Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Natasha Persaud conducted an unannounced Required Annual Inspection. LPA was greeted and allowed entry into the facility by Administrator, Natalie Bond. Administrator, Todd Brooks arrived during the visit. LPA, accompanied by licensee’s staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows and screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. Hot water temperature at taps accessible to clients measured at 114 F. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water were observed on the premises. Per the licensee's staff, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher was were serviced within the last 12 months. First aid kits were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA reviewed multiple staff and client records/files. The reviewed files contained required documents. Confidential records were stored in locked areas. No deficiencies were observed or cited during today's annual inspection. However, due to time restraint records were not reviewed. A continuation visit will be conducted at a later date for record review only. May 7, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Arian Golbakhsh conducted an unannounced, required Annual Inspection. The facility file and personnel report was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Administrator Natalie Bond. Co-Administrator Todd Brooks arrived later during the visit. The facility's license shows a capacity of six (6) non-ambulatory residents, one (1) of which may be bedridden. Bedridden resident may reside in bedroom #1. The facility additionally has a hospice waiver for two (2) residents with total care addendum. During today's inspection there were six (6) residents in care. LPA and Administrator Bond toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Hot water temperature at taps accessible to clients were all compliant: common bathroom sink was 119F. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least two (2) days of perishable food, and at least seven (7) days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. However, LPA observed that knives were stored in an unsecured cabinet, accessible to residents in care. A Deficiency was cited per California Code of Regulations (CCR) 87309(a). [Continued on LIC 809-C] [Continued from LIC 809] No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water exist on the premises. Per Administrator Bond, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguisher was serviced within the last 12 months. First aid kit was incomplete and LPA provided a Technical Violation (TV). Required licensing postings were observed in visible areas of the facility. LPA interviewed one (1) staff and two (2) clients, and interviews did not reveal any licensing or regulatory concerns. LPA reviewed facility records. The files reviewed by LPA contained required documents. Confidential records were stored in locked areas. LPA discussed with Administrator Bond about the Technical Support Program offered by the Department, and created a referral for the facility. A Deficiency and a Technical Violation (TV) were cited during the inspection. An exit interview was conducted with Administrator Bond to whom a copy of this report, the TV form, and the Licensee/Appeal Rights (LIC 9058) were provided. Jul 22, 2024Complaint investigation reportUnsubstantiated
Allegation investigated: Staff sexually abused a resident while in care Staff did not provide a safe environment
Licensing Program Analyst (LPA), Sabel Martinez, conducted an unannounced follow up complaint investigation visit, and delivered complaint findings. The LPA introduced himself and disclosed the purpose of the visit to Administrator Natalie Bond. Throughout the investigation, the Department secured records and conducted interviews with external and internal sources, including residents and staff. It was alleged staff sexually abused a resident while in care. On 02/21/2024, it was reported to the Department a caregiver had kissed Resident # 1 (R1) in the lips. The initial report did not disclose a time frame, only that it had occurred at night, and the name of the suspected abuser was not provided. An interview with an external source revealed the name of the Suspected Abuser (SA), noted the incident had occurred approximately one year prior, during the night shift. The Department interviewed several staff who did not recall any concerning interactions between staff and R1. Unsubstantiated Staff did recall a former resident would enter R1’s room occasionally during the day, but there was no concerning interactions between that resident and R1. Review of facility records revealed R1 was a seventy-four (74) year-old resident diagnosed with several comorbidities, including Mild Cognitive Impairment. An interview with R1 did not yield any evidence that corroborated the incident occurred. When asked If anyone had kissed, or touched R1 inappropriately, R1 said no. Furthermore, several attempts were made to contact the SA, but all attempts were unsuccessful. It was also alleged staff did not provide a safe environment. On 02/21/2024, it was reported to the Department R1’s roommate had been verbally and physically aggressive toward R1, and staff had not addressed this concern. Interviews with several sources, including staff, confirmed R1 and R1’s roommate sometimes negatively engaged each other. These behaviors included arguing and verbal aggression towards each other. Staff had placed a barrier between each bed and had begun to redirect them away from each other. The facility was at full capacity; therefore, a room change was not available at the time. When interviewed, R1 did not recall anyone at the facility, including R1’s roommate, grabbing, nor hurting R1. Based on the evidence obtained, there was not enough evidence to prove the alleged violations occurred, therefore, the allegations were Unsubstantiated. The facility was licensed for a capacity of six (6) non-ambulatory residents, of which one (1) may be bedridden in room # 1. The facility had a hospice waiver for two (2) residents. During the visit the LPA reviewed resident and staff records. A carbon monoxide detector, fire extinguisher, and telephone were present and operational. Per staff, fire rms, nor ammunition were kept at the facility. The facility was clean, sanitary and in good repair. Pathways were free of obstructions and slip hazards. Resident bedrooms contained the required furnishings. Due to time constraints, a return visit on a subsequent day is needed to complete the annual inspection. The facility was licensed for a capacity of six (6) non-ambulatory residents, of which one (1) may be bedridden in room # 1. The facility had a hospice waiver for two (2) residents. The LPA, accompanied by staff, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. Allegation investigated: Staff did not safeguard resident’s personal belongings
Licensing Program Analyst (LPA), Sabel Martinez, conducted an unannounced follow up complaint investigation visit, and delivered complaint findings. The LPA introduced himself and disclosed the purpose of the visit to Adminstrator Mariaelena Villa. Throughout the investigation, the Department secured records and conducted interviews with external and internal sources, including staff and residents. It was alleged staff did not safeguard a resident’s personal belongings. On 3/6/2023, it was reported to the Department staff were not safeguarding a resident’s personal belongings, including hygiene supplies, food, clothing, electronics, and jewelry. The Department was able to contact one of the staff who allegedly was involved in the items going missing. This staff denied taking any of the items in question. The Department attempted multiple contacts with an additional staff who may allegedly removed items, but these attempts were not successful. (See LIC 9099C for continuation of report.) Unsubstantiated Interviews with both internal and external sources did not report any concerns with personal belongings going missing and some interviews reported the resident may have misplaced some of the items. Although some of the interviews confirmed the resident had reported some items missing to facility staff, there was no evidence to prove the items were missing due to staff not safeguarding the items.
