On September 18, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to deliver the findings of the allegations. LPA met with and explained the purpose of the visit. During the course of the investigation, LPA reviewed staff records, facility records, and conducted interviews.
