LPA met with Administrator, Nellie Johnson, and explained the purpose of the visit. During the course of investigation, the Department interviewed facility staff, residents in care, and obtained pertinent documents relevant to the complaint investigation such as, resident’s (R1) physician’s report, appraisal/needs and services plan, identification and emergency information, medication list, physician’s orders, medication administration records (MAR), staff trainings, fire clearance, employee roster, and resident roster.
