Licensing Program Analysts (LPAs) Manuel Monter and Marcella Tarin conducted an unannounced complaint investigation visit and met with Residential Living Director Sandy Mirasol. On September 26, 2024, the Department received a complaint alleging a resident was inappropriately touched by a staff. It has been alleged that R1 was inappropriately touched on September 19, 2024. On September 28, 2024, LPA obtained and reviewed Independent, Assisted and Memory Care Unit’s copy of resident and staff rosters including names of residents who were recently, temporarily, and permanently admitted at their Skilled Nursing Facility (SNF) which is under the CA Department of Health. Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced visit to conclude the complaint investigation. LPA Monter met with ADM and stated the purpose of the visit. On 4/4/2023, the Department received a complaint with the above allegation. On 4/11/2023 the Department conducted the initial investigation of financial abuse by private caregiver PC1 and PC2 who misappropriated resident R1’s finances and applies undue influence toward R1 for financial gain. On 4/04/2023, the Department conducted an initial investigation visit to the facility that a resident is being financially abused. LPA and LPM spoke with Executive Director (ED) Bradley Burgoyne. On 05/12/20, the department received a complaint with the allegation that the facility staff will not provide responsible party (RP) with resident’s medical records. On 05/26/20, LPA Larios conducted an initial 10-day investigation. ED was interviewed and LPA requested resident’s documents which facility provided on 05/29/20. Continued on LIC9099-C. LPAs met with facility administrator Bradley Burgoyne (Admin). During the investigation, LPAs toured the facility, interviewed 4 residents, 2 staff, and 2 witnesses, and reviewed resident files. During interviews with facility residents, 0 out of 4 residents interviewed stated that had been financially abused, 0 out of 4 resident stated that they were aware of others experiencing financial abuse at the facility. 2 out of 4 residents interviewed stated that they interacted with caregiving staff on a regular basis and have never experienced untoward conduct. 1 out of 4 stated that they didn't need receive cargiving services but approved of caregiving services. 1 out of 4 stated that they contracted private caregiving services.
