Unannounced complaint visit made out to this facility on 10/26/2022 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Sheri Kimbro, who was briefly interviewed. Current census was 87 residents. The purpose of this visit was to deliver the findings of this complaint investigation to this facility and it's designated Administrator. Based on a review of documents and forms, it was learned that S1 has been employed at this facility since January 2022. A statement submitted by designated personnel representing the corporation for this facility revealed the appointment for S1 as the facility designated Administrator, Executive Director, and contained all of the necessary forms to support this action. A transfer request for the fingerprints and clearance for S1 were included in these documents. This agency has investigated the complaint allegation(s). Unannounced complaint visit made out to this facility on 09/29/2023 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator Sheri Kimbro. A brief interview was conducted with the facility designated Administrator at this time. Current census was residents. Based on interviews conducted during the course of this investigation, it was learned that R1 was prescribed for a medication, Benazepril HCL (Lotensin) 5 mg, which was ordered to be dispensed once daily in a tablet form. It was learned that facility medication technicians dispensed this medication daily to R1 without any issues of the medication being refused, missed, or lost. Based on a review of the Medication Administration Record and personal file for R1, it was observed that this particular medication was initially discontinued by the attending physician for R1. Unannounced complaint visit made out to this facility on 09/29/2023 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator Sheri Kimbro. A brief interview was conducted with the facility designated Administrator at this time. Current census was residents. Based on interviews conducted during the course of this investigation, it was learned that R1 was diagnosed with Type II Diabetes and required a mechanical diet and sugar free meals. It was learned that facility wait staff and kitchen staff were aware of these issues with R1 and prepared meals accordingly and always offered sugar free meal items to R1. Based on a review of the Medication Administration Record and personal file for R1, it was observed that all medications (14 prescribed) and (8) PRN medications were documented on the QuickMAR dispensing log system for this facility. The purpose of this complaint visit was to conclude this investigation and present the findings to the facility designated Administrator. Based on interviews conducted during the course of this investigation, it was learned that residents would often times activate their pendants, or pull cords, for a variety of reasons requiring assistance from the facility staff. It was learned that upon activation, a signal would be sent to the front desk notifying them that a resident required assistance indicating the room number.
