Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced complaint visit to deliver investigative findings regarding the above allegation. LPA identified herself and was granted entry by Edward Alozie, Licensee. LPA stated the purpose of the visit and reviewed the findings of the complaint with licensee Alozie. The Department’s investigation consisted of interviews with staff, residents, and outside sources, records review of relevant documents pertinent to this investigation, and LPA observations. On September 2, 2025, it was alleged that staff pushed a resident while in care. Upon review of resident #1’s (R1) records, the Physician’s Report (LIC602) states that R1 has an underlying medical condition which AI describes as a progressive form of memory loss caused by damage to the brain’s blood vessels. Licensing Program Analyst (LPA) Dawn Segura conducted an unannounced visit to deliver investigative findings. LPA was granted entry into the facility by Edward Alozie, Licensee, to whom she disclosed the purpose of the visit. Community Care Licensing (CCL) has investigated the above listed complaint allegation. The investigation consisted of a tour of the facility and interviews of residents and staff. It was alleged that a resident in care was not accorded dignity in their personal relationships with staff. It was reported that licensee and staff were yelling at each other in the presence of Resident 1 (R1) and other residents in care and that staff yelled at R1 directly. During the course of the investigation, it was determined that the licensee and his spouse resided and worked in the facility. Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced visit to deliver findings regarding the above-mentioned allegations. LPA met with LIcensee Alozie identified herself, and stated the purpose of the visit. The Department’s investigation consisted of outside source and staff interviews, and a facility tour. LPA also secured and reviewed pertinent records. It was alleged Resident1 had unexplained injuries. A record review revealed R1 was recently admitted on On Septmeber 16, 2021, at discharge from a hospital stay, during which R1 had experienced a seizure. R1 has a diagnosis of Dementia with Delusions and an Epilepsy Disorder. Interviews with facility staff revealed, on October 23, 2021, the Administrator (S1) was in the kitchen and R1 was sitting in the chair in the living room.
