Allegation investigated: Facility staff handled resident in a rough manner

On 3/23/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced visit to the facility for the purpose of delivering investigative finding into the allegation listed on the complaint report. LPA Flores met with Administrator Nikkita Mundhada and explained the purpose of the visit. Information received alleged Staff #1 (S1) handled Resident #1 (R1) in a rough manner. Interview with (3) three staff report that R1 was very difficult to work with often refusing care assistance. Staff report that R1 was temporarily restricted to a wheelchair and went against physician orders by attempting to ambulate independently. Staff recall an interaction where R1 was sliding off of the bed and S1 assisted R1 by placing R1’s legs back onto the bed. Staff report that R1’s responsible persons were present to the incident and can account that S1 did not handle R1 in a rough manner. Allegation investigated: Staff did not treat resident with dignity and respect

On 8/25/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced visit to the facility to investigate the allegation listed above. LPA Flores introduced herself to House Manager, Alexa Cardenas, and explained the purpose of the visit. Alexa granted LPA Flores entry, and a tour of the facility was conducted. LPA Flores did not observe any health and safety concerns. On 8/18/2025, Community Care Licensing (CCL) received a complaint alleging staff did not treat residents with dignity and respected. Information received alleged Resident #1 (R1) was called inappropriate names by Staff #1 (S1). During the time of visit, there were (2) two staff, (5) five residents, and the Administrator present. LPA Flores conducted record review, obtained copies of pertinent documents, interviewed staff and residents. Interviews conducted could not corroborate that the allegation occurred. Licensing Program Analyst (LPA) Kathleen Banrasavong conducted an unannounced visit to deliver findings for a complaint investigation regarding the above allegation. LPA met with Administrator, Nikita Mundhada, where the LPA explained the purpose of the visit and the elements of the allegation. The investigation consisted of observations, interviews with staff members and residents, and a review of records. On December 6th, 2024, Community Care Licensing received a complaint alleging that facility staff do not treat residents with dignity and respect. The complaint alleged that Resident 1 (R1) had their feelings hurt by staff. It was reported that R1 was upset with staff because staff was trying to limit his soda consumption due to R1’s health issues. Information obtained from interview with Administrator stated that the facility does not purchase soda for clients. It was stated that R1 personally purchases their drinks, which R1 has the right to. LPA met with Administrator, Nikita Mundhada, where LPA explained the purpose of the visit and the elements of the allegations. The investigation consisted of observations, interviews with staff members and residents, and a review of records. On August 28th, 2024, Community Care Licensing received a complaint alleging staff does not provide an adequate amount of food to residents. It was reported that staff provided little breakfast in the morning. Information obtained from Administrator indicated that the facility provides three meals a day to residents and residents are able to receive additional servings. LPA interviewed Resident 1 (R1), who indicated that they are provided three meals a day; no concerns.
