Licensing Program Analyst (LPA) Amy Rodgers conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA Rodgers was welcomed by, identified herself to, and discussed the purpose of the visit with Caregiver Natalie Bond. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory, two (2) can be under Hospice Care, and one (1) resident may be bedridden. During today's visit all six (6) Residents were present. LPA, accompanied by licensee’s staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows and screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility’s ambient internal temperature was compliant. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water were observed on the premises. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA reviewed multiple staff and client records/files and did not raise any significant licensing concerns. No deficiencies were observed or cited during today's annual inspection. Allegation investigated: Licensee failed to protect resident from self-neglect.

Licensing Program Analyst (LPA) Amy Domingo conducted an unannounced visit to deliver the findings in the above-mentioned complaint allegations. LPA Domingo identified herself and discussed the purpose of the visit with Todd Brooks, Administrator. The Department’s investigation consisted of unannounced facility visits, interviews with facility staff, residents, outside sources, and records review. On 10/21/25, it was alleged that the licensee failed to protect the resident from self-neglect. Records reviewed revealed the resident’s needs and service plan were current and included interventions addressing behaviors and conditions that could lead to self-neglect. Unsubstantiated (Continued from LIC9099) The records review also incorporated the Appraisal and Reappraisal Records: Assessments were conducted and documented, with no indication of unmet needs or deterioration due to self-neglect, as R1 would go to the hospital when needed. The files incorporated progress notes that verified staff documented consistent monitoring and interventions, including reminders, assistance with ADLs, and communication with healthcare providers. LPA Domingo observed R1's condition during the visit and the resident appeared appropriately groomed, nourished, and safe in their environment. The resident’s living area was clean and free of hazards, with no signs of neglect or unsafe conditions. LPA interviewed two (2) staff members and they reported awareness of the resident’s needs and described appropriate interventions to support the resident’s well-being. Resident 1 (R1) was interviewed and expressed satisfaction with care and did not report being left without assistance or support. Three (3) outside sources were interviewed and confirmed that the facility communicates regularly and addresses concerns promptly. The Department has investigated a complaint with the above allegations. The Department has determined there is not a preponderance of evidence to prove that the alleged violation occurred; therefore, the allegations are unsubstantiated. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory, two (2) can be under Hospice Care, and one (1) resident may be bedridden. During today's visit all six (6) Residents were present. LPA, accompanied by licensee’s staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows and screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. Hot water temperature at taps accessible to clients were all compliant: Kitchen sink was 112 F, Bathroom #1 sink was 112 F, and Bathroom #2 sink was 111 F. Refrigerator temperature was 41 F and freezer temperature was -1 F. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters accessible to clients. Per the licensee's staff, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPAs interviewed multiple staff and clients. LPA Domingo reviewed multiple staff and client records/files. The interviews did not raise any significant licensing concerns. The reviewed files contained required documents. Confidential records were stored in locked areas. Licensee's staff also presented proof of current/active business liability insurance and surety bond. No deficiencies were observed or cited during today's annual inspection. Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conduct an annual licensing inspection. LPA identified herself to Administrator Natalie Bond and was granted entry into the facility. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory, two (2) can be under Hospice Care, and one (1) resident may be bedridden. During today's visit all six (6) Residents were present. During today’s visit, LPA Correia, accompanied by Administrator Bond, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment (PPE). Hot water temperature at taps accessible to residents in care were in regulation measuring, and the facility temperature was 78 F. There was at least 2 days of perishable food, and at least seven (7) days non-perishable food present, all safely stored and labeled. Smoke and carbon monoxide detectors, as well as a fire extinguisher were all present and operable, and up to date. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to residents. Medications and toxins are stored in locked areas inaccessible to residents in care. Per Administrator Bond there are no firearms or weapons on the facility grounds. There were no bodies of water on the premises. Resident and staff records were available and complete. Administrator Bond's certification is current until July 2,2025. No deficiencies cited during today's visit. This report was discussed with Licensee Brooks, who joined the inspection at a later time.
