Allegation investigated: Facility is malodorous
On 8/11/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced 10-day complaint visit. LPA met with caregiver, Walaa Elmoraly and explained the purpose of today's visit. Regarding to the allegation of facility is malodorous, the reporting party stated that the facility smells of urine. During today's visit, LPA toured the dining room, kitchen, bathrooms, upstairs and downstairs resident rooms and common area. LPA did not observed urine smell. LPA interviewed the caregiver who stated that no one has expressed any concerns regarding to urine smell. Base on interviews, and observations during the course of investigation, this allegation is unsubstantiated. Although the above investigations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
