On October 8, 2025, at approximately 8:30 AM, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced follow-up visit regarding a complaint. During the visit, LPA Richard met with staff member Orlando Trajico. LPA explained the purpose of the visit. Later, LPA Richard was joined by the Administrator, Cheryl Dela Cruz. The investigation included the following actions: The LPA conducted interviews with the Administrator (A1) and two staff members (S1 and S2), one resident( R2) Additionally, the LPA interviewed the Placement Agency, Innovage (PAI). The LPA obtained and reviewed the Resident Roster and the Staff Roster dated 07/01/25. The LPA attempted to obtain and review the resident records for Resident R1, specifically the Face Sheet, Admission Agreement, Physician’s Report, or LIC 602A. Licensing Program Analyst (LPA), Stephanie Torres, conducted an unannounced visit to the facility to deliver the findings of the investigation into the above allegation. The LPA met with Administrator, Cheryl De La Cruz, and informed her of the purpose of her visit. The LPA started the investigation on October 03, 2022; staff/resident interviews were conducted, records reviewed, copies obtained, and a tour conducted. It was alleged Resident One (R1) developed a urinary tract infection (UTI) due to inadequate care. Interviews revealed R1 was not hospitalized while residing at the facility and did not develop any other conditions other than a rash on the genitals. Interviews reported, the facility did not provide sufficient care regarding incontinence services. A record review was conducted; no medical assessment (Physician's Report) was observed on file. Documentation was found on file detailing logs for incontinence assistance provided by staff. Licensing Progran Analyst (LPA) Jennifer Semin conducted an unannounced visit to deliver the findings for the above compalint allegations. LPA mer with Sonia De La Cruz. The investigation consisted of interviews with staff, resident and relevant parties. Regarding the first allegation, Lack of supervision resulting in resident wandering from the facility. Facility staff stated Resident 1 (R1) is independent and able to leave the facility unassisted. R1 stated R1 is independent and leaves the facility to visit with friends and go shopping. Relevant parties stated R1 is independent to leave the facility unassisted. The second allegation, Facility staff failed to ensure resident was receiving medical treatments.
