between facility staff, medical providers and emergency responders. Following a holiday home visit, R1 showed continued decline for which emergency care was provided. R1 passed away in hospital on 12/26/25 of natural causes. Issues discussed included documentation of resident change of status, physician contact and written medical guidance as well as retention and understanding of the terms of a resident POA agreement. LPA, Admin also discussed resident appraisal and plans of care requirements for whoever the licensee designates for the task. As a result of today’s inspection, no deficiencies were noted. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 11/14/25 to conduct a Annual Inspection utilizing the CARE inspection tool. Caregivers notified Admin. Admin arrived to assist with the visit. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. The home is very clean and residents stated they are happy with care. LPA reviewed 3 resident files. Files are complete and well organized. . LPA reviewed 2 staff files. Files are complete. LPA and administrator discussed and LPA advised on several topics No deficiencies are being cited as a result of todays inspection. Type of visit: Case Management - Other
On 12/10/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit and met with caregiver/ designees . On 11/27/24 the department received a incident report for R1 requiring hospitalization on 11/19/24 for a change in condition. LPA requested resident records and reviewed information provided. R1 has returned from the hospital and returned to baseline. LPA advised that PRN medications be documented with date, time, medication, reason and result. Additionally, LPA advised that licensee confirm parameters for physician contact or emergency services when PRNs are not effective as intended. Lastly, R1 has a medication with blood pressure parameters. LPA advised that non-clinician caregivers cannot determine to give or hold a medication based on blood pressure- advised discussing the orders with the physician and have a plan in place for that medication. On 11/22/24, a Non-compliance conference was held via a Teams conference call. Present were: Regional Manager, Alycia Berryman, Licensing Program Manager, Maribeth Senty, Licensing Program Analyst, Kevin Mknelly, Administrator, Anita Heydon (representing licensee A Loving and Joyful Home RCFE LLC). A non-compliance plan was developed with the licensee on today's date as it relates recent compliance history. The licensee was in agreement with the drafted non-compliance plan. Licensee requested a TSP referral. RO to make the referral. No new citations are issued as a result of today's meeting. On 11/13/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit to amend a previous report . Caregivers notified administrator of the visit and Anita Heydon arrived to assist. LPA presented an amended report, written 6/20/24, to dismiss the citation that was issued at that time. During today's visit, LPA observed 4 residents in care with two caregivers. Residents appeared to have their care needs met. As a result of today’s inspection, no deficiencies were noted. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 10/22/24 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met the Administrator who assisted LPA with the inspection. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA observed unsecured cleaning supplies and staff medication that did not pose a risk to residents. Technical violation advisory issued. The home is very clean and residents stated they are happy with care. LPA advised detailed practice of emergency procedures to address resident mobility issues and have a consolidated emergency information binder. LPA reviewed 3 resident files. Files are complete and well organized. Advisory issued for annual review/ participation of residents of their care plans. LPA reviewed 2 staff files. Files are complete. No deficiencies are being cited as a result of todays inspection. On 9/12/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit and met with Caregivers . Administrator , Anita Heydon, notified and arrived to assist On 9/9/24, The department received an incident report regarding R1's hospitalization on 8/28/24 and death while hospitalized on 8/31/24. During todays visit, LPA reviewed and requested copies of R1's records. Records to include LIC 602, centrally stored medication records, medication administration records (including missed/ refused medications), pre-admission records, Resident care/ observation notes , record of physician/ hospital contact, Blood pressure record, and admission agreement. Also requested training records for S1 and S2. LPA interviewed caregivers and Administrator. As a result of today’s inspection, no deficiencies are cited at this time. Report reviewed. On 7/1/24, Licensing Program Analyst (LPA Kevin Mknelly, arrived for a case management visit. LPA notified caregiver of the reason for the visit. Admin was contacted and arrived to assist. LPA is following up on the citation issued on 6/25/24. LPA conducted interviews with 5 of 5 residents and interviewed the Administrator. During interviews it was found that one resident (R1) has a prohibited health condition for which an exception has not been granted by the department. R2 is receiving wound care and Admin found that they exceed allowable stage level.. As a result of this inspection, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care. (B) This poses a potential Health and Safety risk, or personal rights violation, to clients/residents in care. LPA reviewed the report with the Administrator. 87615 Prohibited Health Conditions (a) Persons who require health services for or have a health condition including, but not limited to, those specified below shall not be admitted or retained in a residential care facility for the elderly: (1) Stage 3 and 4 pressure injuries. (2) Gastrostomy tubes. This requirement was not met based on interview and record that found R1 was admitted 6/17/24 with a g-tube. and R2 has greatwer than stage II pressure injuries. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection. LPA met with Ritu Rani, caregiver, who contacted the Administrator, Anita Heydon, by phone. LPA spoke to the Administrator by phone and stated the reason for the inspection. Administrator arrived to the facility shortly. Also present was staff, Roma Devi. LPA was informed there were (5) residents present at the start of the inspection and (1) resident returned from the hospital during today's inspection. During today's inspection, LPA obtained copies of documentation from each resident's file. LPA and Administrator toured the facility and observed (4) residents to be resting in their rooms, (1) resident to be finishing lunch in the kitchen, and (1) resident to be watching television in the common area. Type of visit: Case Management - Deficiencies
**This document was amended on 11/13/24 at 9:00 AM to reflect updated findings following the Department granting an appeal. Some additional information has been added to this report as well as language removed that resident was financially abused by staff. ** Licensing Program Analysts (LPA) Kevin Mknelly arrived unannounced to amend findings, originally delivered 6/20/24, that resulted from a complaint investigation at another of the licensee’s care homes. LPA met with Anita Heydon, and explained purpose of visit. During the course of the Audit investigation, the Department reviewed bank records for accounts belonging to resident (R1), facility records, other records and conducted interviews. The department found that (R1) was always responsible for his own financial and medical decisions and paid the facility with his own checks for rent and reimbursement for items he requested. Although there were some charges made to (R1's) debit card that did not appear to be for (R1), such as gasoline for a vehicle, there was no evidence found that the facility financially abused (R1). Further review indicated that (R1's) rate was not increased at this facility. Based on this information, the citation issued 6/2024 for a deficiency for personal rights violation, was dismissed upon appeal. Exit interview. Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 11/1/23 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met with staff and explained the purpose of the visit. LPA requested for staff to notify Administrator that LPA is present at the facility to conduct an annual inspection. Administrator stated that they would be available by phone if needed arrived to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, 6 resident bedrooms, 3 bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA advised that a new facility sketch be submitted to reflect that Rm 4 is a resident room and Rm 5 is a staff room. LPA reviewed 6 resident files. An advisory was issued to insure LIC 602s are accurate for resident conditions and that LIC625s are done annually, including reflecting resident participation. LPA reviewed 2 staff files. LPA advised that documentation of training contains dates of when training occurred. No deficiencies are being cited as a result of todays inspection.
