Allegation investigated: Licensee did not provide refund to resident's responsible party
Licensing Program Analyst (LPA) Renita Hall conducted an unannounced visit to open a complaint and deliver findings. LPA was allowed entry by Richard Edwards, Licensee. LPA identified herself disclosed the purpose of the visit, and shared findings with the Licensee. On June 12, 2024, the Department received a complaint that the Licensee had not refunded the responsible party after Resident 1 (R1) 's death in May 2024. R1 lived at the facility for 3 days from May 13, 2024, to May 15, 2024, and passed away on May 15, 2024. On June 18, 2024, the Department investigated the above-listed complaint allegation. The investigation consisted of a tour of the facility an interview with the Licensee and a review of records. Licensing Program Analyst (LPA) Iby Strong conducted an unannounced visit to deliver findings on the above complaint allegations. LPA identified herself and discussed the purpose of the visit with Caregiver Ann Nicholas. On July 19, 2023, Community Care Licensing (CCL) received a complaint alleging Resident 1 (R1) was physically abused by Staff 1 (S1), multiple residents were verbally abused by Staff 2 (S2), facility did not have sufficient food for all the residents and facility was not maintained in clean condition. During the investigation, LPA Strong conducted interviews, a facility inspection, and reviewed facility records. According to allegations, on an unidentified date, R1 was hit by S1 causing bruises to mouth and shoulder. Interviews with an outside source revealed that R1 has a continuous mouth sore that appear as if it were a bruise. Outside source also revealed that R1 tends to flail in their wheelchair causing bruises throughout their body.
