Licensing Program Analyst (LPA) Amy Domingo conducted an unannounced visit to deliver findings regarding the above complaint allegation. LPA introduced themselves and disclosed the purpose of the visit to Natalie Bonds, Administrator. On 3/14/24, it was alleged lack of supervision, resulting in resident self-neglect. The Department’s investigation consisted of unannounced facility visits, interviews with facility staff, residents, outside sources, and records review. Licensing Program Analyst (LPA) Domingo conducted an interview with Resident 1 (R1), who willingly participated. During the interview, R1 demonstrated cognitive awareness by accurately stating the current year, the name of the current president, their date of birth, and their current address. Licensing Program Manager (LPM) Simon Jacob and Licensing Program Analyst (LPA) Alyssa Ramirez conducted an unannounced visit to conclude the investigation that was initiated on 9/7/23 and issue findings. LPM and LPA stated the purpose of the visit and was granted entry by Caregiver Mary Lou Diaz and met with Licensee’s Todd & Jean Brooks It was alleged that the facility did not arrange or provide transportation to medical appointments for Resident 1 (R1) and that staff confiscated R1’s television. The department’s investigation included facility tour, a review of facility records, and interviews with facility staff and residents. Records review revealed that R1’s Admissions Agreement states that responsible persons may and are encouraged to provide basic transportation. Facility may provide resident with transportation when responsible person or an alternative arrangement is unavailable. This is an amended report originally signed on 5/18/2023. Licensing Program Analyst (LPA), Sabel Martinez, conducted an unannounced complaint investigation visit to deliver findings. The LPA introduced himself and disclosed the purpose of the visit to Caregiver Marilyn Ancho. Administrator Todd Brooks arrived during the visit. Throughout the investigation, the Department secured pertinent records and conducted interviews with internal and external sources. It was alleged staff did not provide incontinence care in a timely manner. It was reported to the Department staff did not assist residents and residents had to wait up to 45 minutes to be assisted with incontinent care. Interviews with external and internal sources revealed there had been occasions where Staff #1 (S1) had not responded to residents’ requests for assistance with incontinence care. This is an amended report to a report signed on 3/22/23. Licensing Program Analyst (LPA), Sabel Martinez, conducted an unannounced complaint investigation visit to deliver findings. The LPA introduced himself and disclosed the purpose of the visit with Administrator, Todd Brooks, and Caregiver, Marilyn Ancho. Staff, Jean Brooks arrived during the visit. Throughout the investigation, the Department secured pertinent records and conducted interviews with internal and external sources. It was alleged staff did not treat residents with dignity. Interviews with internal and external sources corroborated multiple staff had raised, or yelled at residents, and staff had been witnessed to ignore the residents. Based on evidence obtained, the allegation was Substantiated. The deficiency was cited in accordance with California Code of Regulations, Title 22, and listed on the LIC 9099D.
