Licensing Program Analyst (LPA) Becky Mann conducted an unannounced visit to the facility to initiate a complaint investigation. LPA met with Breeana Redwine, Administrator and discussed the purpose of the visit. The investigation consisted of LPA pertinent record reviews and interviews with staff and residents. The allegation that Residents are equipped with catheters when not required to be. Based on interviews with administrator, staff, and outside hospice agency, all have denied this allegation. Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conclude a complaint investigation regarding an allegation that the facility did not provide a refund upon resident’s death. LPA met with staff Breeana Redwine. LPA interviewed licensee Kamal Ebadpour and witness. Interviews concluded that a refund was provided. It was later negotiated for a sum that both facility representative and witness agreed upon. Licensing Program Analyst (LPA) Stephanie Williams made an unannounced visit to the facility in order to deliver findings for the above allegations. LPA Williams identified herself to Staff Member, Breeana Redwine, who was also informed of the purpose of the visit. LPA Williams spoke with Administrator, Kamal Ebadpour, over the phone. The investigation consisted of direct observations, records review, and interviews with staff and residents. In regards to allegation #1, LPA Williams interviewed Staff #1 (S1) who denied witnessing or having knowledge of Staff #2 (S2) physically abusing Resident #1 (R1). LPA Williams interviewed S2 who denied physically abusing R1. LPA Williams interviewed Staff #3 (S3) who also denied witnessing or having knowledge of S2 physically abusing the residents.
