Allegation investigated: Facility has pests

On 11/15/23, Licensing Program Analyst Renee Campbell arrived to the facility to deliver findings regarding a complaint. LPA Campbell was met by Paul Hardin, Administrator and Georgia Wilcomb, Licensee and explained the purpose of the visit. Based on observation during the tour of the facility LPA observed bedbugs in the room of R2. The licensee states they are using Terminex to rid the facility inside and out of pest, however, the facility is still active with bedbugs. Based on observation and interviews conducted the preponderance of evidence standards has been met, therefore, the above allegation(s) is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22 Division 6, Chapter 8, deficiencies are being cited on the attached 9099D during this visit. Based on interviews and information gathered during the course of this investigation, it was observed that the facility food supply was sufficient to meet the 2-day perishable and 7-day non perishable food supply requirements at this time. A review of the food items was conducted for the interior food storage units, as well as, the exterior food storage units as well. There were no expired food items observed for the perishable foods nor the non perishable food supply. It was observed that meals were prepared, and offered, for all (3) meals (breakfast, lunch and dinner) throughout the day. Based on interviews, facility residents felt assured that they were receiving their medications by the facility staff who were diligent about handling, dispensing, and documentation in accordance with the law. Licensing Program Analyst (LPA) Victoria Brown and Dana Garcia, Associate Governmental Program Analyst (AGPA) arrived unannounced to conclude a complaint investigation on 10/5/22 at 10:00AM. The team met with Georgia Wilcomb and Paul Steven Hardin, Administrator and stated the purpose of todays visit. The facility is licensed for a capacity of 15 residents. The team requested copies of the Official Pest Prevention Inc. invoices to include description of what type of service was provided. LPA received a copy of the Account Statement Report for dates 1/1/22 - 12/31/22 which indicated the invoice amounts and balances along with specific work that was performed. The team interviewed resident #1 (R1) - (R4) during this visit. The team toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. The team observed there are 3 cats around the perimeter of the facility which are not allowed inside. Unannounced complaint visit made out to this facility on 03/02/2022 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator Paul Steve Hardin. A brief interview was conducted with the facility designated Administrator. Current census was 11 residents. The purpose of this complaint visit was conducted to complete the investigation and present the findings to the facility designated Administrator at this time. Based on a physical plant tour of this facility, it was observed that the facility did have an issue with flies being present. It was observed that flies were present in the common areas as well as the resident rooms and restrooms. Based on a review of the resident bedrooms, it was observed that staff are not maintaining and making sure that the bedrooms were kept clean and free of any debris and pests.
